The purpose of this study was to investigate the effects of apply to home based exercise program that consisted of lower extremity exercise on isokinetic knee strength in elderly at nursing home. Thirty eight subjects were participated in current study and divided two groups that home exercise program group and control group (home exercise group 76.83±4.06 years, control group 78.82±3.00). The subject’s isokinetic knee strength was measured by Biodex isokinetic system 4 (Biodex medical. U.S.A) at pre-intervention, post-intervention. We measured muscle’s peak torque, endurance and ratio(H/Q ratio). Home exercise program group has receive lower extremity exercise program that consisted of 6 exercises for 5 weeks. Control group has no intervention. The home exercise program group has significant change of muscle’s peak torque and endurance after applying intervention. But muscle ratio has no significant change both groups. On basis of these results, we concluded that home based exercise program that consised of lower extremity exercise has improved isokinteic knee strength in elderly at nursing home.

Throughout his life Herman Melville was fascinated by the question of how to regard his fellow man. Allen Schwab says Melville spent a lifetime fascinated with the contradictions and ambiguities which inhere in the condition of being human. Melville's "Bartleby, the Scrivener" is also a probe into the fundamental conditions of human existence. Bartleby is the central character in the story but it would not be a completely fair interpretation without mentioning the relations between Bartleby and the narrator. They represent two aspects of one self. Melville's propensity to think in polar terms is innate. This polarity refers to the presence of opposites understood to be mutually dependent and experientially one. With Bartleby sitting near him in his office, the narrator comments that privacy and society are conjoined. While the narrator is the social self whose utmost concern is social adaptation, Bartleby is the one that wants to keep his privacy without adapting to social rules compelling him to obey. According to Freud, the emergence of consciousness is a painful and difficult departure from the unconscious realm of libidinal plenitude. This process involves a symbolic socialization and acceptance of the social order as a member of a family and society. Bartleby's negative resistance is what happens after the symbolic socialization. One experiences a number of restrictions and frustrations of wants in life. In a sense, the frustration of wants is man's lot, but Bartleby refuses tot accept it. Why the author seizes on the archetypal conflict between self and society is not hard to understand. When Melville wrote the story, he was in a pit of despair owing to the repeated failure of his works to find readers and economic difficulties. At that time, to cater to the tastes of the common readers meant a renunciation of his creativity. What Bartleby rejects is the desperate human condition, that is, "the irreducibly irrational in human existence." It can be said that Bartleby wants to go back to the state preceding the original human condition. So it is rather an archetypal and mythic desire. He will not communicate a world or a cosmos that wants to enslave him. He comes by eternal freedom at the cost of his life.

After using the international theme in his first writing period(mid 60s to 1881), Henry James does not use it for about 25 years. In his Major Phase, the theme appeared again but there's great change in it. According to Jagdish Sharma the change is a departure from "emphasized internationalism" and the departure is very drastic. Therefore the notable difference between the earlier characters and the later ones is that the earlier are portrayed externally while the later internally. The Ambassadors is the typical work of the later dramas and is also one of the best of James's productions. In it the author focuses his mind on the "process of vision" of the hero, Strether. So it can safely be said that to analyze 'the process of vision' is the best way to understand James's literary world. The American Strether comes to Paris to take Chad home whose mother he is engaged to. He is somewhat different from James's early heroes of international novel in that he is neither simple-minded nor innocent but rather complicated and intelligent. He is from the first exhilarated and feels extraordinary sense of escape in his new environment. He is very responsive and sensitive so he begins to correct his misunderstandings on Paris and Paris people. At last Chad is to him not a corrupted youth but looks to be a very smart one. Strether's change of vision is associated with the image of water and the symbol of garden. Water is "most constant image for experience." All the people in Paris are like deep water that contains innumerable experiences. In order to get out of the "tin mould" of a life, to be immersed in the fathomless medium like "silent fish" is indispensable. All the vision and awareness has to do with the symbol of garden. Louise Dauner says the crucial scenes are presented in gardens and the meaning of them leads us to the essence of Jamesian themes. Garden reminds us of the Eden and in order to get knowledge the prohibited fruit is necessary. The moment Strether witnesses the adultery of Chad and Vionnet he experiences "the ritual death." Thus he acquires the wider vision that they are no better than he and he is neither better nor worse than they. In this sense the primary theme of James is "the necessity of experience." Marilyn Hardwig says by the time James wrote this later novel he saw that the association of Europe and America is having reached a stage of "achieved social fusion." In his early novels only James's American characters get something from their keeping company with the Europeans, but in his later novels both Americans and Europeans achieve "perfection" through the intermingle of cultures. Beyond the perfection lies the Jamesian vision of the complete fusion of both societies.

