        Helicobacter pylori 감염과 사회경제적 요인에 대한 연구

        목 적: 우리 나라 청소년기 소아의 H. pylori 감염 유병율을 조사하고, 사회경제적 요인이 청소년의 H. pylori 감염에 어떠한 영향을 미치는지 알아보고자 하였다. 대상 및 방법: 1996년 10월 부천시에 살고 있는 10~15세의 532명(남아 285명, 여아 247명)의 소아를 대상으로 채혈을 시행하여 혈청학적인 방법(serum IgG antibody)으로 H. pylori 감염을 진단하였다. 설문지를 통하여 성별, 연령, Hollingshead index에 따라 분류한 사회경제적 수준, 주거형태(전세 또는 자가), 혼잡지수(crowding index)를 조사하였는데, 532명 중 375명(70.5%, 남:여=205:170)의 설문이 회수되었다. Hollingshead index는 부모의 교육정도와 직업을 고려하여 구분하였고 crowding index는 식구수/방수로 구하였다. 각각 단변수 회귀분석 후 의미있는 결과를 모아 다중회귀분석을 시행하였다. 결 과: H. pylori 유병률은 남아에서 17.1% (32/205), 여아에서 16.5% (28/170)로 통계적으로 유의한 차이는 없었다(P=0.88). 10~11세, 12~13세, 14~15세의 세 연령군으로 나누어 비교한 결과, 연령이 증가할수록 H. pylori 유병률은 각각 10.3% (7/68), 15.9% (25/157), 20.7% (31/150)로 유의한 증가를 보였다(P=0.05). 사회경제적 수준을 Hollingshead index로 구하여 세 군으로 나누어 비교한 결과, 사회경제적 수준이 높을수록 H. pylori 양성은 각각 20.0% (23/115), 16.0% (39/244), 6.3% (1/16)로 감소하는 역비례 관계를 보였으나 통계적으로 유의한 차이는 아니었다(P=0.16). 전세와 자가를 비교했을 때 H. pylori 양성은 각각 15.7% (22/140), 17.5% (41/235)로 유의한 차이가 없었다(P=0.66). 혼잡지수(crowding index: 식구수/방수)가 1.5 미만일 때 H. pylori 양성은 16.0% (26/163), 1.5 이상일 때 17.5% (37/212)로 유의한 차이가 없었다(P=0.70). 다중회귀분석 결과 H. pylori 감염에 대해 연령은 odds ratio 2.2 (95% confidence interval 0.9~5.4), Hollingshead index에 의한 사회경제적 수준은 odds ratio 3.6 (95% confidence interval 0.5~28.9)이었다. 결 론: 부천시 청소년기 소아의 H. pylori 감염유병률은 16.8%이며, 부모의 교육정도와 직업에 따른 사회경제적 수준이 가족내 혼잡도, 주거형태보다 감염에 더욱 영향을 미치는 것으로 보인다. Purpose: This study was conducted to evaluate the association between H. pylori infection and socioeconomic status and to determine the current prevalence of H. pylori infection in Korean adolescents. Methods: A structured questionnaire was sent to the children's parents to obtain demographic information on the parents and environmental information. Of the 532 questionnaires sent out, 375 (70.5%; 170 girls and 205 boys) were returned. Their ages ranged from 10 to 15 years (mean, 12.9 years). After collecting blood samples, we measured serum IgG antibody to H. pylori using ELISA method. The association of risk factors such as age, sex, socioeconomic class, type of house, and crowding index with H. pylori infection were analyzed by multiple regression analysis. Socioeconomic status was estimated from the parents' education and occupation using a modified Hollingshead index. Results: The prevalence rate of H. pylori infection was 16.8% (63/375). It increased with age (10.3% at 10~11 years, 15.9% at 12~13 years, and 20.7% at 14~15 years). The H. pylori infection was inversely related to the socioeconomic class (6.3% for the upper class, 16.0% for the middle class, and 20.0% for the lower calss). Crowding condition and type of house did not affect significantly on seroprevalence of H. pylori infection. After logistic regression, we found that the odds ratio for age was 2.2 (95% confidence interval 0.9~5.4), and for socioeconomic status, 3.6 (95% confidence interval 0.5~28.9). Conclusion: The prevalence of H. pylori infection in Korean adolescents was 16.8%. It related inversely to socioeconomic status but was not statistically significant. Socioeconomic status based on parents' education and occupation seemed to affect more on H. pylori seroprevalence than crowding or type of house did.

