      • 비스테로이드성 항염진통제의 투여시기가 구강외과 술 후 통증에 미치는 효과에 대한 연구

        Background: Many studies on efficacy of preemptive analgesia have been processed in different ways. But the value of preemptive analgesia is still controversial. The goal of this study was to compare analgesic effect of an NSAID according to three different administration times for oral surgical pain. Patients and Methods: Using a randomized, parallel-group, single-center, and active-controlled test design, this study was conducted to healthy 80 patients undergoing a surgical removal of an impacted mandibular third molar requiring bone removal. The oral NSAID was first administered 1 hour preoperatively, or 1 hour postoperatively, or no scheduled administration in pre or postsurgery. Whenever patients felt at least moderate pain (score ${\ge}$ 5 on a 10-point scale) after surgery, they were instructed to take the same drug. Pain intensities and times to the first and second onset of postoperative pain from end of surgery were assessed for 24 hours. Results: Of the enrolled eighty subjects in this study, 25 patients were assigned to preemptive, 26 to post-treatment and 29 to no treatment group. The demographic distribution and duration of surgery in the three groups were statistically similar. The mean time to first onset of postoperative pain was significantly prolonged in post-treatment group (277.2 minutes, p < 0.05) compared to preemptive (158.4 minutes) and no treatment group (196.5 minutes). The mean time to second onset of postoperative pain was not significantly different among the three groups. No significant statistical difference was found among the mean pain intensities at the first and second onset of postoperative pain in the three groups. Conclusions: In this small selected group of subjects and limited study design, the analgesic effects of NSAID administered preoperatively were no longer effective for postoperative pain. The results in this population imply that scheduled postoperative analgesics before pain development are adequate for postoperative analgesia without preoperative administration.

      • 청소년동반자가 상담에서 경험하는 어려움에 대한 연구

        본 연구는 청소년동반자가 상담에서 경험하는 어려움에 대해 분석하고 청소년동반자의 사례 개입의 효과성을 높이기 위한 기초적인 자료를 제시하는 데 목적이 있다. 이를 위하여 경기도권에서 활동 중인 청소년동반자를 대상으로 청소년동반자가 상담에서 경험하는 어려움에 대한 설문을 실시하였다. 연구결과 사례 개념화 및 개입에 대하여 각 연차에 따른 상담의 어려움을 느끼는 요소에 차이가 발생하였으며 청소년동반자가 상담에서 경험하는 어려움의 유형으로는 사례 개념화 및 개입, 상담에 대한 부정적 반응, 상담자 특성, 내담자 이해와 수용, 내담자의 비협조적 태도와 관련한 요소를 통하여 확인하였으며 이와 관련하여 청소년동반자의 활동 경력 및 연령으로 세분화하여 분석 조사하였다, This research aims to analyze the difficulties which Youth Companion might experience during the counselling as well as present the basic research materials to heighten the effect in case intervention of Youth Companion. we conducted a survey of 102 of Youth Companion who are working in Gyeonggi-do area about the difficulties in the counselling. Our research object, Youth Companion were classified into their activity career under 3 years. The result of research shows that upon the conceptualization and the intervention of the case, Youth Companion with their activity career under 1 year besides over 1 year under 3 years were having huge difficulties in searching for themselves and setting up the purpose of the counselling. Especially Youth Companion with career under 1 year complained of the problem in setting up the specific counselling, specific exploring, and dealing with a superficial matter of the client. Youth Companion with career over 1 year under 3 years experienced problems in giving aid to the client as well as exploring the client concretely. In matters of the negative inner reaction to the counselor, they were stressed by the counseling and having trouble when they got angry. Except the difficulties they had in common, Youth Companion with career under 1 year is not having huge difficult about the negative inner reaction to the counselling. But Youth Companion with career over 1 year under 3 years got stressed by the counselling and having difficult in feel impotent as a counselor. On the character of the counselor, Youth Companion found it difficult to react emotionally to the client as well as Youth Companion didn"t show their emotion to the client in common. But Youth Companion with career under 1 year didn"t have any trouble in the counselling with the client except the common difficulties which everyone had. Only Youth Companion with career over 1 year under 3 years fall into low spirits after the counselling and had difficulty expressing negative emotion to the client. In connection with the understanding and the acceptance of the client, Youth Companion with career under 1 year had difficult when they didn"t notice the client"s mind or didn"t understand the client"s problem. Youth Companion with career over 1 year under 3years had difficult in understanding the culture of the clients. Therefore, there is an urgent need for various of backing measures such as self education and case management for Youth Companion according to the activity career, support for the supervision and differential reward system according to the activity career as well as case management.

