The heart could be subjected to damage by violent chest trauma, and it could lead to myocardial contusion, septal and valve damage, hemopericardium, cardiac rupture, coronary thromboembolism, myocardial infarction, ventricular aneurysm. Though the incidence of heart injury in multiple or blunt chest trauma cases is high, it could be overlooked due to difficulty in diagnosis and masked symptoms. Therefore, careful evaluation is important for proper management. We experienced a case of traumatic ventricular aneurysm which developed due to a fall in healthy young male, without the evidence of coronary obstruction in the cardiac catheterization and coronary angiogram. We thought that traumatic ventricular aneurysm was refractory to medical therapy, so surgical management is necessary to prevent fatal complications: rupture, thrombosis, arrythmia and heart failure.