      • Mollaret Meningitis로 진단한 무균성 뇌막염 1례

        저자들은 14세 남아에서 4년간 3차례 발병한 Mollaret meningitis 1례를 경험하였기에 문헌 고찰과 함께 보고하는 바이다. 아울러 정확한 진단과 감별을 위해 뇌척수액의 세포학적 검사, herpes virus 동정, 뇌자기공명촬영 추적 검사 등이 필요할 것으로 사료된다. Mollaret meningitis is a rare syndrome first described by Mollaret in 1944, which is reccurent aseptic meningitis with characteristic clinical and spinal fluid cytologic findings. No etiology has been established. Several authors suggested the association with herpes virus infection, some found intracranial epidermoid tumor eventually in patients diagnosed of Mollaret meningitis. We experienced a case of 14-year old male who had 3 episodes of recurrent aseptic meningitis during four years. The patients initially presented with clinical and laboratory feature of bacterial meningitis, however, extensive serological investigation and cerebrospinal fluid analysis failed to reveal a specific cause. Immune system studies were unremarkable. Cranial computed tomography performed during the attack and magnetic resonance imaging when the patient was asymptomatic were both normal. A tentative diagnosis of Mollaret meningitis was established at the 3rd episode. We report this case with a review of related literatures.

        관계형 DBMS를 이용한 XML 질의 처리 시스템 XPERT의 개발

        본 논문은 관계형 DBMS를 사용한 XML 질의 처리 시스템 XPERT(XML Query Processing Engine using Relational Technologies)의 개발 내용을 소개한다. 본 논문의 XPERT에서 제안하는 XML 저장 방식은 XML 문서를 여러 구성 성분별로 나누어 관계형 테이블에 저장하는 분할(decomposition 또는 shredded) 방식을 사용하고, 분할된 관계형 테이블을 바탕으로 XML 질의를 SQL로 변환하고, 관계형 DBMS에서 변환된 SQL을 실행하여 결과를 반환하는 방식을 사용한다. 제안한 XQuery 변환 방식은 먼저 XQuery의 구문 분석을 통하여 AST(Abstract Syntax Tree)를 생성하고, AST를 순회하면서 SQL 문장을 생성한다. 생성된 SQL 문장은 XML 문서의 경로를 사용함으로써 XQuery 연산의 조인 횟수를 감소시키며, 계층적 정보 검사나 문서에 내재된 결과의 순서를 지키기 위하여 사용하는 순서 정보는 Dewey 번호를 효과적으로 사용한다. 특히 XQuery의 XPath와 FLWOR 연산을 SQL로 변환하고 실행하는 효과적인 방법을 제시하고 제안된 XPERT시스템의 프로토타입을 개발하여 그 기능을 평가한다. This paper introduces the development XPERT(XML Query Processing Engine using Relational Technologies) which is based on relational model. In our system we have used a decomposed approach to store XML files in relational tables. XML queries are translated to SQLs according to the table schema, and then they are sent to the relational DBMS to get the results back. Our translation scheme produces AST(Abstract Syntax Tree) by analyzing XQuery expressions at first. And on traversing AST proper SQLs are generated. Translated SQLs can reduce the number of joins by using path information and utilize dewey number to preserve document originated orderings among compoments in XML. In addition we propose the efficient algorithms of XPath and XQuery translation. And finally we show the implementation of our prototype system for the functional evaluations.

        도서관의 시맨틱 기반 사물인터넷(IoT) 적용에 관한 연구

        본 연구에서는 도서관에 사물인터넷(Internet of Things) 개념을 적용하여 사람, 사물, 데이터 등의 정보가 서로 유기적으로 연계되는 생태학적인 네트워크 공간으로서의 해석을 시도하였다. 먼저 도서관의 물리적 공간에 존재하는 다양한 사물과 인터넷 공간에 존재하는 데이터를 사물인터넷 요소로 정의하였다. 다음으로 다양한 사물들이 상이한 메타데이터와 통신 프로토콜을 통해 정보를 교환하고 유용한 정보로서 서로 통합하기 위한 시맨틱 웹 기술 적용을 제안하였다. 마지막으로 인터넷 공간속에서 다양한 사물인터넷의 요소들이 실시간으로 공유하는 정보를 도서관 관리 및 이용자 서비스 등 특정 목적에 활용될 수 있는 방안으로서 사물인터넷 모델을 제시하고자 하였다. This study aims to apply the concept of Internet of things to library and interpret a library as ecological space where human, thing and data are communicating and organically networking through internet. First, we defined various things existing in physical spaces of the real world and internet spaces as elements of Internet of Things. Then we proposed to apply a semantic web platform to integrate different meta-data formats and communication protocols from things. Finally, we provide the model of Internet of Things based on Semantic web to utilize a variety of information from things for library management and user service.

