        Depantment of pulmonary Internal Medi-cme, College of Oriental Medicme, Kyung Hee University. Seoul Korea I could come to the conclusion like the following after studying many western and eastern books on sputum. 1. Sputum is a kind of tangible dirty pathogenic fliud secreted by respiratory tract, and mucous which is mixed substance of secretion from trachea and bronchus and foreign body. And it is expectorated or swallowed after moved to pharynx by ciliary movement 2. In oriental Medicme, there are wind, cold, damp, hot, dry stagnant, Qi sputm, while in western medicne, mucus, serous, purulent, bloody. 3. Morphologically, cold and damp sputum are similar to mucous sputum, wind sputum to serous, and hot, dry and stagnant sputum to purulent or mucopurulent. The classification of sputum by observing and comparing type, color and amount of sputum might be of great use to inspection of respiratory disorders.

        각막굴절교정수술 후 각막 절삭량에 따른 안압 보정방법의 비교

        Purpose : The purpose of this study was to evaluate the intraocular pressure (IOP) that would be underestimated after corneal refractive surgery according to corneal ablation amounts, and was to suggest the most appropriate corrected IOP method for each group. Methods : The subjects were 1,002 eyes of 501 people (237 men, 264 women) who underwent corneal refractive surgery. The mean age was 26.89±5.18 years. IOP was measured with a non-contact tonometer and 4 correction methods, and was used to correct the underestimated IOP. IOP in three groups with low, moderate and high myopia according to ablation amounts was analyzed postoperative corrected IOP to compare correction methods. Results : The preoperative IOP was 15.67±2.39 mmHg and postoperative IOP was 10.12±2.11 mmHg, which was significantly lower than the mean 5.55±2.09 mmHg. The postoperative IOP calculated by Ehlers' correction method was 14.53±2.47 mmHg, which was 1.14±2.83 mmHg different from the preoperative IOP, showing the most similar to preoperative IOP. In addition, the IOP corrected by the Ehlers equation showed the smallest difference from preoperative IOP in all groups. Conclusion : The Ehlers correction method is shown the most accurate among the four correction methods in low, moderate, and high myopia groups after corneal refractive surgery. It would be a useful method for preventing the late diagnosis and treatment of glaucoma. 목적 : 각막굴절교정수술 후 저평가되는 안압을 다양한 각막 절삭량에 따라 경도, 중등도, 고도 근시 군을 나누어 조사하고, 각 군별로 가장 적합한 수술 후의 안압 보정방법을 제시하고자 하였다. 방법 : 각막굴절교정수술을 받은 평균 연령 26.89±5.18세의 남녀 501명 1,002안(남자 237명, 여자 264명)을대상으로 하였다. 안압은 비접촉식 안압계를 이용하여 측정하였고, 수술 후 저평가된 안압을 보정하기 위해 Ehlers,Shah, Dresden, Orssengo/Pye의 보정방법을 이용하였다. 다양한 각막 절삭량에서의 안압 차이를 비교하기 위해 세 그룹으로 나누어 분석하였다. 결과 : 비접촉식 안압계로 측정한 수술 전 안압은 15.67±2.39 mmHg이었고, 수술 후 안압은 10.12±2.11mmHg로 평균 5.55±2.09 mmHg 만큼 유의하게 낮게 측정되었다. Ehlers의 보정방법으로 산출된 수술 후의 안압은 14.53±2.47 mmHg로 수술 전 안압과 평균 1.14±2.83 mmHg 차이를 보여 네 가지 보정방법 중에서 수술전 안압 측정치와 가장 비슷한 수치를 보였다. 또한 각막 절삭량에 따라 나눈 경도, 중등도, 고도근시 각 군 모두에서 Ehlers 식으로 보정된 안압이 수술 전 안압과 차이가 가장 적게 나타났다. 결론 : 굴절교정수술 후 고도근시를 포함한 다양한 근시군 모두에서 4가지 보정방법 중 Ehlers 보정법이 가장유용한 것으로 판단된다. 이 결과는 각막굴절교정수술 후 저평가되는 안압의 실제 값을 산출하여 녹내장에 대한조기진단 및 치료시기가 늦어지는 것을 막는 데에 활용할 수 있을 것이라 생각된다.

