비구 골절에서 인공 고관절 전치환술의 적용: 급성기와 지연기

The incidence of acetabular fractures in the elderly has increased because of the increasing elderly population. To determine the treatment plan for acetabular fractures, the patient’s age, gait ability, presence or absence of osteoporosis and osteoarthritis, underlying disease, and fracture pattern should be considered. The application of total hip arthroplasty for acetabular fractures with the proper indications can be expected to have a good prognosis. In this paper, the application of total hip arthroplasty as a treatment method for acetabular fractures is divided into acute and delayed phases. 최근 노인 인구가 증가함에 따라 노인의 비구 골절 발생 빈도 또한 증가하고 있다. 이러한 비구 골절의 치료 계획을 결정하기 위해서 환자의 나이, 보행 능력, 골다공증 및 골관절염의 유무, 동반된 기저 질환, 골절의 형태 등을 참고해야 한다. 적절한 적응증을 바탕으로 한 비구 골절의 치료로써, 인공 고관절 전치환술을 적용하는 것은 좋은 예후를 기대할 수있다. 이 논문에서는 비구 골절의 치료 방법으로 인공 고관절전치환술의 적용을 급성기와 지연기, 각각의 경우로 나누어이에 대해 논의하도록 하겠다.

Fungal cell walls have essential roles in the life of the fungi life. The main components of fungal cell walls are carbohydrates, which are interconnected into a lattice. Considerable biochemical and genetic effort has been directed at the identification and characterization of the steps involved in cell wall biosynthesis. Structural studies show that the β-1,6-glucan plays a central role in wall structure, attaching not only mannoproteins via their glycosylphosphatidylinositol(GPI) remnant but also other cell wall polysaccharides,β-1,3-glucan and chitin. Genetic approaches have identified genes that upon disruption result in β-1,3-glucan, β-1,6-glucan and chitin defects of varying severity, often with reduced growth or lethality. Studies on the biosynthesis ofβ-1,6-glucan and cell wall proteins strongly indicated that the secretory pathway is heavily involved in the biosynthesis of cell surface components and in their transport to the cell surface. Based on knowledge of β-1,3-glucan and chitin synthesis, it is likely that the bulk of B-1,6-glucan polymer synthesis occurs at the cell surface, but requires key prior intracellular events. With the recent availability of new tools, including proteomics, the field is poised for a convergence of biochemical and genetic methods to identify and characterize the biochemical steps in the synthesis of the cell wall components.

In this paper, flicker noise characteristic and channel hot carrier degradation of NMOSFETs with plasma nitrided oixde (PNO) and thermally nitrided oxide (TNO) are analyzed in depth. Compared with NMOSFET with TNO, flicker noise characteristic of NMOSFET with PNO is improved significantly because nitrogen density in PNO near the Si/$SiO_2$ interface is less than that in TNO. However, device degradation of NMOSFET with PNO by channel hot carrier stress is greater than that with TNO although PMOSFET with PNO showed greater immunity to NBTI degradation than that with TNO in previous study. Therefore, concurrent investigation of the reliability as well as low frequency noise characteristics of NMOSFET and PMOSFET is required for the development of high performance analog MOSFET technology.

Purpose In malignant melanoma, recurrence is often observed in distant areas from the primary site. While FDG PET is asensitive imaging for detecting malignant lesions, the role of FDG PET in posttreatment surveillance period has not beeninvestigated sufficiently. The aim of this study was to evaluate the value of PET during posttreatment surveillance in melanoma. Methods A total of 76 melanoma patients who underwent FDG PET during surveillance period after completion of the firsttreatment were retrospectively enrolled. PET scans were grouped according to the purpose and clinical situations, routinesurveillance, or evaluating clinical suspicion. Final diagnosis of recurrence was determined by complete clinical evaluation orlong-term follow-up. In each situation, the diagnostic role of FDG PET was assessed. Results A total of 143 scans of 76 patients were analyzed: 51 for clinical suspicion and 92 for routine surveillance. In the clinicalsuspicion group, PET correctly diagnosed non-recurrence in 10 cases (20%). In routine surveillance group, 16 cases (17%)presented recurrence, all of which was correctly diagnosed on PET. NPVand PPV were 100% and 76%, respectively. In subgroupanalysis, sensitivity and NPV were higher in the low-risk group (stages I–IIA) than in the high-risk group (stages IIB–IV), whilespecificity and PPV were higher in the high-risk group. Conclusion In conclusion, FDG PET is an effective diagnostic tool in posttreatment surveillance of melanoma. Even in caseswithout clinical suspicion, melanoma recurs in a considerable proportion of patients, which can be sensitively diagnosed on PET.
