        Subbrow Approach as a Minimally Invasive Reduction Technique in the Management of Frontal Sinus Fractures

        Background: Frontal sinus fractures, particularly anterior sinus fractures, are relativelycommon facial fractures. Many agree on the general principles of frontal fracturemanagement; however, the optimal methods of reduction are still controversial. In this article,we suggest a simple reduction method using a subbrow incision as a treatment for isolatedanterior sinus fractures. Methods: Between March 2011 and March 2014, 13 patients with isolated frontal sinusfractures were treated by open reduction and internal fixation through a subbrow incision. The subbrow incision line was designed to be precisely at the lower margin of the brow inorder to obtain an inconspicuous scar. A periosteal incision was made at 3 mm above thesuperior orbital rim. The fracture site of the frontal bone was reduced, and bone fixation wasperformed using an absorbable plate and screws. Results: Contour deformities were completely restored in all patients, and all patients weresatisfied with the results. Scars were barely visible in the long-term follow-up. No complicationsrelated to the procedure, such as infection, uncontrolled sinus bleeding, hematoma, paresthesia,mucocele, or posterior wall and brain injury were observed. Conclusions: The subbrow approach allowed for an accurate reduction and internal fixationof the fractures in the anterior table of the frontal sinus by providing a direct visualization ofthe fracture. Considering the surgical success of the reduction and the rigid fixation, patientsatisfaction, and aesthetic problems, this transcutaneous approach through a subbrowincision is concluded to be superior to the other reduction techniques used in the case of ananterior table frontal sinus fracture.

        고령자 주거만족도와 우울감 상관성 분석에 기반한‘주거우울’연구

        With the increase of the elderly population, problem on social isolation and loneliness have grown in Korea. Elderly dwelling issues have also been gaining increased attention since dwelling environment is discussed in conjunction with elderly’s health and life quality. In this study, the data was collected retrospectively of 350 elderlies who live as single households during 1 September, 2017 to 30 September to identify how to define and measure dwelling depression. The content validity and reliability were evaluated and the correlation between the depression and the dwelling satisfaction were compared. Lastly, a regression analysis on the classification of the dwelling depression was performed. The results show that depression appearing in elderly are more likely to experience a dwelling unsatisfaction. Our results contribute to an understanding and measurement of the dwelling depression which has not been sufficiently specified. 우리나라는 급격히 증가하고 있는 고령 인구 증가에 따라 고령자의 사회적 고립 및 우울 등의 문제가 심각하게제기되고 있다. 또한, 고령자의 건강과 삶의 질의 향상 요인으로 주거 환경에 대한 중요성이 부각되면서 고령자 주거에대한 논의는 더욱 강조되고 있다. 본 연구에서는, 이와 같은 현실을 고려하며, 2017년도 9월 1일부터 9월 30일까지 7 개 시·구에 거주하는 65세 이상 남·여 단독가구 고령자 350명을 대상으로 단독가구 고령자의 주거우울 개념 및 주거우울 유형을 실증적으로 정립·분석하였다. 측정도구의 신뢰도 및 타당성 검토와 우울과 주거만족의 상관분석을 실시하였고, 주거우울 유형 분류를 위해 회귀분석을 시행하였다. 그 결과, 고령자 우울과 주거만족의 상관을 확인하였고, 주거우울지수를 산출하여 단독가구 고령자의 우울 상태를 판정할 수 있도록 하였다. 본 연구를 통해 새롭게 개발된 주거우울 척도와 유형이 단독가구 고령자의 주거우울정도에 대한 정확한 진단을 가능하게 할 것으로 기대된다.

      • 한국사회의 이주 집단으로서 귀환 입양인과 그들의 자생적 운동이 주는 시사점

        이 글은 한국 사회에 존재하는 특수한 이주 집단인 귀환 해외 입양인들의 조직화와 운동을 분석하고 있다. 기존에 정체성이나 사회 적응 혹은 서구사회에서 인종문제의 틀에서 다루어졌던 해외 입양인들을 한국 사회 내부의 이주민으로 위치지움으로써 사회 운동의 세력으로써 재조명하고자 한다. 따라서 디아스포라를 ‘개개인의 정신적 상흔이 공정인 위상을 획득하는 것’으로 재개념화 하고 이러한 틀에 따라 귀환 입양인들의 조직화와 운동을 분석한다. 분석의 대상으로 삼고 있는 단체는 『국외입양인연대(Adoptee Solidarity Korea: 이하 애스크)』와 『진실과 화해를 위한 입양인 모임(Truth and Reconciliation of Adoptee Community in Korea: 이하 트랙)』이다. 또한 이 장에서는 이들의 다양한 운동 중에서 애스크가 미혼모들을 대상으로 벌이고 있는 운동과 트랙의 ‘인정 운동’을 다룰 것이다. 이 두 운동이 귀환 입양인들이 한국사회에 적극적으로 개입하고 사회 변혁에 동참하고자 하는 의지를 잘 드러내고 있는 현장이라고 판단되었기 때문이다. 본 연구는 입양인 운동의 장을 통하여 디아스포라가 한 사회에 갖게 되는 요구사항들과 시민적인 기여의 지점들을 강조한다. 따라서 ‘비국민’과 ‘국민’으로 양분하며 ‘권리’와 ‘의무’를 배타적으로 부여 하는 것이 아닌, 그 사이에 보다 다양한 성원권을 상상해야 한다는 요청을 이론적이거나 도의적 관점이 아닌 경험적 실재에서 유추하고자 한다.

