목적: 천추에 발생한 척색종에 대해 수술적 치료를 시행했던 환자들의 장기 추시 결과를 알아보고자 하였다. 대상 및 방법: 천추 척색종으로 수술을 시행받았던 4예를 대상으로 하였으며, 평균 추시 기간은 8.3년(3~11)이었다. 3예에서 후방 도달법을, 1예에서는 전후방 도달법을 통해 종양의 완전 절제를 시행하였으며, 1예에서 제 2천추 신경근 이상을, 3예에서 제 3천추 신경근 이상을 보존하였다. 술 후 배뇨, 배변 장애 유무를 포함한 하지의 운동, 감각 장애 유무를 평가하였고 수술 부위 감염 등의 합병증과 추시상 국소적인 재발 및 원거리 전이 유무를 조사하였다. 결과: 전 예에서 술 후 하지의 운동 장애는 없었으나 1예에서 우하지의 방사통이 심하였고, 1예에서 수술창의 감염이 있었다. 술 후 배뇨 기능은 2예에서 정상이었고 1예에서는 간헐적 요실금을, 1예에서는 지속적인 자가 도뇨를 시행하였다. 배변 기능은 1예에서 정상이었고, 2예에서는 변비를 보였다. 추시상 1예에서 재발 및 전이가 없었고, 1예에서 수술 부위의 재발이, 1예에서 원거리 전이 및 수술 부위의 재발이 있었다. 결론: 천추에 발생한 척색종에 대하여 수술적 절제 후 평균 8년 이상의 생존으로 만족할 만한 결과를 보였으며, 조기 발견을 통한 적극적인 수술적 치료가 상부 천추 신경근의 보존으로 기능 소실의 최소화와 함께 생존을 연장할 수 있는 방법으로 생각된다. Purpose: To evaluate the long term results of surgical treatment for sacral chordoma. Material and Method: We reviewed the records of 4 patients with sacral tumor treated surgical resection. Mean follow-up duration was 8.3 year(3~11). All cases were performed complete resection of tumor mass through posterior approach in 3 cases and anterior-posterior approach in one. The most caudad nerve-roots spared were the second sacral roots in one and the third sacral roots in 3 cases. Functions of voiding and defecation including neurological symptoms of lower extremities, other complications of surgical treatment, and local recurrence or distant metastasis of tumor were evaluated. Result: There was no motor deficit in all cases, but radiating pain was developed in one and wound infection in one. Bladder function was preserved in 2 cases, intermittent incontinence in one, and doing intermittent catheterization in one. Bowel function was preserved in one and 2 cases were suffered from constipation. At the last follow-up, disease-free was in one, local recurrence in one, and local recurrence with distant metastasis in one. Conclusion: The result of surgical resection for sacral chordoma is satisfactory showing average 8 years survival. Early detection and aggressive surgical treatment is the best to prolong survival and to minimize functional deficit with preservation of upper sacral nerves.

유리관의 관경이 4 mm인 형광램프에서 종단방전과 횡단방전의 방전 특성, 발광 특성, 그리고 분광 특성을 상호 비교 분석하였다. 제작된 형광램프의 주입 기체는 3 종류이며, 수은 혼합 기체인 Ne(95%)+Ar(5%)+Hg(2 mg), 무수은의 순수 Xe 100% 및 Ne(96%)+Xe(4%)의 혼합 기체이다. 각 형광램프 샘플들의 기체 압력은 8 종으로 압력 범위는 $5{\sim}300\;Torr$이다. 상기 램프의 특성을 조사하기 위하여 제작된 샘플 램프의 전체 수는 주입 기체 3 가지와 각 압력 8 가지로서 총 24 개이다. 수은 형광램프는 양광주 발광을 특징으로 하는 종단방전에서 발광 휘도 및 효율이 매우 높고, 횡단방전에서는 거의 발광하지 않는다. 무수은 제논 형광램프는 횡단방전에서 비교적 높은 휘도 특성을 보이며, 종단 방전에서는 방전 전압이 높고 발광 효율도 매우 낮다. 제논 형광램프의 횡단방전은 Ne+Xe(4%)인 혼합 가스보다 압력($\sim$수 100 Torr)이 높은 순수 제논 가스의 발광 효율이 더 높다. 본 실험을 통하여 수은 램프와 무수은 제논 램프의 방전 방식의 차이를 확인하였다. 수은 형광램프는 양광주를 활용하는 종단방전의 형태로서 종래의 튜브형이 바람직하다. 무수은의 제논 형광램프는 높은 압력($\sim$수 100 Torr)의 순수 제논 기체에 대하여 방전 경로가 짧은 횡단방전을 특징으로 하는 평판형이 바람직하다. The properties of discharge, luminance, and spectroscopy are investigated in a longitudinal and transverse discharge fluorescent lamps with tube of outer diameter 4 mm. The sample lamps are prepared to be three kinds of gas composition such as mercury lamps of Ne(95%)+Ar(5%)+Hg(2 mg), the mercury-free lamps of Xe 100% and Ne+Xe(4%). The gas pressure is in the range of $5{\sim}300\;Torr$. In the mercury lamps, the longitudinal discharge having a positive column is high in luminance and efficiency, while the transverse discharge is no luminance at all. In the Xe-lamps, the transverse discharge shows relatively good in efficiency as compared with the longitudinal discharge which has a high discharge voltage and a low luminance and efficiency. In the transverse discharge of relatively high efficiency, a pure Xe(100%) gas discharge has a higher efficiency than the mixture gas of Ne+Xe(4%). Through these experiments, the properties of mercury and xenon lamps are verified. In the mercury lamps, the longitudinal discharge of tubular fluorescent lamps is high in luminance and efficiency, while the transverse discharge of flat panel fluorescent lamps are low in luminance efficiency. In the mercury-free lamps, the flat fluorescent lamps of transverse discharge having a high pressure ${\sim}100\;Torr$ with the pure Xe-gas are verified to be suggestable.

