        一次醫療 役割 評價를 위한 評價道具에 관한 基礎硏究

        The primary health care service in primary health care center rests major area to resolve the rapidly increasing health care demand and problems of supply in the health care delivery system. Therefore, the strengthening of primary health care role and the evaluation of the program should be needed. In this study, a schematic model for evaluation of primary health care role consisted with 2 phases; 'Assessment' and 'Feed-back' phase, and its measuring instrument was developed and tested for the practical utilization. The measuring instrument was consisted with 47 questions ; 28 of those were requested by health care consumers and 17 were from physician's judgements. For assessing the program, in the first phase of the model, the Primary Health Care Index was calculated, but also the scores classified by five essential elements of primary care such as accessibility, accountability, comprehensiveness, continuity and co-ordination. In feed-back phase, data obtained were recategorized into management variables such as 'Utility and efficiency', Model of primary care', Medical doctor performance', 'Medical record keeping', 'Referral', 'Paraprofessionals' et al, to feed back for improvement of future program. Twelve physicians and 362 registered families from one of primary health care model clinic, the Family Practice Model Clinic of Seoul National University Hospital were selected to examine the reliability and applicability. The following conclusions were obtained; 1. On the test for the measuring instrument 1) The reliability of the instrument, by comparison of the variance value between subgroups by socio-demographic variables, was proven to be significant except on the accountability. 2) On the consistancy between items for each element of primary care, Cronbach' s Alpha showed to be higher as above 0.7, but on accountability and comprehensiveness, the adjustment seemed to be necessary. 2. Using this scheme, the assessment scores of FPMC in SNUH, the scores of the 5 primary care elements were calculated in the range of sixty to eighty points, so it showed that primary health care was adequately served in this model clinic. 3. On analysing by management variables in the FPMC, some problems to be improved for future program was shown by the items with lower score and open questions.

      • 북한이탈여성의 외상경험과 우울 : 낙관성의 중재효과

        This research was conducted in order to examine North Korean women fugitives and the moderating effect of optimism the relationship between these fugitives' traumatic experience and depression. Subjects of research were 200 North Korean women fugitives who inhabit throughout 15 regions in South Korea, and the scales employed are:‘North Korean fugitives' traumatic experience scale', ‘Depression scale (CES-D)', ‘Optimism scale (LOT-R)'. A frequency analysis was administered in order to examine the frequency of traumatic experiences and a standard multiple regression analysis was administered to verify the influence of traumatic experience on depression, and the results exhibited that the regression model of traumatic experience was significant. The Pearson correlation analysis was conducted so as to inspect the correlation of traumatic experience, depression, optimism. The results displayed that traumatic experience in North Korea had a positive relationship with traumatic experience during escape from North Korea, depression and negative strategy. Optimism had a negative relationship with depression, and a positive relationship with positive strategy. Lastly, a hierarchical multiple regression analysis was administered to verify the moderating effect of optimism in the relationship between traumatic experience and depression. Results exhibited that although optimism significantly forecasted the variant of depression, neither the correlation between traumatic experience in North Korea and optimism nor that between traumatic experience during escape from North Korea and optimism could significantly forecast the variant of depression. Thus, the moderating effect of optimism could not be verified. However, the correlation of traumatic experience during escape from North Korea and neither optimistic nor negative strategy could significantly forecast the variant of depression.       • 유아교육기관의 특별활동에 대한 예비교사의 인식

        The purpose of the study is to examine perception of student teachers of extra-curricular activities in Kindergartens and child care centers. Questionaries were corrected by 70 student teachers from 28 kindergartens and 36 child care centers and in-depth interview with 3 student teachers. Major findings were that mostly respondents reported doing many extra-curricular activities in Kindergartens and child care centers. English, physical activity, and traditional instrumental training were popular extra-curricular activities in kindergartens and child care centers. For student teachers, extra-curricular activities are viewed as a way of diverse experience for young children and means of visible educational output. But student teacher was aware of discrepancy with many extra-curricular activities and educational purposes of early childhood institutions. In additions, student teachers proposed that adequate extra-curricular activities were introduced to kindergartens and child care centers. 이 연구는 예비교사가 실습하는 유치원과 어린이집의 특별활동 실태를 조사하고 이에 대한 예비교사의 인식을 알아봄으로써 유아교육실습 및 교사 발달에 대한 기초 자료를 제공하고자 한다. 실습기관 유치원 28곳과 어린이집 36곳의 특별활동 실태를 분석한 결과, 가장 많이 실시하는 특별활동은 영어, 체육, 국악의 순으로 나타났다. 예비교사들은 특별활동이 많아짐으로 인하여 유아들이 산만하고 주의집중하기 어려우며 교사와 유아 간에 친밀감이 부족하다고 인식하였다. 또한 유아교사의 역할 역시 가르치는 교수적인 측면보다 유아들의 안전과 시간을 관리하는 역할의 비중이 증가하고 있는 것으로 인식하였다. 적절한 특별활동은 유아들의 다양한 경험을 위해서 필요하지만 과다한 특별활동은 유아교육기관의 잘못된 인식과 교사로서의 정체감 형성에 부정적인 영향을 미치는 것으로 인식하였다. 유아교육기관의 특별활동의 가장 큰 문제는 특별활동 강사의 부족한 자질과 보여주기 위한 교육의 차원으로 결과물을 강조하면서 경쟁적으로 특별활동이 많아지는 것을 언급하였다.