        Removal and Growth Inhibition of Red-tide Organisms by Blue-Min Treatment

        Blue-Min was initially developed as an adsorbent for harmful gas removal and recently improved to apply to livestock, agriculture and aquaculture as an assistant feed. In the Blue-Min treatment, growth of harmful algae (Cochlodinium polykrikoides and the others causing the red-tide in the ocean) were inhibited below 10% in comparison with control and coagulation removal of harmful alge with Blue-Min treatment was more efficient than that of yellow loess treatment. It would be expected that the Ble-Min can be useful for the extirpator against the red-tide organisms and restrain the toxic algal growth around the fish aquaculture using the assistant feed. Recently, its utility has become to be diverse as it was revealed that aquaculture productivity increase by its application and, in addition, that it improve the water quality or sediment conditions in the aquaculture of Chinese White Shrimp. When Blue-Min was treated with the proper dose, the growth inhibition of Microcystis aeruginosa and lsochrysis galbana, which are typical red-tide organisms in freshwaters and food organisms in aquaculture, respectively, were less than that of marine red-tide organisms, while their growth slightly increased with low concentration treatiment. In addition, polyunsaturated fatty acids (PUFA) content of I. galbana slightly increase with the Blue-Min treatment. Through our research, the Blue-Min has diverse and comples function against various biological organisms and is proved as a biological activator or depressor.

        SSP 시나리오별 굴 양식 생산량 예측력 비교

        Climate change is a major global problem. Oysters, one of the most representative farmed fish in Korea, are attracting attention as candidates for blue carbon, an alternative to carbon neutrality. This study is analyzed by the SSP scenarios to determine the impact of oyster aquaculture production according to climate change. Based on the analysis, future productions of oysters are predicted by the SSP scenario. Significant differences by the SSP scenario are confirmed through predictive power tests among scenarios. Regression analysis was conducted from January 2001 to December 2014. As a result of the analysis, water temperature, water temperature quadratic term, salinity, salinity quadratic term, and month × water temperature cross term were estimated as significant variables. Oyster production which is predicted by the SSP scenario based on the significant variables from 2015 to 2022 was compared with actual production. The model with the highest predictive power was selected by RMSE and MAPE criteria. The predictive power was compared with the MDM test to determine which model was superior. As a result, based on RMSE and MAPE, the SSP1-2.6 scenario was selected as the best model and the SSP1-2.6, SSP2-4.5, and SSP3-7.0 scenarios all showed the same predictive power based on the MDM test. In conculusion, this study predicted oyster aquaculture production by 2030, not the distant future, due to the short duration of the analytical model. This study was found that oyster aquaculture production increased in all scenarios and there was no significant difference in predictive power by the SSP scenario.

        북한의 사이버공격과 대응방안에 관한 연구

        This study aims to present the necessary elements that should be part of South Korea’s National Defense Strategy against the recent North Korean cyber-attacks. The elements proposed in this study also reflect the recent trend of cyber-attack incidents that are happening in the Unites States and other countries and have been classified into the three levels of cyber incidents: cyberwarfare, cyberterrorism and cybercrime. As such, the elements proposed are presented in accordance with this classification system. In order to properly take into account the recent trend of cyber-attacks perpetrated by North Korea, this paper analyzed the characteristics of recent North Korean cyber-attacks as well as the countermeasures and responses of South Korea. Moreover, by making use of case studies of cyber-attack incidents by foreign nations that threaten national security, the response measures at a national level can be deduced and applied as in this study. Thus, the authors of this study hope that the newly proposed elements here within will help to strengthen the level of Korea’s cyber security against foreign attacks, specifically that of North Korea such as the KHNP hacking incidents and so on. It is hoped that further damage such as leakage of confidential information, invasion of privacy and physical intimidation can be mitigated.