        뇌졸중에 병발된 폐렴환자의 임상적 고찰

        Objective : Pneumonia is a frequent complication in stroke and is very important as a prognostic factor. So We had a clinical study about pneumonic patients combined with stroke in which we administered both antibiotics and oriental herbal medicine to them. Methods : The 14 cases were patients that admitted visited 31CU at neurologic department in Kyunghee medical center from the 1st of October to the 31st of November, 1998. because of stroke. We selected cases complicated pneumonia. We diagnosed stroke as brain CT or MRI and pneumonia as chest simple X-ray, fever, sputurn culture et al. We divided the group into several categories(衛分證, 氣分證, 營分證, 血分證, 正虛邪退證) and medicined them both Paerum-bang(肺炎方) and antibotics. We evaluated the results as increased signs and some tests including chest simple X-ray, GOT, GPT, BUN and Creatinine. Results : Among total 14 cases, cases of perfect cure including improvement were 10, aggravation were 2 and discontinued cases were 2. We observed 17.4 days on the average. Their functions of liver and kidney weren't aggravated as medicining both antibiotics and oriental herbal treatment. Conclusion : We suggest that eastern treatment need to improve resistance of pneumonic patients and may reduce the side effect of western antibiotics therapy.

        경옥고(瓊玉膏)가 결핵균(結核菌)에 미치는 영향(影響)에 관(關)한 실험적(實驗的) 연구(硏究)

        Objective : In order to know the antibacterial effects of Kyungok-go against Mycobacteria tuberculosis, Methods : In this study, I inverstigated these effects in terms of combination of other antibiotics with and without Kyungok-go on several different media conditions. Results: On Ogawa medium, Kyungok-go of the $10{\mu}/ml$ concentration showed the anti-Mycobacteria tuberculosis activity against antibiotic drug-sensitive strain. On Middle-blue medium, Kyungok-go of the $10{\mu}/ml$ concentration showed the anti-Mycobacteria tuberculosis activity against antibiotic drug-sensitive strain. Kyungok-go showed the anti mycobacteria tuberculosis activity with the meaningful result above a certain concentration. The resistance against M, tuberculosis as the concentration of Kyungok-go was decreased significantlly on the high concentration($500{\mu}/ml$) When rifampicin and Kyungok-go were used together, the resistance was decreased with the statistical significance as to the persistant antibacterial effect against M. tuberculosis, When ciprofloxacin and Kyungok-go were used together, the resistance was decreased with the statistical significance as to the persistant antibacterial effect against M. tuberculosis, The combination of treatment, Kyungok-go with both rifampicin and ciprofloxacin, showed much better antibacterial effect against M, tuberculosis than antibiotics alone. Conclusions : This study shows that Kyungok-go has antibacterial effect against M. tuberculosis and in the combination of treatment, Kyungok-go with antibiotics, showed much better antibacterial effect against M. tuberculosis than antibiotics alone,.

        행인(杏仁)이 천식관련 chemokine 분비와 호산구 chemotaxis에 미치는 영향

        Objective: Eosinophils are typically characterized by a bilobar nucleus with highly condensed chromatin and cytoplasm containing two major types of granules, specific and primary granules, and lipid bodies. The role of inflammation in asthma and other allergic diseases of the airways is widely appreciated, and airway inflammation is now included as a defining feature of asthma. The importance of the presence of eosinophils in the airways of patients with fetal asthma has long been recognized, but the mechanism by which these cells are recruited and retained in the lungs are only now being elucidated. Eotaxin is a potent and specific eosinophil chemoattractant that is mobilized in the respiratory epithelium after allergic stimulation. Methods : Water extracts of Armeniacae Amarum Semen(AAS) and pulmonary epithelial cell lines A549(alveolar typeII epithelial cells) and human eosinophils were used. Cytotoxic effects of AAS and MIS assay were estimated, as well as the effects of AAS on chemokines from prestimulated A549 cells by sandwich ELISA and RI-PCR. Chemotaxis assay was conducted on prestimulated eosinophils treated with AAS. Results : In this study it is demonstrated that $TGF-{\alpha}$, IL-4 and $IL-1{\beta}$ induced the accumulation of chemokine mRNAs in the alveolar epithelial cell lines A549 in dose-dependent manner. Eotaxin and IL-8 were inhibited by AAS in dose-dependent manner(p<0.05). Eosinophil migration was inhibited at high concentrations of AAS(p<0.05). Conculusions : These findings are indicative of suppression of eotaxin and IL-8, and suggest that this is accomplished through AAS treatment. This raises the possibility that AAS is of therapeutic value in diseases such as asthma.
