      • 치밀형 유방에서 유방특이감마영상검사의 유용성 평가

        Purpose: This study was to evaluate usefulness of $^{99m}Tc$-MIBI scintimammography for dense breast by comparing concordance in test results between Tc-99m MIBI scintimammography and mammography whose effect was proved the most as an imaging tool depending on breast density and at the same time by examining limitation on evaluation depending on density of breast tissue. [Materials and Methods] In the period from December 2010 to July 2011, this study targeted 150 patients who took both of $^{99m}Tc$-MIBI scintimammography and mammography conducted by using breast gamma camera in this hospital. Breast density was classified to the four levels of pattern 1~4 based on the results of mammography. $^{99m}Tc$-MIBI scintimammography was conducted with the LCC, the RCC, the LMLO, and the RML one minute after intravenous injection of 99mTc-MIBI 7400 MBq (20 mCi) while analysis was made for concordance in test results of $^{99m}Tc$-MIBI scintimammography and mammography. [Results] Among the 150 patients, pattern 1 was found in 3 patients, pattern 2 in 44 patients, pattern 3 in 61 patients, and pattern 4 in 37 patients. There were 5 patients who showed the case where it was impossible to determine density of breast tissue due to foreign body inserted to breast. The concordance ratio of the results between $^{99m}Tc$-MIBI scintimammography and mammography was 95.5% for pattern 2, 95.1% for pattern 3 and 94.6% for pattern 4. This demonstrated that the concordance rate decreased according to the increase in breast density. [Conclusion] When there was limitation on evaluation of breast specific gamma imaging test results due to increased intake in breast tissue or surgical site, the concordance rate was 6.8% for pattern 2, 16.3% for pattern 3 and 18.9% for pattern 4. This demonstrated that the degree of limitation on evaluation of breast specific gamma imaging test results increased according to the increase in breast density.

        B형 만성 간염 환자에서 인터페론 단독 요법과 인터페론과 라미부딘 병합 요법의 비교

        Background/Aims: Hepatitis B virus (HBV) is associated with clinically significant chronic infection that may lead to the development of cirrhosis or even hepatocellular carcinoma. However, interferon alpha is effective in only 25-40% of chronic hepatitis B patients. The purpose of this study was to evaluate the effectiveness of interferon and lamivudine combination treatment comparing with interferon alone. Methods : Twenty patients with chronic hepatitis B(moderate to severe activity) were randomized to with interferon alone (11 patients) or combination (9 patients) therapy for 24 weeks. The patients were monthly evaluated, biochemically and serologically. Results : There was no statistical difference between both groups in HBeAg seroconversion rate and ALT normalizaion rate at the end of treatment. But, HBV DNA suppression rate was significantly higher in combination group (100%) than the interferon alone group (54%). No serious side effects were noted in both groups. Conclusion : Combinaton therapy with alpha interferon plus lamivudine given for 24 weeks appears safe and is well tolerated. But, combination therapy was no more efficient than interferon alone in HBeAg seroconversion except HBV DNA suppression rate during the treatment period.

        버섯균사체로 배양된 인삼 Saponin과 β-Glucan 함량 변화

        인삼을 배지로 사용하여 목질진흙버섯(Phellinus linteus), 영지버섯(Ganoderma lucidum) 및 노루궁뎅이버섯(Hericium erinaceum) 균사체를 접종 배양하여 얻어진 균사체 인삼배양물과 원료 인삼에 대한 사포닌과 $\beta$-glucan의 변화를 분석하였다. 원료인삼의 구성사포닌 함량은 22.47mg/g이었으나 균사체로 배양된 인삼의 사포닌 함량은 목질 진흙버섯이 39.78 mg/g, 영지버섯이 55.88 mg/g 그리고 노루궁뎅이버섯이 32.69 mg/g으로 원료인삼에 비해 증가하였다. 균사체 인삼배양물의 사포닌은 PPT계는 감소하였고 PPD계는 증가하였다. 구성사포닌 중 ginsenoside Rd는 원료인삼에 비해 균사체 인삼배양물에서 각각 17, 30 및 12배 증가하였다. 3종의 균사체와 균사체 인삼배양물들의 총 glucan 함량은 각각 46.24, 34.74 및 34.02% 그리고 28.77, 17.78 및 21.77%이었다. $\beta$-glucan 함량은 균사체에서는 각각 29.14, 19.44 및 23.39%이었고 인삼배양물에서는 각각 8.85, 5.46 및 5.51%로 나타나 목질진흙버섯이 균사체와 인삼배양물에서 가장 높은 함량을 나타내었다. This study investigated the changes of saponin and $\beta$-glucan content on the cultured ginseng with mushroom mycelia of Phellinus linteus (PL), Ganoderma lucidum (GL), and Hericium erinaceum (HE). Cultured ginsengs with mushroom mycelia were extracted with 80% ethanol, fractionated with n-buthanol, and analysed for ginsenosides by high performance liquid chromatography (HPLC). Crude saponin content of raw ginseng was 4.11% (d.b) but cultured ginseng with mushroom mycelia of PL, GL, and HE were increased to 6.74, 6.77 and 6.23% (d.b), respectively. Ginsenoside-Rd, among the 12 ginsenosides which were available for analysis, was remarkably increased to 13.61, 24.26, and 32.69 mg/g, respectively (raw ginseng: 0.80 mg/g). The $\beta$-glucan content of cultured ginseng with mushroom mycelia of PL, GL, and HE were decreased to 8.85, 5.51 and 5.46% rather than mushroom mycelia of 29.14, 19.44, and 23.39% (d.b), respectively.

