Due to high power ratings and low conduction loss, the IGBT has become more attractive in switching power supplies However, its turn-on and turn -off characteristics cause severe switching loss and switching frequency limitation. This paper proposes 24㎾, 48V, high efficiency half-bridge DC-DC converter using paralleled IGBT-MOSFET switch concept, where each of IGBT and MOSFET plays its part during on -periods and switching instants. The switching loss is analyzed by using the linearized model and the opteration of the converter are investigated by simulation results.<br/> <br/>

민영의료보험 가입이 일부 경ㆍ요추부 염좌 환자의 입원에 미치는 영향

이 연구는 2008년 6월 1일부터 2008년 8월 31일까지 광주ㆍ전남지역에 소재하고 있는 20개 의료기관(병원 3, 의원 17)에서 신경학적 증상이 없는 단순 경ㆍ요추부 염좌 진단을 받고 입원 및 외래치료를 받은 환자 449명(입원=384명, 외래=85명)을 대상으로 민영의료보험 가입이 입원에 미치는 영향을 규명하기 위해 시행되었다. 구조화된 설문지를 이용하여 조사한 응답 결과에 대해 단일변량분석을 시행하고 입원여부와 유의한 관련성이 있는 변수를 독립변수로 하여 다단계 로지스틱회귀분석을 실시한 결과와 그 함의는 다음과 같았다. 관련된 요인을 통제한 후에도 민영의료보험 가입여부의 입원에 대한 비차비는 3.31(95%CI 1.41-9.58)로서 민영의료보험 가입은 입원여부에 독립적으로 유의한 영향을 미치는 요인임이 확인되었다. 그 밖에 조사대상 환자군의 입원에 영향을 미치는 요인들을 변수군별로 보면 환자의 사회인구학적 요인 변수군의 결정계수(R²)가 0.226로 가장 컸고 다음으로 의료기관 및 의료인 특성 변수군의 R²=0.122, 질병 특성 변수군의 R²=0.108 등이었으며 민영의료보험의 가입여부의 R²=0.013으로 결정계수 값이 크지는 않았다. 이 연구는 경증질환 진단을 받은 환자를 대상으로 현재 중소규모 의료기관에서 진료중인 환자를 대상으로 민영의료보험 가입여부와 입원여부의 관련성을 구명한 국내 최초의 실증연구로서 민영의료보험의 가입이 초래하는 도덕적 해이의 존재와 정도를 일정부분 규명하였다는 점에서 그 의의를 찾아 볼 수 있다. 전 국민 의료보험제도 하에서 이는 민영의료보험 자체 뿐 아니라 건강보험에 대한 외부효과도 존재할 수 있음을 시사하는 결과로서 이를 내부화하기 위한 적절한 조치의 마련이 필요할 것으로 판단된다. Background: In Korea, private health insurance has neglected to induce externality on national health insurance by moral hazard. Therefore, we conducted this study in order to explore the influence of private health insurance on unnecessary medical utilization among patients with cervical or lumbar sprain. Method: The study examined a population of 449 patients (admission, 384; out-patient; 85) diagnosed with simple cervical or lumbar sprain without neurological symptoms at 20 small hospitals or clinics in Gwangju and Jeollanam provinces from Jul. 1 to Aug. 31 2008. The data were collected using structured, self-administrated questionnaire which collected information such as whether or not the patient was admitted (as a dependent variable), whether or not they had private health insurance (as a independent variable), and covariates such as socio-demographic characteristics, the factors related to the sprain, and characteristics of the insurance provider. Results: From hierarchical multiple logistic regression analysis, it was found that the admission rate of patient with private health insurance was higher than that those without it (Odds ratio=3.31, 95% Confidence interval; 1.14-9.58), meaning that private health insurance was an independent factor influencing the admission of patients with these conditions. Other determinants of admission were patient age and physician referral. Conclusions: This study is the first empirical study to explore the influence of private health insurance on inducing moral hazard in admission services, specifically among patients with cervical or lumbar sprain. Regulation of benefits provided by private health insurance may be necessary, as the effect of this moral hazard may mean existence of externality.