        Thoracic Scoliosis in Patients with Primary Spontaneous Pneumothorax

        Background: Primary spontaneous pneumothorax (PSP) affects patients without clinically apparent lung disorder found in tall and thin young male. Scoliosis refers to curves exceeding 10° Cobb angle observed through chest X-ray and affects 2% to 4% of adolescents. Both conditions are commonly encountered in primary health care setting. The aim of this study is to access the correlation of thoracic scoliosis and PSP in adolescent. Methods: A retrospective analysis was conducted for patients diagnosed for PSP in Konyang University Hospital between January 2010 and March 2017. Chest X-rays of 222 patients and 155 normal control (NC) cases were reviewed to measure the Cobb angle. Greater than 10° of Cobb angle is diagnosed as scoliosis. Results: Scoliosis in patient with PSP has higher incidence than that of NC group (p<0.001). Median value of Cobb angle is 12.9° in PSP group and 14.7° in NC group. Directional relationship between scoliosis and pneumothorax in PSP group is also observed; 40.5% cases are ipsilateral and 59.5% are contralateral. Conclusion: PSP patients tend to have thoracic scoliosis more commonly compared with normal healthy adolescent. Scoliosis may contribute to heterogeneity of alveolar pressure which exacerbates subpleural bleb formation that can cause pneumothorax. The causal relationship is unclear and further studies are needed in the future.

        Unusual Pseudoaneurysm of the Dorsalis Pedis Artery after an Iatrogenic Injury

        Aneurysm and pseudoaneurysm of the dorsalis pedis artery (DPA) are rare vascular entities. Pseudoaneurysms of the DPA are commonly due to blunt trauma, sharp penetrating injury, fracture, or iatrogenic injury. Herein, we report the case of a patient with a rare iatrogenic pseudoaneurysm that occurred after blood sampling. The diagnosis was suspected based on palpitation of a pulsatile mass on the dorsal foot and confirmed by color Doppler ultrasound and computed tomography angiography. Surgical treatment was successfully performed by reconstruction with an autologous venous graft. The patient recovered well, with no ischemic complications.

        Superior Effect of Ultrasonic Homogenization to Mechanical Agitation on Accelerating Reaction Rates in Asymmetric Ring Opening of Epoxides

        Double-layered mesoporous silica was prepared for use as an efficient support for the encapsulation of chiral salen Co-BF3 complexes in the pore channels. The heterogenized chiral salen complex was applied successfully to the asymmetric ring opening (ARO) of terminal epoxides using water, alcohols, and carboxylic acids as nucleophiles. Although the homogeneous salen catalyst exhibited higher catalytic activity than the heterogenized catalyst in ARO reactions as expected, the heterogeneous salen catalysts exhibited high catalytic activities that were sufficient for practical applications. This study evaluated the unique contribution of ultrasonic irradiation as a powerful mixing medium to improve the kinetics in comparison to conventional mechanical agitation during the ARO reactions. Both the homogeneous and heterogeneous chiral salen complexes catalyzed enantioselectively the racemic or chiral epoxides under ultrasonication. The adoption of ultrasonic irradiation in the catalysis resulted in a superior mixing effect to vigorous mechanical stirring. The reaction time was shortened to obtain the highest enantiomeric excess (%ee) when ultrasonic irradiation was applied to the reaction system. Through the ARO reaction of epoxide by phenol derivatives, the highly enantio-enriched β-blockers could be synthesized easily from racemic terminal epoxides or chiral ones using chiral cobalt–salen catalysts. The regioselective ARO reaction of (R)-ECH with propionic or butyric acid proceeded smoothly at room temperature under ultrasonic exposure in the presence of the heterogenized catalyst. The corresponding glycidyl alkylates were produced with excellent optical purity of up to 98%ee. The chiral cobalt–salen complexes encapsulated in MCM-41 retained its catalytic activity and enantioselectivity for three recycles. Consequently, the heterogeneous chiral catalysts anchored in MCM-41 offered the practical advantages of facile reuse through simple recovery from the reactant and products.

        The Catalytic Activities of New Metal Oxide Catalysts Modified by Graphene Oxide to Produce Propene and its Economic Evaluation

        New types of catalysts, “pure ferrite”, “ferrite coated with graphene oxide (GO‐ferrite)”, and a “reduced form of graphene oxide with ferrite (RGO‐ferrite)”, were synthesized to produce propene through the oxidative dehydrogenation (ODH) of propane. The performance of these catalysts (i.e., yield and selectivity) was examined through laboratory experiments over the temperature range, 623 to 923 K. Based on the experimental results, the contribution of GO to the selectivity to propene was determined, and a mathematical model of the reaction system was developed and applied. The model was applied to an economic evaluation of each catalyst, and the annual profits of these three different types of catalysts were compared.
