        유리체가 전방을 채운 난치성 녹내장 환자에서 유리체절제술 후 시행한 섬유주절제술의 임상경과

        목적: 유리체가 전방을 채우고 있는 난치성 녹내장 환자에서 25게이지 무봉합 경결막 유리체절제술 후 시행한 섬유주절제술의 1년 임상경과를 알아보고자 하였다. 대상과 방법: 전방 내 유리체가 존재하고 최대약물치료에도 안압이 조절되지 않는 위수정체와 무수정체 녹내장 환자들을 대상으로 유리체절제술을 시행하고 순차적으로 마이토마이신C를 이용한 섬유주절제술을 시행하였다. 섬유주절제술 후 최소 12개월 이상 경과 관찰한 환자들의 의무기록을 후향적으로 분석하였다. 완전 수술 성공은 안압하강제 사용 없이 안압이 18 mmHg 이하이고, 수술 전에 비해 20% 이상 하강한 경우이며, 제한적 수술 성공은 안압하강제 사용과 관계없이 안압이 18 mmHg 이하이고 수술 전에 비해 20% 이상 하강한 경우로 정의하였다. 결과: 총 7명 8안(위수정체군 4명 4안, 무수정체 3명 4안)이 본 연구에 포함되었다. 섬유주절제술 후 12개월에 제한적 수술 성공률은 87.5%, 완전 수술 성공률은 62.5%였다. 섬유주절제술 전 평균 안압은 28.1 ± 3.5 mmHg에서 수술 후 마지막 방문 시 15.0 ± 3.7 mmHg로 유의하게 감소하였다(p=0.012). 섬유주절제술 전 사용한 평균 안압하강제 수는 4 ± 0개에서 수술 후 마지막 방문 시 1.5 ± 2.1개로 유의하게 감소하였다(p=0.010). 모든 증례에서 망막박리, 유리체출혈, 낭성황반부종, 유리체 감돈은 발생하지 않았다. 결론: 유리체가 전방을 채우고 있는 난치성 녹내장 환자에서 경결막 무봉합 유리체절제술 후 시행한 섬유주절제술은 효과적이고 안압감소 방법임을 알 수 있었다. Purpose: To evaluate the 1-year clinical outcome of subsequent trabeculectomy following 25-gauge transconjunctival sutureless vitrectomy in refractory glaucoma with vitreous filling of the anterior chamber. Methods: This study was a retrospective and consecutive case series study. We reviewed the medical records of pseudophakic and aphakic glaucoma patients with vitreous filling of the anterior chamber who underwent subsequent trabeculectomy with mitomycin C (MMC), following 25-gauge transconjunctival sutureless vitrectomy. All patients had been followed up for more than 12 months. Complete surgical success was defined as an intraocular pressure (IOP) ≤18 mmHg and IOP reduction ≥20% without glaucoma medication. Qualified surgical success was defined as IOP ≤18 mmHg and an IOP reduction ≥20% with or without glaucoma medication. Results: Eight eyes of seven patients (four eyes of four patients with pseudophakic glaucoma and four eyes of three patients with aphakic glaucoma) were included in this study. The cumulative probability of qualified success was 87.5%, and the cumulative probability of complete success was 62.5% at 12 months after trabeculectomy. The mean IOP decreased from 28.1 ± 3.5 mmHg preoperatively to 15.0 ± 3.7 mmHg at the final visit (p = 0.012). The mean number of glaucoma medications decreased from 4 ± 0 to 1.5 ± 2.1 at the final visit (p = 0.010). Complications including retinal detachment, vitreous hemorrhage, cystoid macular edema, and vitreous incarceration into the fistula were not observed. Conclusions: Transconjunctival sutureless vitrectomy and subsequent trabeculectomy with MMC is an effective method for controlling IOP in pseudophakic and aphakic glaucoma with vitreous filling of the anterior chamber.

        양안성 미만성 포도막 멜라닌세포 증식증으로 나타난 샘암종

        목적: 양안 미만성 포도막 멜라닌세포 증식증으로 나타난 위 샘암종 증례를 보고하고자 한다. 증례요약: 78세 남자가 3개월 전 발생한 양안 시력저하 주소로 내원하였다. 당뇨 외 특이 병력은 없으며, 4년 전 양안 백내장수술을 받았다. 교정시력은 우안 안전수지, 좌안 20/160로 측정되었고, 안압 및 전안부는 정상이었다. 양안 안저에서 여러 개의 둥근 모양의 반점이 후극부에 관찰되었고, 망막하 망막색소상피 과색소 및 탈색소 병변이 동반된 삼출망막박리가 관찰되었다. 빛간섭단층촬영에서 색소성 병변은 시세포와 망막색소상피 사이에서 과반사를 보이는 망막하 삼출물로 확인되었고 망막하액과 망막내액이 관찰되었다. 양안 미만성 포도막 멜라닌세포 증식증으로 진단 후 영상학적 전신 평가를 시행하였다. 복부 컴퓨터 전산화단층촬영에서 악성종양으로 의심되는 종괴가 다수 관찰되었다. 상부위장관 내시경 조직검사를 시행 후 샘암종으로 진단되었다. 결론: 원발악성종양에 의해 이차적으로 발생한 양안 미만성 포도막 멜라닌세포 증식증은 다발성의 색소성 망막하 삼출물을 동반한 삼출망막박리로 나타나며, 이와 같은 안저 소견을 보이는 환자에서는 전신검사를 통해 악성종양에 대한 평가가 필요하다. Purpose: We report bilateral, diffuse, uveal melanocytic proliferation caused by a stomach adenocarcinoma. Case summary: A 78-year-old male complained of visual impairment 3 months in duration. He had diabetes and had undergone cataract surgery on both eyes 4 years prior. His best-corrected visual acuity was counting fingers in the right eye and 20/160 in the left eye. Both intraocular pressures were normal. The anterior segments yielded no specific findings. The maculae exhibited multiple, round, patchy, pigmented or depigmented lesions with exudative retinal detachment. Fluorescein angiography revealed multiple hyperfluorescent lesions. Optical coherence tomography revealed that the hyper-reflective exudates lay between the neurosensory retina and the retinal pigment epithelium. We diagnosed bilateral, diffuse, uveal melanocytic proliferation and performed a systemic evaluation. Computed tomography revealed several mass lesions in the lung, stomach, and lymph nodes; these appeared to be malignant. An esophagogastroduodenoscopic biopsy confirmed that the lesions were adenocarcinomas. Conclusions: A bilateral, diffuse, uveal melanocytic proliferation induced by a primary malignant tumor exhibited multiple, pigmented subretinal exudates associated with exudative macular retinal detachment. In patients with such findings, it is necessary to evaluate the malignant tumor status of remote organs.