        젓갈(염장발효식품)에서 분리한 호염세균의 생리 및 성장특성

        다섯 종류의 젓갈로부터 다섯 종의 호염성 세균을 분리하였다. 이들의 165-rDNA 염기서열을 분석한 결과 한 종은 Halomonas subglaciescola, 다른 종들은 H. marina에 속하는 것으로 판명되었다. H. subglaciescola, H. marina IN, and H. marina SH-2는 염도가 $3\%$에서 $20\%$에 이르는 넓은 범위의 배양 조건에서, H. marina SQ and H. marina SH-1는 염도가 $8\%$ $19\%$에 이르는 비교적 좁은 범위의 배양 조건에서 성장하는 것으로 나타났다. $15\%$ NaCl을 포함하는 LB 배지에서 배양한 H. subglaciescola, H. marina IN, H. marina SQ, H. marina SH-1, H. marina SH-2의 최대 균체량은 흡광도로서 각각 3.2, 4.5, 4.5, 5.7, 4.2 이었으나 $5\%$의 NaCl을 포함하는 동일 배지에서 최대 균체량은 각각 2.2, 1.1, 0.7, 0.2, 2.4 이었다. 균체의 주사전자현미경 사진에서 균체의 표면에 부정형의 이물질이 부착되 어 있는 것으로 확인되었다. 각 균체의 배양액에서 점성을 갖는 물질을 분리하여 대장균의 배양액에 첨가하였을 때 대장균은 $15\%$ NaCl을 갖는 배지에서 성장하였다. 이것은 호염균의 세포 표면에 부착한 물질이 NaCl의 차단효과를 갖기 때문으로 생각된다. Two species of halophilic bacteria were isolated from five salted seafoods and identified by 16S rDNA sequenc­ing homology. One was identified as Halomonas subglaciescola and other four strains were belong to Halomo­nas marina. The identity of all isolates with standard organisms was above $95\%.$ H. subglaciescola, H. marina IN, and H. marina SH-2 grew in salinity condition from $3%\;to\;18\%$ NaCl but growth of H. marina SQ and H. marina SH-l grew in salinity environment from $8\%\;to\;17\%.$ Maximum biomass of H. subglaciescola, H. marina IN, H. marina SQ, H. marina SH-1, and H. marina SH-2 growing in LB medium containing $15\%$ NaCl were about 3.2, 4.5, 4.5, 5.7, and 4.2, however the maximum biomass in LB medium containing $5\%$ NaCl were about 2.2, 1.1, 0.7, 0.2, and 2.4 as optical density at 660 nm, respectively. In scanning electron micrograph, unknown material (mucus) attached to outer membrane of all isolates was observed. When mucus isolated from halophilic bacterial cell was added to culture of E. coli, E. coli grew in medium containing $15\%$ NaCl.

        대구경 콘크리트 충전형 합성기둥의 전단 설계식 분석

        콘크리트 충전형 합성기둥(Concrete filled steel tube, CFT)은 철근콘크리트, 강구조보다 뛰어난 변형 능력과 강도 갖고 있다. CFT 현장타설말뚝은 지진하중, 토양액상화 등에 의해 큰 국부전단력을 경험하게 되며 이를 방지하기 위해 큰 직경으로 설계되어 진다. 그러나 현행 기준의 전단 설계식은 CFT의 구속효과를 고려하지 못하고 있으며, 상당히 보수적인 설계식을 제시하고 있다. 전단 설계식의 개선을 위한 선행 전단 실험연구가 거의 없으며 그조차 소구경의 실험연구로만 존재한다. 이 연구는 대구경 콘크리트 충전형 합성기둥의 개선된 전단 설계식을 제안하기 위한 기초연구로써, 원형 CFT 기둥 전단의 선행 실험연구와 현행 설계기준 비교분석을 실행하였다. Concrete filled steel tube(CFT) has outstanding deformation capacity and strength in comparison with reinforced concrete or steel tube. CFT drilled shaft, which is developed large shear force due to seismic load and soil liquefaction, is designed as large diameter. However, shear design equations of the current standards do not consider bond stress of CFT and it results in extremely conservative design. Currently, previous studies for improving shear equations scarcely exist and are impossible applied to large CFT drilled shafts since these studies focus on only small scale experimental research. In this study, eventually to propose improving shear equation of large diameter CFT, it is preliminary research to compare and investigate the previous studies and current standards.

