        흡수성 고정판과 나사를 이용한 중수골 골절의 치료

        Purpose: Metacarpal fractures are common hand injury that may require operative intervention to ensure adequate reduction and stabilization. Traditionally, titanium miniplate was used for rigid fixation of bone fractures. However, the use of permanent plate lends itself to multiple complications such as infection, exposure of the hardware, tendon adhesions, tendon rupture, prolonged pain, bony atrophy and osteoporosis (stress shielding), metal sensitization, and palpation under the skin. This study evaluated the usefulness and stability of biodegradable plates and screws for treatment of metacarpal bone fractures. Methods: There was 17 patients who had surgery for metacarpal bone fracture from April 2007 to June 2010. All patients had open reduction and internal fixation. We used absorbable plates and screws (Inion CPS^®) for internal fixation. Postoperative results were assessed with x-ray. Stability of plates and screws, healing process and its complications were observed by clinical and radiographic assessment. Results: All patients were successfully reduced of bone fracture, and fixations with absorbable plates and screws were stable. The mean follow up period was 7.1 months. 2 patients complained postoperative pain, but they were relieved with analgesics. All patients experienced transient stiffness, but they were relieved with active assistive range of motion after removal of splint. No patients suffered complications which could be occurred by using metallic plate. Conclusion: There was no critical complications such as re-fracture or nonunion among patients. No patients suffered side effects related with metallic implants. Biodegradable implants can offer clinically stable and attractive alternative to metallic implants to stabilize metacarpal bone fractures in the hand.

        Unilateral pedicled transverse rectus abdominis musculocutaneous flap and unilateral free deep inferior epigastric artery perforator flap as a surgical alternative in bilateral autologous breast reconstruction

        Background Bilateral microsurgical autologous reconstruction is known to increase operating time, costs, and complications compared to unilateral procedures. This study aimed to determine whether a unilateral pedicled transverse rectus abdominis myocutaneous (TRAM) flap and a unilateral deep inferior epigastric artery perforator (DIEP) free flap could be a feasible option for bilateral reconstruction in selected circumstances. Methods A retrospective chart review identified patients who underwent unilateral pedicled TRAM and unilateral DIEP reconstruction for bilateral breast reconstruction between 2011 and 2014. Surgical outcomes, complications, and aesthetic scale questionnaire responses were evaluated. Results Fourteen patients were included in this study. Ten patients received bilateral immediate reconstruction, while four patients with a previous history of mastectomy underwent unilateral immediate reconstruction and contralateral delayed reconstruction. All flaps survived without any major complications. A case of nipple-areolar skin necrosis on the pedicled TRAM side and a case of mild abdominal bulging at the free DIEP donor site were reported. There was no partial flap necrosis or palpable fat necrosis. On the aesthetic outcome scale, the free DIEP flaps scored significantly higher than did the pedicled TRAM flaps for overall shape, the upper medial and lower lateral quadrant, and the lateral chest wall. Conclusions Our findings suggest that a unilateral pedicled TRAM flap together with a unilateral free DIEP flap could be performed as a bridging surgical option as institutions move toward bilateral free-flap reconstructions, as a way to reduce operating time and the risk of microsurgery-related complications with acceptable donor site morbidity and aesthetic outcomes.

        The Resolver usually used in industry is the absolute angle analog sensor that must be in order driving BLDC (brushless DC) motor, and it needs RDC(Resolver-to-Digital converter) for changing the output signal to digital to be applied to the SVPWM(Space Vector Pulse Width Modulation) algorithm. Commonly used S/W RDC needs trigonometric function. What it takes a lot of calculation time of processor is gotten at weak point. In this paper, S/W RDC is realized except trigonometric functions as a result of feedback resolver outputs after filtering using FIR filter. thus, processing time is reduced. So. One-chip DSP Controller operating the Vector Control, RDC, and SVPWM can be designed.

        다층신경망을 이용한 전단모드 회전형 MR 댐퍼의 모델링

        자기변성유체(magnetorheological fluid)에 관한 연구는 MR 장치의 개발, MR 장치의 수학적 모델링 및 시뮬레이션, 그리고 MR 장치를 채용한 시스템의 제어 알고리즘 개발에 관한 연구로 구분된다. 시뮬레이션을 통한 제어 알고리즘 개발을 위해서는 MR 장치의 비선형 응답을 예상하기 위한 신뢰성 높은 수학적 모델이 요구된다. 또한 MR 장치 시스템을 제어하기 위해서는 제어기에서 요구하는 댐핑력을 출력하기 위한 MR 장치의 전류(또는 전압) 입력 값이 필요하며, 이 입력값을 얻기 위해서는 역댐퍼 모델이 필요하다. 이러한 이유로 MR 장치의 모델링 및 역댐퍼 모델링은 MR 장치개발의 중요한 역할을 담당하며 이에 관한 많은 연구가 요구되고 있다. 본 연구에서는 전단모드 회전형 MR 댐퍼의 모델링을 위해 개발된 MR 댐퍼를 이용하여 동특성 시험기를 제작하였으며, 전단모드 회전형 MR 댐퍼의 특성을 연구하기 위한 실험을 수행하였다. 시험기 시험결과를 통해 모델링에 필요한 시험 데이터들을 획득하였으며 다층신경망을 이용하여 전단모드 회전형 MR 댐퍼의 모델 및 역모델을 구하였다. Scientific challenges in the field of MR(magnetorheological) fluids and devices consist in the development of MR devices, the mathematical modeling and simulation of MR devices, and the development of (optimal) control algorithm for MR device systems. To take a maximum advantage of MR fluids in control applications a reliable mathematical model, which predicts their nonlinear characteristics, is needed. A inverse model of the MR device is required to calculate current(or voltage) input of MR damper, which generates required damping force. In this paper, we implemented test a bench for shear mode rotary MR damper and laboratory tests were performed to study the characteristics of the prototype shear-mode rotary MR damper. The direct identification and inverse dynamics modeling for shear mode rotary MR dampers using multi-layer neural networks are studied.