        The manufacturing processes of the head hardened rails and the principles of the heat treatment for carbon steels are introduced in this paper. To get the good wear resistance of the rail, many kinds of microstructure had been developed for the rail head hardening. One of these is the tempered martensitic structure, which is very hard but brittle because of Quenching-Tempering Process. Another is the fine pearlitic structure by Slack Quenching. Now Banitic structure steels are emerging as a potential new material to replace eutectoid pearlitic steels for rail. The main reason of this change is due to the limitaion of pearlitic microstructure with regard to mechanical properties and wear resistances of railway rails.

        This report contains the first-year outcome of a three-year joint project between South Korea and China - KEI(Korea Environment Institute) and CAEP(China Academy for Environmental Planning), adopted in the 21st ROK-China Joint Committee on Environmental Cooperation. By analyzing and comparing the two countries’ institutions regarding environmental governance and public participation, we seek to find relevant policy implications to advance institutional developments, and derive further cooperation projects. To this end this report presents a general survey of the relevant institutions in each country and case studies to portray the actual implementation of the institutions. For the surveys we categorized the institutions in three groups of (1) access to information, (2) public participation in decision-making process, and (3) access to justice, following the framework of the Aarhus convention. As for the access to information in Korea, the Official Information Disclosure Act in administrative law led to a substantial increase in the amount of governmental information disclosure, and environmental laws have basic rules in information provision and access and more specific obligations of large-scale pollutant dischargers to disclose relevant information. In terms of the participation in decision-making process, Korea ensures public participation in major policy decisions by obliging procedures as public hearings. Major planning in environmental area requires public hearing, and the environmental impact assessment procedures are operated under detailed provisions on public briefings and hearings. Furthermore, based on the Local Autonomy Act several forms of direct democracy including referendum and recall were made available, so that people can have more diversified channels to participate in the public matters. Regarding the access to justice, the environmental damage relief system and the environmental dispute adjustment system are taking effect to alleviate the practical difficulties of damage relief and dispute resolution in environmental matters. Regarding the case studies to illustrate the actual implementation of governance institutions in Korea, we presented two cases related to controversial locational decisions of environmental facilities and initiation of referendum, in the cities of Samcheok and Namhae. Environmental facilities often entail degradation of environmental qualities in the area, leading to conflict among the residents, between the supporters who welcome the economic gains and the opponents who condemns the environmental loss. In order to reach a consensus and obtain understanding among the residents to make the best decision, it is crucial to provide the relevant information, hear the public opinion, and let the public participate in the locational decision-making. The referendum in Korea also can be a useful tool to base the local government’s decision on the public opinion. As a response to construction plan of power plant in the area, the Namhae city successfully conducted the referendum in early stage of decision-making and cancelled the construction project in reflection of the public opinion, whereas the conflict prolonged much longer in the Samcheok city where the city government was less eager to gather the public opinion before the decision-making. China is producing much legislative outputs for public participation in environmental protection in recent years. Regarding the access to information, the Environmental Information Disclosure Method is declared in 2007 to stipulate the extent, procedures, and responsibilities of information disclosure. The Environmental Protection Act dictates the national and local governments to regularly publish the state of environment, and the major pollutant dischargers to disclose their information on emission and violation. In terms of participation in decision-making process, the administrative law stipulates the public hearings to be held for major policy decisions, and the environmental impact assessment is based on a number of rules and guidelines describing the detailed procedures and methods for the public participation such as public hearing. Regarding the access to justice, China is giving the general public more authorities in monitoring and penalizing the environmental violations. For the case studies of China, two localities are illustrated for their implementation of the public participation system in local scale. The city of Jiaxing takes a multi-faceted approach: citizens’ reporting of environmental violation is encouraged with commendations and monetary rewards; roundtable meeting is held for public participation in major decision-makings; citizen counselors are recruited to have a voice in deciding penalties for environmental violators and in endorsing construction projects; the city government organizes the “Eco-Civilization Promotion Group” for raising environmental awareness, and makes efforts for better public communication.