        청소년 건강위험행태 감소의 긍정적 영향요인: 성장 발달자산 중심의 생태학적 접근

        This study assessed the extent to which developmental assets predict or contribute to the reduction of health risk behaviors among Korean adolescents. The concept of developmental assets was introduced by Benson and his colleagues as building blocks for healthy child and adolescent's development. Their framework of 40 developmental assets encompasses positive external environmental factors and internal personal qualities which strongly influence adolescents' quality of life. We analyzed data from our ‘Korean Youth Health Related Behaviors and Developmental Assets Survey.’ The survey was based on self-reported questionnaires we distributed to 3,739 grade 9 students at eight schools across two cities in South Korea in 2001. The 40 developmental assets were measured by using 5-point Likert scales, and health risk behaviors were measured in terms of the prevalence or the experience rate. We identified the predictors of each health risk behavior among the 40 developmental assets using multiple logistic regression analyses. We found that the assets of 'restraint', 'positive peer influence', and 'time at home' were commonly the most important predictors associated with decreased reports of most health risk behaviors except depression. 'Self-esteem' was the most important predictor of decreased reports of depression and suicide. Other internal and external developmental assets were identified as important predictors to specific behaviors. We concluded that the positive asset promotion approach would be more effective and important than the risk factor approach in preventing effectively health risk behaviors among adolescents, and that intervention programs should include more comprehensive and adolescent-based strategies to promote adolescents'health and quality of life by a multi-level multi-dimensional approach.

        本 연구의 목적은 아동의 情意的 特性과 知的 能力이 어떠한 관계가 있는가를 밝히려는 데 있다. 本 연구에서 검증하려는 가설은 다음과 같다. 가설 1. 아동의 情意的 特性과 知的 能力 사이에는 正的 相關이 있을 것이다. (가설 1-1) 성취동기와 知的 能力 사이에는 正的 相關이 있을 것이다. (가설 1-2) 知的 好奇心과 知的 能力 사이에는 正的 相關이 있을 것이다. (가설 1-3) 자아개념과 知的 能力 사이에는 正的 相關이 있을 것이다. (가설 1-4) 대인관계와 知的 能力 사이에는 正的 相關이 있을 것이다. (가설 1-5) 持久力과 知的 能力 사이에는 正的 相關이 있을 것이다. 위의 假說을 검증하기 위하여 서울시내에 거주하는 3세∼5세 어린이 37명을 대상으로 유아지능검사를 실시하고, 그 어머니에게나 가족에게 자녀에 대한 意見檢査를 실시하였다. 假說檢證을 위한 統計處理方法으로는 아동의 정의적 특성점수와 I.Q.와의 積率相關계수(pearson의 productmoment correlation coefficient)를 산출하였다. 검증 결과(假說 1-1), (假說 1-2)는 P>0.05의 有意水準에서 긍정되었고, (假說 1-3), (假說 1-4), (假說 1-5)는 P>0.1로서 부정되었다. 本 연구를 통해서 얻은 결과는 다음과 같다. 1. 학령전아동의 성취동기와 知能과의 사이에는 正的 상관이 있다. 2. 학령전아동의 知的 好奇心과 知能과의 사이에는 正的 相關이 있다.

      • 지방자치단체의 건강증진사업 활성화 방안 : An Educational Approach 교육적 접근을 중심으로

        This paper will discuss about how we can foster educational support mechanisms to facilitate health promotion programs at the local level. Health promotion in Korea is in the early developmental stage; it has only been since the Health Promotion Act was legislated in 1995, the health promotion programs have been planned and implemented. In the context of the recent decentralization process, local health departments have a major responsibility for developing and implementing health promotion programs at the local level. The short history of health promotion in Korea has meant that local public health departments have limited experience and organizational capacity for health promotion planning & practice. The results of one survey for investigating the progress of health promotion at the local level are instructive. The survey demonstrated that the public health workers recognized that the lack of personnels, insufficient budget, the lack of policy & the organizational support, the lack of skill & knowledge to be effective health promotors, the lack of guidance for health promotion practice were major barriers to implementing health promotion programs at the local level. The object of this paper is to suggest some ways of overcoming barriers to implementing health promotion programs at the local level This paper emphasizes on the importance of educational supports as well as environmental supports - legislative, policy, organizational, economical - in building the organizational capacity and infrastructure of local health department for health promotion. It suggests some ways of providing educational supports to the public health workers at the local level. and supports the positions that educational opportunities for training in health promotion can be better provided to the public health workers at the provincial level rather than at the national level. It argues that the educational & training programs should be developed and based on the educational need assessment; that the application of the main educational principles & theoretical models for health promotion be used to develop educational programs for the public health workers; and that professional health organizations should make plans to provide more educational programs at their annual conferences of at other convenient times. These kinds of educational supports facilitate the ability of public health workers to improve their capacity for health promotion practice at the local level and help to alleviate some of the pressure of state resources.
