        아버지와의 스포츠 활동 프로그램 참여에 따른 유아 자아유능감 및 아버지 양육참여의 변화

        The purpose of this study was to investigate the effects of the father-child sports activity based program on young children's self-competence and fathers' participation in rearing. The subjects were 35 five-year-old children in a kindergarten and their fathers. The experimental group of children and their fathers participated in father-child sports activity program biweekly for 12 weeks. To verify the effect of the program, the pre-test and post-test were conducted by using the children's self-competence scale, the father rearing participation scale for children, and three sub-constructs in the inventory of father involvement. The data were analyzed by t-test analysis using SPSS WIN program. The results of the study indicated that children's physical and social self-competence had increased after they participated in the father-child sports activity program. The results also showed that children perceived that their fathers' participation in rearing had increased after they participated in the father-child sports activity program. Finally, the results revealed that fathers perceived that their time spent on working together with children had increased after they participated in the father-child sports activity program. 본 연구의 목적은 아버지와 함께 스포츠 활동 프로그램에 참여한 유아의 자아효능감이 어떻게 변화할 것이며 유아와 아버지가 인식한 양육참여도는 어떻게 변화할 것인지를 아버지-유아 스포츠 활동 프로그램의 효과 검증을 통해 제시하는데 있다. 연구대상은 S시에 위치한 한 유치원의 만 5세 유아와 아버지를 대상으로 하여 실험집단 18명과 통제집단 17명을 선정하였다. 연구도구는 유아를 대상으로 한 자아유능감 검사와 아버지 양육참여도 검사와 아버지를 대상으로 한 아버지 양육참여도 검사를 수정·보완·삭제하여 사용하였다. 연구절차는 실험집단과 통제집단을 대상으로 한 사전검사와 12주 동안의 아버지-유아 스포츠 프로그램 적용, 그리고 사후검사 순으로 진행되었다. 자료분석은 수집한 실험집단과 통제집단의 사전검사 자료는 t-검증을 통해 집단 간 동질성을 분석하였고, 처치 후 수집된 실험집단과 통제집단의 사후검사 자료도 t-검증을 통하여 집단 간 차이를 비교하여 실험처치의 효과를 분석하였다. 연구결과, 아버지-유아 스포츠 활동 프로그램에 참여한 유아의 전반적인 자아유능감이 향상되었고 유아는 아버지의 양육참여 정도가 긍정적으로 변화하였다고 인식하였다. 또한 아버지-유아 스포츠 활동 프로그램에 참여한 아버지는 양육참여 하위요인 중 자녀와 함께 하는 시간이 증가하였다고 인식하였다. 본 연구의 결과는 스포츠 활동을 통한 아버지-유아 참여 프로그램의 긍정적 효과를 보여줌으로써 아버지 참여 프로그램의 새로운 접근을 모색했다는 점에서 중요한 의의가 있다고 할 수 있다.

        미용코성형술에서 코뼈자름술의 적용

        Nasal osteotomy for aesthetic results is required in patients who have broad nasal bone. Since the procedure is difficult, many operators hesitate the surgery. From January 2007 to July 2009, we performed 77 cases of nasal osteotomy, consisting of paramedian oblique medial osteotomy or median oblique-medial osteotomy and percutaneous lateral osteotomy. Most of the patients had satisfying results, however three patients had asymmetric shape and inappropriate inward movement of nasal bone which required revision with percutaneous lateral osteotomy. Osteotomy procedure varies,depending on surgeon’s preference and experience, however, each surgeon should be aware of good and weak points of those procedures and use the most suitable method. In order to achieve maximal satisfying results, preoperative analysis of patients and minimal invasive handling should be done. Based on these efforts, this method could broadly be used in the aesthetic field. (J Korean Soc Aesthetic Plast Surg 16: 15, 2010)

        Obtaining Maximal Stability with a Septal Extension Technique in East Asian Rhinoplasty

        Recently, in Korea, the septal extension graft from the septum or rib has become a common method of correcting a small or short nose. The success rate of this method has led to the blind faith that it provides superior tip projection and definition, and to the failure to notice its weaknesses. Even if there is a sufficient amount of cartilage, improper separation or fixation might waste the cartilage, resulting in an inefficient operation. Appropriate resection and effective fixation are essential factors for economical rhinoplasty. The septal extension graft is a remarkable procedure since it can control the nasal tip bidirectionally and three dimensionally. Nevertheless, it has a serious drawback since resection is responsible for septal weakness. Safe resection and firm reconstruction of the framework should be carried out. Operating on the basis of the principle of “safe harvest” and rebuilding the structures is important. Further, it is important to learn several techniques to manage septal weakness, insufficient cartilage quantity, and failure of the rigid frame during the surgery.