        P2P(Peer-to-Peer) 시스템은 분산 네트워크 환경을 효율적으로 이용할 수 있는 기술을 제공하며, 다양한 분야에서 활발하게 연구되고 있다. 특히 최근에는 분산 해쉬 테이블(Distributed Hash Table)을 이용한 구조적 P2P 시스템에서 부하 분산 기법 기법에 대해 많은 연구가 진행되고 있다. P2P 시스템은 피어의 능력의 이질성 및 빈번한 참여, 탈퇴로 인해 부하의 불균형이 발생하게 된다. 본 논문에서는 기존의 방식에 피어의 활용도를 고려하여 부하 분산 기법을 수행하도록 개선하여 부하 분산 기법 효과를 향상시키는 기법을 제안한다. 제안하는 방식은 부하 분산 기법을 수행하도록 개선하여 부하 분산 기법 효과를 향상시키는 기법을 제안한다. 제안하는 방식은 부하 분산 기법 수행과 피어의 참여 및 탈퇴 시 가상 서버의 이동이 이뤄질 피어의 선택 방식에 있어 피어의 활용도를 고려함으로써 부하 분산 기법 효과를 향상시킨다.

        The Exhibition and Convention Center went beyond being a space for simple exhibitions and conferences and served as a landmark to increase regional competitiveness and brand value. Additionally, as the world economy has become increasingly global, it has become larger and more modern. Based on this, Germany, the United States, China, Japan, and Korea are continuously expanding facilities related to the exhibition and convention industry, including the construction of new exhibition and convention centers and expansion of facilities. This study examined the research trends of exhibition and convention centers in Korea to date. After conducting a literature review focusing on definition, development process, and classification of scale, previous research was analyzed focusing on research direction, research method, and research subject.

        버섯균사체로 배양된 인삼 Saponin과 β-Glucan 함량 변화

        저칼륨혈증성 주기성 마비로 발현한 원발성 쇼그렌 증후군

        We describe a 46-year-old woman with hypokalemic paralysis as the initial manifestation of Sjogren`s syndrome. Sjogren`s syndrome is an autoimmune exocrinopathy, characterized by keratoconjuntivitis sicca and xerostomia. Among the extraglandular manifestations of Sjogren`s syndrome, renal tubular involvement, especially renal tubular acidosis, is the most often latent or minimally symptomatic. Renal tubular acidosis is estimated to be present in 25~30 percent of the cases. Hypokalemic paralysis may serve as a clinical marker for more severe renal disease in patient who has primary Sjogren`s syndrome with renal tubular acidosis, even though it is a rare manifestation of Sjogren`s syndrome.

        배양기간에 따른 버섯균사체 인삼배양물의 항산화활성

        인삼의 천연배지에 상황버섯(Phellinus linteus), 영지버섯(Ganoderma lucidum) 및 노루궁뎅이버섯(Hericium erinaceum) 균사체를 접종한 후 10, 20, 30, 40, 50일 동안 배양된 인삼발효물 80% 에탄올 추출물에 대한 항산화활성을 살펴보았다. 총 폴리페놀 함량은 149.63~205.91 ㎎/g범위로 영지버섯 균사체 인삼배양물에서 가장 높았다. DPPH법에 의한 항산화활성의 IC??값은 1.16~6.48 ㎎/mL범위로 노루궁뎅이버섯 균사체 인삼배양물에서 가장 높았고, ABTS에 의한 총 항산화력은 1.37~4.03 ㎎ AA eq/g 범위로 상황버섯 균사체 인삼배양물이 가장 높았다. α-Glucosidase 억제활성은 50일 배양된 상황버섯 균사체 인삼배양물에서 92.51%로 가장 높게 나타내었으며, tyrosinase저해활성은 40일 배양된 영지버섯 균사체 인삼배양물에서 13.21%로 가장 높게 나타났다. 인삼을 균사체로 배양할 경우 40일 정도 배양하면 항산화활성이 증가된 인삼제품을 만들 수 있으리라 생각된다. This study was conducted to investigate the antioxidant activities of the cultured ginseng with mushroom mycelia (Phellinus linteus (PL), Ganoderma lucidum (GL), and Hericium erinaceum (HE)) during cultivation periods of 10, 20, 30, 40, and 50 days. The lyophilized powder from the cultured ginseng with mycelia was extracted with 80% ethanol, and then evaluated for antioxidant activities. Total phenolic contents ranged from 149.63 to 205.91 ㎎/g, and the highest value was 80% EtOH extract from the cultured ginseng with GL at 30 days. The highest antioxidant activity (IC??) for DPPH was 1.16 ㎎/mL in the cultured ginseng with HE at 40 days, and total antioxidant activity for ABTS was the highest value of 4.03 ㎎ AA eq/g in PL cultivation at 30 days. α-Glucosidase inhibitory activity was the highest value of 92.51% in EtOH extract from the cultured ginseng with PL at 50 days, and tyrosinase inhibitory activity was highest value of 13.21% in GL cultivation at 40 days. These results suggest that mushroom mycelium cultivation period for enhancement of antioxidant activity might be 40 days.