        거셋플레이트 연결부가 가새골조의 내진 성능에 미치는 영향

        The purpose of this paper is to improve the inappropriate analysis results when the end of the brace on braced frame is applied as pinned connection in practice. The stiffness of the gusset plate connection on the braced frame has the amount of between pinned and rigid connection, and the analysis model that applies the stiffness of the connection must be used for accurate performance evaluation. In this study, the stiffness of the gusset plate designed by the balanced design procedure are quantified, and applied to the analysis model to simulate the gusset plate connection. The proposed model was verified through nonlinear static analysis (pushover analysis) of SAP2000. The effect of the connection on the seismic performance of the braced frame was analyzed by comparing the proposed model and pinned model. As a result, it was confirmed that the performance of the braced frame was evaluated conservatively in practice, and the ductility was overestimated. Therefore, it is important to consider the connection for accurate and economical performance evaluation.

        대구경 콘크리트 충전형 합성기둥의 전단성능에 관한 해석적 연구

        콘크리트 충전형 합성강관(Concrete Filled steel Tube, CFT)는 우수한 연성과 강도를 발휘하며 건축물의 기둥 및 해양구조물의 교각 등에 적용되고 있다. 현존하는 CFT 전단 설계식은 지나치게 보수적이며 이는 CFT의 경제성과 시공성에 영향을 미친다. 그러나 합리적인 전단 설계식 제안을 위한 실험 연구는 거의 존재하지 않는다. 이 연구는 원형 콘크리트 충전 강관의 개선된 전단 설계식을 제안하기 위한 해석적 연구이다. 선행 연구에서 제시한 원형 CFT 해석 모델을 참고하여 해석 연구를 수행하였으며 해석 모델은 기존 실험 연구 결과를 이용하여 검증하였다. 검증된 모델을 이용하여 변수 연구를 수행하였으며 전단성능에 끝단길이, 콘크리트의 압축강도, 직경두께비가 미치는 영향을 평가하였다. Concrete filled steel tube(CFT), which has superior ductility and strength, is used for building column, bridge piers of ocean structure. Shear design equations of CFT existing in structural design provisions are excessively conservative. It has an effect on constructability and the economics of CFT. However, to suggest the reasonable shear design equation, experimental studies on the shear capacity of CFT have been rarely conducted. This study is analytical research to suggest improved shear design equations of large-diameter concrete-filled steel tubes. This analytical research was conducted to apply finite element analysis model of CFT based on the prior research. It was verified by comparison with prior test results. The verified model was used for parameter studies to estimate the influence of overhang length, concrete compressive strength and diameter-thickness ratio on shear strength.

        쉬어탭 공유 접합부를 갖는 비내진중심가새골조의 내진거동

        특수중심가새골조(Special Concentrically Braced Frames)는 우수한 경제성, 비탄성 변형 능력, 연성 등의 이점 때문에 횡하중 저항 시스템으로써 다양한 구조물에 적용하고 있다. 이러한 이점으로 국내 외 연구진의 특수중심가새골조에 대한 연구가 지속적으로 진행되어 설계법과 기준이 비약적으로 발전한 반면에, 과거에 설계된 중심가새골조의 내진 성능에 대한 연구는 미약하다. 특히, 1988년 이전에 설계된 중심가새골조는 현재 기준과 비교하여 상대적으로 적은 설계요구를 반영하였기 때문에 내진 거동에 대한 평가가 시급한 실정이다. 본 연구에서는 이러한 중심가새골조를 비내진중심가새골조(Non-Seismic Concentrically Braced Frames)라 칭하고, 워싱턴대학교에서 실시된 사례 조사를 토대로 비내진중심가새골조의 대표적인 접합부인 쉬어탭 공유 용접접합부를 갖는 골조의 내진 거동에 대한 유한요소모델을 제시하고 검증하였다. 이 모델을 이용하여 특수중심가새골조와 대비되는 비내진중심가새골조의 연성 능력, 강도, 비탄성 변형 능력에 대해 규명하여 내진 성능을 평가하였다. Special concentrically braced frames(SCBFs) have distinctive advantages in considerable seismic performance, which make engineers widely use SCBFs as lateral-load resisting systems in buildings and have researchers to develop SCBFs design methods. Compared to the extensive research of SCBF, comparatively little information is currently available on the performance of SCBFs designed and constructed before the early 1990's. Prior to 1988, concentrically braced frames(CBFs) design requirements were substantially less restrictive. As a result, many existing structures designed to these requirements may not ensure ductility and pose a significant concern in current buildings. In this study, these older frames are referred as non-seismic braced frames(NCBFs). In order to investigate the seismic behavior of NCBFs, finite-element(FE) models of SCBF and NCBF were suggested and verified using case investigation of NCBF conducted on the University of Washington. Using these models, the seismic behavior of NCBF with shared welding shear tab, which is the representative of the types of connections, was established and compared with the seismic performance of SCBF.