경추에서 발생한 호산구 육아종의 수술적 치료 - 3예 보고 -

척추에서의 호산구 육아종은 흔하지 않으며, 특히 경추 및 후방 신경궁에서의 발생은 드물다. 또한 호산구 육아종은 자기 공명 영상을 포함한 방사선학적 검사상 추체의 붕괴, 척추경의 소실 및 척추 주변 연부 조직의 종물 형성 등의 소견을 보이므로 다른 악성 골종양이나 전이성 종양 또는 결핵성 척추염 등과 감별을 요한다. 저자들은 각각 제 4 경추, 제 7 경추의 후방 신경궁, 그리고 제 4 경추체에서 발생한 호산구 육아종에 대하여 수술적 치료 후 완치를 보인 3례를 경험하였기에 문헌 고찰과 함께 보고하고자 한다. The involvement in the spine of eosinophilic granuloma is not common. Especially, involvement of cervical spine and posterior neural arch is quite rare. In addition, radiographic findings including magnetic resonance images of eosinophilic granuloma are manifested as vertebral body collapse, loss of pedicle and paravertebral soft mass formation; it must be differentiated with other conditions, such as malignant bone tumor, metastatic cancer or tuberculous spondylitis. We experienced three cases of eosinophilic granuloma involving posterior neural arch of C4, C7, and body of C4 respectively, which were performed surgical treatment and achieved complete healing. We report these cases with review of literatures.

본고의 목적은 무령왕 발굴 40주년을 맞이하여 그간의 연구 성과물을 문화산업적으로 활용할 수 있도록 하는데 있다. 이에 무령왕의 역사문화적 가치를 살펴보고, 스토리텔링을 통해 대중성을 높이며, 이를 바탕으로 다양한 콘텐츠를 예시하였다. 무령왕의 역사문화적 가치는 일일이 언급하지 않아도 된다. 무령왕릉 자체가 세계문화유산 잠재목록에 등록될 정도로 주목을 받고 있기 때문이다. 본고에서는 학술적인 면보다는 대중들이 흥미를 느끼고 감동을 받을 수 있는 소재에 중심을 두었으며, 그의 일대기를 4단계로 나누어 그 가치를 제고하였다. 무령왕의 스토리텔링은 사극 열풍의 추세와 부합될 수 있는 구성을 갖추었다. 출생은 ``誕生說話``로, 권력을 거머쥐는 과정은 ``國人共謀``로, 시대가 요청하는 개혁 군주로서의 모습은 ``更爲强國``으로, 무령왕릉의 발굴과 영향은 ``神話創造``로 구성할 수 있다. 이 모든 막의 구성은 사료에 바탕을 두면서도 무한한 상상력이 가미된 흥미진진한 요소가 많았다. 무령왕이 더할 나위 없이 좋은 소재인 것은 이를 콘텐츠로 활용할 수 있는 여러 조건을 갖추었다는 점이다. 현재 문화재청에서 추진하고 있는 공주고도 보존 및 육성사업과 연계하여 다양한 콘텐츠를 개발할 수 있기 때문이다. 무령왕 시대를 배경으로 백제의 의식주를 복원하고, 가상현실을 제작한다면 그 파급 효과가 상당하다. 이는 지역 경제에 실질적으로 보탬이 될 수 있는 문화산업의 구체적 사례가 될 것이라 확신한다. The purpose of this study was to promote the cultural and industrial uses of the research results related to King Muryeong in commemoration of excavating his tomb 40 years ago. The study reviewed his historical and cultural value, explored ways to increase his popularity through storytelling, and proposed various contents based on the findings. There is no need to enumerate the historical and cultural values of King Muryeong one by one since his tomb has attracted so much attention that it was placed on the potential list of the world cultural heritage. The study focused on the materials that people could take interest in and be moved rather than academic aspects, trying to enhance his value in four stages of his life. King Muryeong`s story has some elements to join the current vogue of historical dramas. His birth is a "birth tale"; his process of grasping power tells "people complicity"; his aspects as a reform monarch demanded by the times tell "becoming a powerful nation again"; and the excavation and influences of his tomb present "creation of legend." Those acts contain many interesting elements with infinite imagination added to the historical facts. King Muryeong is one of the best materials because he has various conditions for content utilization. He can be used to develop a variety of contents in relation to a project of preserving and promoting the ancient capital, Gongju, by Cultural Properties Administration. Once Baekje`s food, shelter, and clothing are reproduced with a focus on his period along with virtual reality, they will have enormous ripple effects. He will provide a specific case of cultural business to make practical contributions to local economy.
