A Study of TiN-Coated Metal-on-Polymer Bearing Materials for Hip Prosthesis

The TiN-coated metal-on-polymer hip prosthetic pair has the potential to reduce wear debris of UHMWPE (ultra-high molecular weight polyethylene) and to prevent metallic-ion-induced cytotoxicity. However, high quality and adherent film is a key to the clinical success of hip prostheses. In this study, titanium nitride (TiN) films were deposited on stainless steel using plasma immersion ion implantation & deposition (PIII&D)technique to create high-quality film and an adherent interface. The chemical state and composition were analyzed by X-ray photoelectron spectroscopy (XPS), Auger electron spectroscopy (AES) and energy dispersive spectroscopy (EDS). The mechanical properties of the films were characterized using a micro-hardness tester and a pin-on-disk wear tester, and an x-ray diffractometer (XRD) was used for a crystallographic analysis. The PIII&D-treated TiN films showed a stoichiometric and (200) preferred orientation and micro-hardness up to 150 % higher than untreated film. A TiN-coated specimen using the PIII&D process also showed less UHMWPE wear compared to untreated specimens. The volumetric wear rate of UHMWPE could be reduced by as much as 42 % compared to when Co-Cr alloy was used. The results of this study show that advanced TiN-coating via the PIII&D process is a viable means of reducing UHMWPE wear in the metal-on-polymer bearing couple.

송대에 문치주의 정책과 과거제를 통해 등장한 사대부들은 선우후락(先憂後樂)의 사명의식, 천하를 천자와 함께 다스린다는 천하공치의식(天下共治意識), 계왕개래(繼往開來)의 이학적 도통의식을 가지고 자신의 주체적 역량을 펼치고자 하였다. 이들은 강한 자아의식과 현실적지적 자신감을 바탕으로 송대의 사상과 학문을 주도하였고 시문과 예술에도 관심이 많아 이를 통해 그들의 역량을 펼치고자 하였다. 서예도 그들의 주관심 분야의 하나였다. 소식황정견미불의 서예 비평은 공통적인 지향을 가지면서 각각의 차이점을 보였다. 소식과 미불은 그것을 의취(意趣)로 강조하였고, 황정견은 운(韻)으로 표현하였으며. 미불은 또한 팔면의식(八面意識)에서 부정형(不定形) 서예를 통해 자유로운 서예의식을 강조하는 등 각각 상의적 서예비판을 하였다. 이러한 소황미의 비평은 종래에 비하여 몇 가지 특성을 보여 주었다. 우선 비평용어나 개념을 이해하기 쉬운 것으로 바꾸었고, 다음에 전주기(篆氣)를 강조하여 선진시기의 서예에서 새로운 고법기준(古法基準)을 찾았다. 그리고 이왕(二王 왕희지와 왕헌지 부자)을 추존하면서 또한 극복의 대상으로 삼는 탈이왕의식(脫二王意識)을 가졌다. 다음에 이들은 새로운 서통의식(書統意識)을 가졌는데 상의적인 전통성과 이념성에 잘 부합되는 장욱(생몰연도 미상. 당 현종 때 활동)이나 안진경과 양응식을 새롭게 부각시켰다. 소황미의 상의적 서예 비판은 고려에도 영향을 미쳤다. 고려 문인들 중에서 임춘(고려 인종~명종 때 활동)의 서상기인론(書象其人論)과, 이인로의 서지이일론(書之理一論)의 비평의식에서 그 영향을 볼 수 있었다. 그리고 이규보(1168~1241)는 상의적 관점에서 신품사현의 서예의 다양한 특성을 논의하였다. 이러한 요소들은 송대 상의적 비평의식의 영향과 무관하지 않음을 보여주는 사례라 하겠다. 따라서 송대 문화의 생동성을 보여 주는 상의(尙意)라는 개념은 역사성을 가지면서 전통에 대한 재해석과 예술적 일탈, 그들의 이념적 지향 등을 통해 새롭게 승화시킬 수 있었다고 본다.

4월은 유채. 개나리. 벚나무. 매화. 라일락. 오리목. 도토리. 참나무. 또는 사과. 배. 복숭아등을 비롯한 과일 나무 등 이름도 모르는 야생화를 비롯하여 밀원 식물이 1년중 제일 많이 개화된다고 해도 과언이 아닐 것이다. 기후도 적당한데다 밀원이 많다보니 1년중 번식이 제일 잘되는 시기이기도 하다.

초고령사회 대비 동주민센터 거버넌스 구축에 관한 연구: 도쿄도와 서울시 사례의 비교를 중심으로

This study tries to suggest a governance model to systematically respond to how community centers in Seoul will provide administrative services as the super-aged society approaches. For this study, the cases of the Visiting Community Center in Seoul and Regional Comprehensive Care System in Japan are analyzed, and in-depth interviews were conducted to make the actual situation of the field more concrete. According to analytical results, if the society becomes a super-aged society, the proportion of elderly people of various types from severe to mild patients with diseases such as mental illness and heart disease, will be increased. It was confirmed that the service needs to be actively provided. Reflecting these characteristics of the super-aged society, it was also identified that community centers need to be built into an integrated care-type structure that provides essential health and medical services rather than welfare-oriented services. From this point of view, this study suggests the organizational structure of community centers consisting of self-government administration, welfare administration, and integrated care. By hiring and granting authority, improvement plans were prepared and presented to enable prompt health and medical measures and decisions in the field. Finally, a community participation type of governance model was proposed in which community organizations, social economic enterprises, community centers, and public health centers support prevention and prescription simultaneously. 본 연구는 초고령사회가 다가오면서 서울시 동주민센터가 어떻게 행정서비스를 제공할 것인지에 대해서 체계적으로 대응하기 위한 거버넌스 모델을 제시하고자 하였다. 이러한 연구를 위해서 서울시 찾아가는 동주민센터와 일본의 지역포괄케어시스템 사례를분석하였고, 보다 현장의 실태를 구체화하기 위해서 심층인터뷰를 실시하였다. 분석 결과, 초고령사회가 되면 정신질환과 심장질환 등과 같은 질병을 앓는 중증환자부터 경증환자들까지 다양한 유형의 노령층 비율이 증가할 것이므로 복지뿐만 아니라 예방적 서비스 및 응급치료를 포함한 처치활동과 같은 보건의료서비스가 적극적으로 제공될 필요가 있음을 확인하였다. 이러한 초고령사회의 특징을 반영하여 서울시 동주민센터가제공하는 복지중심 서비스를 보건의료 서비스가 필수적으로 제공되는 통합돌봄형 서비스 구조로 개편할 필요가 있음도 파악하였다. 이러한 관점에서 본 연구는 자치행정, 복지행정, 찾아가는 복지, 통합돌봄으로 구성된 동주민센터의 조직구성을 제안하였으며, 동주민센터의 전담간호사를 보건소에서 파견할 것이 아니라 통합돌봄팀의 개방형 전문직으로 직접 채용하여 권한을 부여함으로써 현장에서 신속한 보건의료적 조치와 결정을 할 수 있게 하는 개선안을 마련・제시하였다. 마지막으로 지역사회의 다양한 직능단체, 사회적 경제조직, 마을공동체, 보건소 등이 예방과 처방을 함께 지원하는 지역사회참여형 거버넌스 모델을 제안하였다.
