        저는 "동물병원 노무관리"라는 제목으로 강의를 할 노무법인 대명 소속의 이동현 노무사입니다. 본 강의는 노동법의 범위가 광대하므로 주요 노동법 규정을 알아봄으로써 동물병원 현장의 인사노무관리에 적극 활용할 수 있고 이를 통해 합법적이고 효율적인 인사노무관리를 함으로써 상생, 협력하는 노사 관계를 구축하고 기업경쟁력을 강화하는데 보탬을 됨을 목적으로 합니다. 설명의 순서는 5인 미만 즉 4인 이하 사업장에서 꼭 지켜야 할 주요 노동법규정 과 5인 이상 사업장에서 꼭 지켜야 할 주요노동법규정을 나누어 설명하도록 하겠습니다.

      • 부산 도심의 변천

        근대 이후 부산은 광복동 1도심을 중심으로 발전해 왔다. 1980, 1990년대를 지나면서 서면이 새로운 도심으로 부상했고, 해운대?동래?사상?하단?구포는 부도심 역할이 주어졌다. 하지만 이들부도심 가운데는 아직 중심지 기능을 제대로 하지 못하고 있는 곳이 많다. 부산의 도시 규모와 광역경제권에 대응하는 중심도시 기능을 고려하면 다수의 부도심이 적절히 분산된 네트워크형 도시공간체계의 구축이 필요하다. 이를 위해서는 서면·중앙동·해운대를 광역거점(Tri-Hub)으로 육성하면서 ‘도심재생 프로젝트’ 를 추진해야 한다. 서부산권의 신흥 중심지로 떠오르고 있는 북구 덕천동과 상대적으로 취약한 사상구 사상·사하구 하단 등을 부도심으로 발전시키는 전략도 요구된다. 네트워크형 도시공간체계를 구축하려면 ▲도심별 독자적인 기능과 역할 부여 ▲부도심 가로구역별 건축물 최고높이 완화 ▲부도심 용적률 인센티브제 도입 ▲대중교통망 연계를 통한 중심지 간 네트워크 강화 등이 필요하다. 〈부산의 도심발전 전략 그림 참조〉

      • 原發性上顎洞囊胞症 1例

        This is report of a case of primary mucocele occurring in the left maxillary sinus, which is supposed to be rare. This 32 years old male entered to the National Police Hospital on Oct. 2-1971 with complaints of a palpable mass in the left cheek. The external examination revealed that a walnut sized, moderately hard, and sound shaped mass which has slight fluctuation, was palpated on the left7 maxillary; region, however no tenderness could be demonstrated. Aspiration of the contents disclosed chocolate colored mucoid fluid, and negative finding were detected, in the nasal cavity by ant rhinoscopy. X-ray study of maxillary sinus showed slight cloudiness over the left maxillary sinus, without definite evidence of the bone destruction. Under suspiction of maxillary cyst exploratory operation(caldwell-luis operation approach) was performed and found that a large mucocele of maxillary sinus which, involving the ant and medical bony wall of the sinus. The whole mucosa was carefully separated and removed and a wide enough counter opening was made. Post operative course was satisfactory and no undesirabe sequela occurred up to date.

        노인성 구강-안면 이상운동증 환자 2례에 대한 증례보고

        Objectives : The purpose of this case is to report the improvement after the acupuncture therapy and herbal medicine about two patients with senile oro-facial dyskinesia. Methods : We treated the patient with acupuncture therapy and herbal medicine by evaluating facial, lips, jaw and tongue expression of AIMS(Abnormal Involuntary Movement Scale) and clinical symptom progress. Results : We have recently experienced two cases of senile oro-facial dyskinesia. Two patients were improved significantly through the acupuncture therapy and herbal medicine, so we report it for the better treatment. Conclusion : Oro-facial dyskinesia is stereotyped movements, consisting of smacking and pursing of the lips, lateral deviation and protrusion of the tongue, and occasionally lateral deviation and protrusion of the jaw. Spontaneous oro-facial dyskinesias occur in the elderly and had been said to result from edentulousness. Oriental medical treatment for oro-facial dyskinesia resulted in satisfactory results by diminishing the symptoms progressively during the admission periods. More research of oro-facial dyskinesia is needed.

        Roles of focal adhesion and cytoskeleton dynamics in myogenic vascular regulation

        Ligand-dependent or ligand-independent activation of ion channels or G-protein-coupled receptors contributes to pressure-induced vasoconstriction or vasodilation critical for the maintenance of blood flow response to the physiological alternation of intravascular pressures (e.g., elevation of arterial intraluminal pressure leading to vasoconstriction). A mechanosensitive integrin-dependent focal adhesion complex rearranges cytoskeletal structures for myogenic responsiveness in vascular smooth muscle cells. Myogenic responses maintain blood flow, generate vascular tone, prevent damage to capillaries due to high blood pressure, and reduce edema caused by high capillary hydrostatic pressure. However, enhanced myogenic vasoconstriction causes cardiovascular diseases (e.g., hypertension and vasospasm). Thus, small arteries and arterioles may be equipped with negative regulatory mechanisms to prevent exaggerated myogenic responses. Overall, the studies described in this mini literature review aim to explain that pressure (stretch)-dependent vasoconstriction is controlled by mechanisms underlying negative feedback regulation regarding focal adhesion molecules or cytoskeletons. Since the development of therapeutic approaches for the negative regulations of myogenic vasoconstriction helps overcome vascular disorders, this review focuses on myogenic vascular regulation and its negative feedback mechanisms.