        짧은코변형에서 코외피의 연장방법 : 가로코근걸이 해리와 코경첩 부위 근육융합체의 분리

        Cartilage extension and nasal envelop expansion play a main role incorrecting short or contracted nose. Despite numerous studies for cartilage expansion, there has been no reports of nasal skin elongation methods. We hereby preport a new method for expansion of nasal envelop with a comprehensive understanding of anatomical structures. From April 2009 to September 2010, 6 patients underwent operations to correct short or contracted nose. Two separating procedures were included for nasal envelop elongation; division of muscle (Procerus, Transverse nasali, Levator labii superior alaque nasi: PTL muscles) confluence located at nasal hinge and release of transverse nasalis sling. To estimate the degree of nasal envelop extension, forced skin traction test was performed. Comprehensive research with fresh cadaver was held to study the relationship between nasal SMAS and surrounding structures. Average 3.8 mm elongation was documented by forced skin traction testafter the procedure. In the fresh cadaver study, transverse nasalis sling and PTL muscle confluence were firmly attached to the supportive framework. From our clinical experience and cadaver study, we discovered that release of transverse nasalis sling and division of PTL muscle confluence are the main factors for nasal envelop expansion in short or contracted nose.

        The optical CT was developed to use twisted fibers for 170 kV GIS. The fiber sensor optimized on the optical CT was wound 3 turns and twisted 4 times per a turn at the pipe with a diameter of 130 mm. The measurement error for the linearity which was satisfied the criterion of 'IEC Class-1.0' was less than ± 0.44 % at the transmission type and ± 0.82 % at the reflection type to 2,000 A. At a low current to 200 A, the measurement error was ± 0.69% at the transmission type and ± 1.38 % at the reflection type.

        Quantitative Evaluation of Patient-Specific Quality Assurance Using Online Dosimetry System

        In this study, we investigated the clinical performance of an online dosimetry system (Mobius FX system, MFX) by 1) dosimetric plan verification using gamma passing rates and dose volume metrics and 2) error-detection capability evaluation by deliberately introduced machine error. Eighteen volumetric modulated arc therapy (VMAT) plans were studied. To evaluate the clinical performance of the MFX, we used gamma analysis and dose volume histogram (DVH) analysis. In addition, to evaluate the error-detection capability, we used gamma analysis and DVH analysis utilizing three types of deliberately introduced errors (Type 1: gantry angle-independent multi-leaf collimator (MLC) error, Type 2: gantry angle-dependent MLC error, and Type 3: gantry angle error). A dosimetric verification comparison of physical dosimetry system (Delt4PT) and online dosimetry system (MFX), gamma passing rates of the two dosimetry systems showed very good agreement with treatment planning system (TPS) calculation. For the average dose difference between the TPS calculation and the MFX measurement, most of the dose metrics showed good agreement within a tolerance of 3%. For the error-detection comparison of Delta4PT and MFX, the gamma passing rates of the two dosimetry systems did not meet the 90% acceptance criterion with the magnitude of error exceeding 2 mm and 1.5, respectively, for error plans of Types 1, 2, and 3. For delivery with all error types, the average dose difference of PTV due to error magnitude showed good agreement between calculated TPS and measured MFX within 1%. Overall, the results of the online dosimetry system showed very good agreement with those of the physical dosimetry system. Our results suggest that a log file-based online dosimetry system is a very suitable verification tool for accurate and efficient clinical routines for patient-specific quality assurance (QA).

      • UGT2B7 유전형이 발프로익산(Valproate)의 약동학에 미치는 영향

        UDP-glucuronosyl transferases (UGT) mediate a conjugation reaction by glucuronidation of various clinically important drugs. The functional significance of genetic polymorphisms in these UGTs are main concern of the current researchers of UGTs. Valproate (valproic acid) is an antiepileptic drug glucuronidated mainly by UGT2B7. We investigated the effect of genetic polymorphism on pharmacokinetics of valproate in healthy normal volunteers. Before the clinical study, we screened genotype of UGT2B7-T801A (T:*1, A:*2). Twenty three healthy subjects were enrolled and grouped with regard to the UGT2B7 genotype. The pharmacokinetic profile of valproate was characterized after valproate administration (600 mg once daily for 5 days). The plasma concentrations of valproate were analyzed before and 1, 1.25, 1.5, 2, 2.5, 3, 5, 7, 9, 13, 25, 49 hours after 5th valproate administration by TDX with FPIA method. Although the pharmacokinetic profiles of valproate did not showed significant differences between the UGT2B7 genotype groups, a trend of decrease of clearance as increase of UGT2B7*2 allele was observed. Oral clearance of valproate in the UGT2B7*2/*2 group was 66% of that of the UGT2B7*1/*1 group. The results suggest that the UGT2B7*2/*2 genotype group possibly has a decreased clearance of valproate. Further study may be required to clarify the results.