개방각녹내장 환자에서 비마토프로스트(0.03%)와 티몰롤(0.5%) 혼합 제제의 효능과 안정성

Purpose: We investigated the efficacy and safety of a fixed combination of bimatoprost (0.03% w/v) and timolol (0.5% w/v) (Ganfort ® , Allergan Inc., Irvine, CA, USA; fixed combination of bimatoprost-timolol, BTFC) for open-angle glaucoma patients. Methods: We included 25 right eyes with open-angle glaucoma treated with eye drops. The drops were changed to the BTFC without a wash-out period. The therapeutic and adverse effects were evaluated by comparing the intraocular pressure (IOP) before the change to that at 1, 4, 7, and 10 months thereafter. Results: The mean IOP before the instillation of eyedrop and the mean reduction of IOP at each time was 18.66 ± 8.22 mmHg (n = 21) and 2.47 ± 3.19 mmHg (13.24%) at 1 month, 19.05 ± 8.54 mmHg (n = 18) and 3.68 ± 5.55 mmHg (19.32%) at 4 months, 16.68 ± 2.52 mmHg (n = 16) and 2.13 ± 2.87 mmHg (12.77%) at 7 months, and 20.07 ± 9.91 mmHg (n = 14) and 2.79 ± 4.48 mmHg (13.90%) at 10 months (p < 0.05). Side effects included soreness (two eyes, 7.7%), red eye and allergic symptoms (three patients, 11.5%), and discomfort (three patients, 11.5%); there were no severe adverse events such as permanent vision loss or systemic side effects. Conclusions: BTFC reduced the IOP of open-angle glaucoma patients without causing permanent vision loss or any other serious side effects. 목적: Bimatoprost (0.03%)와 timolol (0.5%) 혼합 제제(Ganfort®, Allergan, Inc, Irvine, CA, USA; fixed combination of bimatoprost/ timolol, BTFC)를 점안하는 개방각녹내장 환자들의 안압하강 효과, 안정성을 평가하였다. 대상과 방법: 개방각녹내장이 진단되어 녹내장 약을 사용하던 25명의 환자들의 우안을 대상으로 하였다. 기존에 사용하던 녹내장 약을 휴약기 없이 BTFC로 변경하였고, 변경 전 안압과 변경 후 1, 4, 7, 10개월 후 안압을 비교하여 효과 및 부작용을 평가하였다. 결과: BTFC를 점안하였을 때 점안 전 평균 안압과 점안 후 안압하강 수치는 각각 1달 후 “18.66 ± 8.22” mmHg (n=21), “2.47 ± 3.19” mmHg (13.24%), 4달 후 “19.05 ± 8.54” mmHg (n=18), “3.68 ± 5.55” mmHg (19.32%), 7달 후 “16.68 ± 2.52” mmHg (n=16), “2.13 ± 2.87” mmHg (12.77%), 10달 후 “20.07 ± 9.91 mmHg” (n=14), “2.79 ± 4.48” mmHg (13.90%)였다(p<0.05). 부작용으로따가움(2명, 7.7%), 결막충혈, 가려움(3명, 11.5%), 불편감(3명, 11.5%)이 나타났고, 영구적 시력 손상을 유발하는 부작용 또는 전신부작용을 호소하는 환자는 없었다. 결론: BTFC는 개방각녹내장 환자에서 안압을 효과적으로 감소시키며, 약제로 인해 전신 합병증을 호소하거나 영구적인 시력저하를일으키는 안구손상이 나타난 환자는 없었다.

