본 연구는 우리나라의 건강보험 거버넌스 개혁을 위한 시사점을 얻고자 매우 유사한 건강보험제도를 운영하고 있는 동아시아 국가(한국, 대만, 일본)의 건강보험 의사결정구조를 비교했다. 이를 위해, 건강보험제도의 핵심영역이라 할 수 있는 관리감독‧운영, 보험료부과‧징수, 급여·수가결정, 진료비심사·평가 영역을 중심으로 주요 행위자(actor)들의 지위 및 기능과 역할관계를 분석했다. 분석결과, 3개국 모두 보험료납부자(가입자, 사용자), 서비스공급자(의약계), 관리운영자(정부, 보험자), 공익대표(전문가, 시민사회단체)가 참여하는 사회적 합의기구를 설치하여 운영하고 있었다. 일본과 대만의 경우, 모든 의사결정과정에서 사회적 합의기구가 최종 결정권자로 기능하지는 않았다. 참여자의 논의와 심의과정을 통해 이해관계자들의 의견을 수렴하여 최종 의사결정권자에게 자문을 제공하기도 하고, 때로는 의결권을 가지기도 했다. 이에 비해, 한국은 보험자의 재정운영에 대한 권한은 상당히 제한적인 반면, ‘건강보험정책심의위원회’를 중심으로 의사결정권한이 집중화되고 인적구성은 획일화되어 있었다. 따라서 우리나라도 각 영역별 특성을 고려하여 주요 행위자를 중심으로 의사결정과정 참여자를 다원화하고, 재정안정화를 위해 통합적 관리운영체계의 강점을 극대화하여 재정책임자인 보험자의 권한 및 책임을 강화할 필요가 있다. This study compared the health insurance decision-making structures of East Asian countries that operate very similar health insurance systems in order to gain implications for the reform of health insurance governance in Korea. To this end, the status and function and role relationship of the main actors were analyzed focusing on the areas of management and supervision, contribution charging and collection, benefits and fee schedules determination, medical expense review and assessment, which are the key areas of the health insurance system. As a result of the analysis, all 3 countries operated social consensus bodies in which the insured, service providers, management organizations, and representatives of the public interest participated. In the case of Japan and Taiwan, the social consensus body did not function as the final decision-maker in all decision-making processes. In comparison, in South Korea, the authority of insurers to manage their finances was quite limited, while decision-making power was concentrated around the Health Insurance Policy Review Committee. Therefore, it is necessary to diversify the participants in the decision-making process around the main actors, taking into account the characteristics of each area, and maximize the strength of the integrated management and operating system for financial stability, thereby strengthening the authority and responsibility of the insurer who is the financial officer.

외국인은 일반적으로 의료이용에 어려움을 겪고 있는 것으로 알려져 있지만, 한편으로는 외국인의 건강보험 부당이용이나 과다이용이 사회적 문제가 되기도 하였다. 그 동안 우리나라에서 외국인의 의료이용에 대한 실증적 탐색과 연구가 충분하지 않았다. 이에, 본 연구는 최근 5년간(2013-2017년) 국민건강보험에 가입한 외국인 전수자료를 분석하여 이들의 의료이용 양상과 특징을 고찰했다. 그 결과, 외국인 건강보험 가입자는 내국인 가입자에 비해 의료 미이용자 비율이 높고, 1인당 입내원일수와 1인당 진료비 등 의료이용이 적었다. 다만, 외국인은 입원 일당 진료비가 내국인보다 다소 높게 나타나 중증의 case mix를 가질 가능성이 있다. Foreigners are known to have various difficulties in accessing medical services in their resident countries. On the other hand, in Korea, illegal or over-utilization of NHIS by a few of foreigners by foreigners became a social problem. However, there was not enough empirical researches for their medical utilization of NHIS foreign subscribers. Thus, this study looked at the pattern and characteristics of medical use of NHIS foreign subscribers through the data of all NHIS subscribed foreigners over the past five years (2013-2017). Foreign health insurance subscribers in Korea have a higher proportion of non-users of medical insurance compared to Korean subscribers, and their overall use of medical care was low, including the number of days per person and cost for medical treatment per person. However, considering the high cost of hospitalization, it is possible that the foreigners have had more serious case mix than Koreans.