        하악 전돌증 환자에서 전산화 단층 촬영을 이용한 하악관의 해부학적 위치에 관한 연구

        Purpose: In this study, we analyzed and compared the anatomical position of the mandibular canal in normal occlusion and mandibular prognathism patients. Patients and Methods: Computed tomography image from 58 patients were divided into normal occlusion group and mandibular prognathism group, and each measurement were taken in the each measuring points(2nd premolar, 1st molar, 2nd molar, 3rd molar, ramus). Measurements were statistically analyzed by student's t-test. Results: BC (Thickness of the buccal cortex) value was 2.3~2.7 mm, CB (Distance from the canal to the lingual aspect of the buccal cortex) value was 1.3~4.3 mm, MC (Diameter of the canal) value was 3.2~3.8 mm, LI (Distance from the canal to the lingual aspect of the lingual cortex) value was 2.0~3.7 mm, TM (Thickness of the total mandible) value was 9.5~12.9 mm and CM (Distance from the canal to the inferior border of the mandible) value was 6.9~17.5 mm. Conclusion: In the comparison between two groups, there was statistically significant difference in CB value of 2nd, 3rd molar between normal occlusion and mandibular prognathism, and other value in the rest of the measuring points didn't show statistically significant difference.

        한국인에서 혈중 전립선특이항원치에 따른 전립선 생검 양성률에 관한 다기관 연구

        Purpose: The incidence of prostate cancer is increasing in Korea, but compared with western counties, the incidence is relatively low. The detection rate of prostate cancer, according to the serum prostate-specific antigen(PSA) level, is reportedly different in Korean men, but this remains to be confirmed. We retrospectively reviewed the data of prostate biopsies, and evaluated the detection rate of prostate cancer from biopsies, according to the serum PSA level in Korean men. Materials and Methods: We retrospectively reviewed the results of 2,422 Korean men who had undergone prostate biopsies at 12 medical centers. Prostate biopsies were performed in cases of high PSA levels, greater than 4ng/ml, or abnormal findings on digital rectal examination. Results: Of the 2,422 men, 39.7% had a positive biopsy. With PSA levels between 4 and 10ng/ml, the detection rate of prostate cancer was 15.9%. This rate was similar to that of the Japanese(15.8%), but quite different from that of American men(25%). With PSA levels above 10ng/ml, 59.5% of men had a positive biopsy. For PSA levels ≥4ng/ml and ≥10ng/ml, the detection rates were 42.1 and 59.5%, respectively. Conclusions: When the serum PSA levels were divided into 4 subdivisions (4.0-10.0, 10.0-20.0 and 20.0-100.0ng/ml and more than 100.0ng/ml), the detection rates were 15.9, 34.1, 66.2 and 93.8%, respectively.

        신경외과 수술로 인한 신경영양각막염 환자에서 동일초점현미경을 이용한 각막미세구조 변화 분석

        Purpose: To investigate the changes of the corneal microstructure of neurosurgically-induced neurotrophic keratitis patients compared to normal human corneas using in vivo confocal microscope (IVCM). Methods: Ten eyes in the normal control group and 11 eyes in the neurosurgically-induced neurotrophic keratitis patient group were included in the present study. After corneal sensitivity tests were performed, thickness of each layer and number of endothelial cells and stromal keratocytes in the cornea were assessed using IVCM. Morphological characteristics of the corneal nerves were measured by ImageJ software. Results: After analysis of corneal thickness layer by layer, the Bowman’s layer was significantly reduced in the neurosurgicallyinduced neurotrophic keratitis patient group compared with the normal control group (<EM>p</EM> = 0.016) and the portion of Bowman’s layer was significantly reduced in the neurosurgically-induced neurotrophic keratitis patient group compared with the control group (<EM>p</EM> = 0.024). The nerve fiber length per square-millimeter became significantly shorter compared to the normal control group (<EM>p </EM>= 0.042). The nerve fiber length per square millimeter showed significant correlations with the number of fibers, number of beading, branching patterns, and nerve tortuosity (<EM>p</EM> = 0.002, 0.002, 0.013 and 0.034, respectively). The number of endothelial cells and stromal keratocytes, the number of nerve fibers and beading, and the pattern of branching and nerve tortuosity showed no significant differences between the normal and neurosurgically-induced neurotrophic keratitis patient groups. Conclusions: Our results showed that decreased thickness of Bowman’s layer may be related to the decreased corneal nerve distribution, secondary to the dysfunction of trigeminal nerve branch innervating the cornea. The microstructural changes of Bowman’s layer can help diagnose the disease and evaluate the current status in neurosurgically-induced neurotrophic keratitis patients.