파장가변 빛간섭단층촬영기와 저간섭성 반사계, 초음파, 자동굴절검사기로 측정한 안구계측 비교

Purpose: We compared optical biometric measurements made using new swept-source optical-coherence tomography, low-coherence reflectometry, autokeratometry, and A-scan biometry. Methods: In total, 86 eyes of 47 patients were included. Axial length, lens thickness, central corneal thickness, anterior chamber depth, and corneal curvature were measured using the Anterion® (Heidelberg Engineering, inc., Heidelberg, Germany) and Lenstar LS 900® (Haag-Streit AG, Koeniz, Switzerland) platforms. Corneal curvature and axial length were measured using the Essilor AKR 750® (Essilor instruments, France, Charenton-le-Pont) and PacScan 300A® (Sonomed Inc., Chicago, IL, USA) platforms, respectively. The evaluated biometric parameters were compared and verified; intraclass correlation coefficients and Bland-Altman plots were used to analyze statistical agreement. Results: The differences between the Anterion® and Lenstar LS 900® platforms in terms of axial length, lens thickness, central corneal thickness, anterior chamber depth, and white-to-white diameter were -0.02 ± 0.08, 0.08 ± 0.13, -2.66 ± 11.12, 0.05 ± 0.12, and 0.08 ± 0.26, respectively. All values were statistically significant. The mean corneal curvature between Anterion® and Lenstar LS 900® had a statistically significant difference of -0.08 ± 0.47, while the difference between the Anterion® and AKR 750® platforms was not significant. Biometric parameters measured using an Anterion® and other devices showed a strong positive correlation when assessed using Pearson’s correlation analysis; there was good agreement between the results when analyzed using intraclass correlation coefficients and Bland-Altman plots. Conclusions: Optical biometric measurements made using the Anterion® platform were not significantly different from those obtained using other devices, but there were significant differences compared to the Lenstar LS 900® platform. Since these significant differences could influence decision making during intraocular lens selection for cataract surgery, the values are clinically useful for reference. 목적: Anterion®으로 측정한 안구 생체계측과 Lenstar LS900®, Essilor AKR 750®, PacScan300A®로 측정한 안구계측을 비교하여Anterion®의 유용성을 알아보고자 하였다. 대상과 방법: 환자 47명, 86안을 선정하여 Anterion®, Lenstar®로 안축장, 수정체두께, 중심각막두께, 전방깊이, 각막곡률, AKR 750® 으로 각막곡률, PacScan 300A®로 안축장을 측정 후, 차이값과 상관관계를 확인했으며, 급내상관계수와 Bland-Altman plot으로 일치도를 분석하였다. 결과: Anterion®, Lenstar®로 측정한 안축장, 수정체두께, 중심각막두께, 전방깊이, 각막윤부직경은 -0.02 ± 0.08, 0.08 ± 0.13, -2.66 ± 11.12, 0.05 ± 0.12, 0.08 ± 0.26으로 유의미한 차이를 보였다. Anterion®과 Lenstar®로 측정한 평균 각막굴절력은 -0.08 ± 0.47로 유의미한 차이를 보인 반면, AKR 750®와 비교했을 때는 차이가 없었다. Anterion®의 측정값은 다른 기기의 측정값과 피어슨상관계수를 검증했을 때 강한 양의 상관관계를 보였고 급내상관계수, Bland-Altman plot에서 높은 일치도를 보였다. 결론: Anterion®은 기존 기기들과 비교했을 때 차이의 절대값은 크지 않으나, Lenstar®의 경우 통계적으로 유의한 차이를 보인 지표들이 있었다. 백내장수술에서 인공수정체를 결정하는 등 임상 적용 시 기존 기기와 다른 결과를 만들 수 있기 때문에 주의할 필요가있다.