        Anti-Notch 구조를 적용한 Spoke Type IPMSM의 출력 및 리플 개선 연구

        This paper applied the Anti-Notch structure to the stator to improve the output and ripple of the spoke type IPMSM (Spoke type inner permanent magnet synchronous motor). Since the permanent magnet of the spoke type IPMSM is located in the radial direction, there is an advantage in that the magnetic flux in the air gap is concentrated and the power is increased. However, the Spoke type IPMSM has the disadvantage of lowering the output and generating noise and vibration because torque ripple and cogging torque are largely due to the magnetic resistance difference of the air gap. To improve this, the 2-D Finite Element Method (FEM) was used, and a design plan to improve output, torque ripple, and cogging torque according to the number of Anti-Notch was studied by applying the Anti-Notch structure to the stator of the existing designed model.

        혈액내과 입원 환자의 낙상 위험 요인과 환자 결과

        Purpose: The goal was to use electronic health records to identify factors and outcomes associated with falls among patients admitted to hematology units. Methods: This retrospective case-control study included data from a tertiary university hospital. Analysis was done of records from 117 patients with a history of falls and 201 patients with no history of falls who were admitted to the hematology unit from January 1, 2013 to December 31, 2014. Risk factors were analyzed using hierarchical logistic regression; patient outcomes were analyzed using multiple logistic regression, Cox proportional hazards regression, and multiple linear regression. Results: Clinical factors such as self-care nursing (OR=4.47, CI=1.64~12.11), leukopenia (OR=6.03; CI=2.51~14.50), and hypoalbuminemia (OR=2.79, CI=1.31~5.96); treatment factors such as use of narcotics (OR=2.06, CI=1.01~4.19), antipsychotics (OR=3.05, CI=1.20~7.75), and steroids (OR=4.51, CI=1.92~10.58); and patient factors such as low education (OR=3.16, CI=1.44~6.94) were significant risk factors. Falls were also associated with increased length of hospital stay to 21.58 days (p<.001), and healthcare costs of 17,052,784 Won (p<.001). Conclusion: These findings can be a resource for fall prevention education and to help develop fall risk assessment tools for adults admitted to hematology units.

        텍스트마이닝을 활용한 식품유통 플랫폼에 대한 소비자 인식 분석 : 토픽모델링 기법을 중심으로

        COVID-19로 인해 비대면 문화가 확산되면서 소비자들의 식품유통 플랫폼 이용률이 증가하고 있다. 본 연구는 식품유통 플랫폼의 최근 2년간 소비자 리뷰를 토픽모델링 기법으로 분석하였다. 이를 통해 식품유통 플랫폼의 대표 3사에 대한 소비자 리뷰의 차이와 코로나 발생으로 식품유통 플랫폼에 대한 소비자 인지하는 키워드에 차이가 존재하는지 확인하였다. 이를 위해 2019년, 2020년 총 2개 연도의 ‘마켓컬리’, ‘쿠팡’, ‘SSG’ 어플리케이션 리뷰 총 17,954건의 데이터를 App Store와 Google Store에서 크롤링하여 수집하였다. 연구결과, 2019년에 비해 2020년의 리뷰의 개수 및 평점 평균이 유의미하게 증가하였으며 각 플랫폼별 증가·감소한 단어와 소비자들이 인지하고 있는 키워드의 차이를 확인하였다. 코로나 발생 이후 온라인 식품유통 플랫폼의 전반적인 이용률과 만족도는 크게 상승하였고, 코로나로 인한 환경 변화로 인해 플랫폼별 서비스 차이를 소비자들이 인지하고 있음을 확인하였다. 본 연구는 식품유통 플랫폼 이해관계자들에게 소비자의 니즈 및 불만 사항에 대한 유용한 정보를 제공하였다. 사회적 이슈에 대한 소비자들의 관심과 행동의 영향력을 파악하여 소비자들의 니즈와 사회 참여 욕구의 발견으로 플랫폼 마케팅을 위한 기초자료로 활용될 수 있다. As the non-face-to-face culture spreads due to COVID-19, consumers use of food distribution platforms is increasing. Based on consumer reviews, this study analyzed the difference in consumer perception of the three representative food distribution platforms (Market Kurly, Coupang, and SSG) and the changes in consumer perception of food distribution platforms before and after COVID-19 using topic modeling. The results confirm that the overall utilization and satisfaction of the food distribution platform increased after the outbreak of COVID-19 in 2020, and changes in word frequencies indicate consumers’ awareness of the difference in services by platform and the impact of COVID-19. This study provides useful information on consumer needs for food distribution platform stakeholders, and can be used as basic data for platform marketing since it informs of consumers social participation needs by capturing the influence of consumers interest in social issues and consequent behavior on the use of food distribution platforms.