        회전형 MR 댐퍼를 이용한 반능동 현가장치의 퍼지 스카이-훅 제어

        최근 고층건물의 방진시스템 및 자동차의 현가장치 분야에 MR 유체를 이용한 반능동 진동제어에 관한 연구가 활발히 진행되면서 MR 유체의 밸브모드 특성을 이용한 선형 MR 댐퍼가 널리 사용되고 있다. 그러나 전단모드를 이용한 회전형 MR 댐퍼에 관한 연구는 초기단계에 머물고 있다. 본 연구에서는, 주행로봇의 무한회전형 액츄에이터용 회전형 댐퍼의 진동제어에 관한 연구를 수행하였으며, 퍼지 로직을 기반으로한 회전형 MR 댐퍼용 진동제어 알고리즘을 제안하였다. 퍼지제어기의 입출력 이득과 멤버쉽 함수의 모양은 유전자 알고리즘을 이용하여 조정하였다. 시뮬레이션 결과는 MR 댐퍼에 충격력이 가해질 때 퍼지 스카이-훅 제어 성능을 보여준다. Recently, a number of researches about linear magnetorheological(MR) damper using valve-mode characteristics of MR fluid have sufficiently undertaken, but researches about rotary MR damper using shear-mode characteristics of MR fluid are not enough. In this paper, we performed vibration control of shear-mode MR damper for unlimited rotating actuator of mobile robot. Also fuzzy logic based vibration control for shear-mode MR damper is suggested. The parameters, like scaling factor of input/output and center of the triangular membership functions associated with the different Linguistic variables, are tuned by genetic algorithm. Simulation results demonstrate the effectiveness of the fuzzy-skyhook controller for vibration control of shear-mode MR damper under impact force.

        Skin-sparing Mastectomy and Immediate Nipple Graft for Large, Ptotic Breast

        Nipple-areolar skin-sparing mastectomy followed by autologous reconstruction in patients with large, ptotic breasts often offers a limited field, resulting in strenuous traction. Skin-sparing mastectomy (SSM) with immediate nipple grafting from the specimen was attempted for such patients. Patients who underwent SSM with immediate autologous breast reconstruction and nipple grafting between September 2016 and February 2019 were evaluated, including 33 nipple grafts in 30 patients. The average weight of the mastectomy specimen was 552.5 g and the average operation time for unilateral mastectomy was 109 minutes. No complete nipple loss or major skin flap necrosis was reported. Adjuvant therapy started after an average of 24 days. SSM with immediate nipple grafting on the autologously reconstructed breast could be an alternative for large, ptotic breasts. It is also useful for patients requiring contralateral balancing procedures or those with bilateral breast cancer in which only one nipple can be spared oncologically.

        Volumetric changes of the pedicled transverse rectus abdominis musculocutaneous flap and the contralateral native breast during long-term follow-up

        Background Serial volumetric changes of reconstructed breasts have not been studied in detail. In this study, we analyzed serial volumetric changes of reconstructed and contralateral normal breasts during long-term follow-up, with a focus on the effect of various adjuvant therapies. Methods Among all patients who underwent immediate breast reconstruction with a unilateral pedicled transverse rectus abdominis musculocutaneous (p-TRAM) flap, 42 patients with valid data from ≥3 postoperative positron emission tomography-computed tomography (PET-CT) scans were included. The volumes of the reconstructed and normal breasts were measured, and the ratio of flap volume to that of the contralateral breast was calculated. Serial changes in volume and the volume ratio were described, and the effects of chemotherapy, radiation therapy, and hormone therapy on volumetric changes were analyzed. Results The mean interval between the initial reconstruction and each PET-CT scan was 16.5, 30, and 51 months respectively. Thirty-five, 36, and 10 patients received chemotherapy, hormone therapy, and radiation therapy, respectively. The flap volume at each measurement was 531.0, 539.6, and 538.0 cm3, and the contralateral breast volume was 472.8, 486.4, and 500.8 cm3, respectively. The volume ratio decreased from 115.1% to 113.4%, and finally to 109.6% (P=0.02). Adjuvant therapies showed no significant effects. Conclusions We demonstrated that the p-TRAM flap maintained its volume over a long-term follow up, while the volume of the contralateral native breast slowly increased. Moreover, adjuvant breast cancer therapies had no statistically significant effects on the volume of the reconstructed p-TRAM flaps or the contralateral native breasts.
