Purpose: Implant retained overdenture has many benefits. However, it is affected by various factors such as bone quality, the number of implants, types of attachments, masticatory pattern, and periodical management. Clinically, these factors are related to prosthetic complications and longevity of implants. In this study, we evaluated patients treated with implant overdenture and considered factors that may be related to implant complications. Materials and Methods: To evaluate various complications, we analyzed the clinical charts of patients treated with implant overdenture from 2005 to 2010. Single or bimaxillary edentulous patients with implant overdenture loaded over 1 year wereincluded. Those patients with systemic factors, unclear recordings, and that did not undergo periodic checks were excluded. Results: In total, 173 implants (53 maxilla, 120 mandible) were installed in 48 patients, and 53 overdentures (14 maxilla, 39 mandible) were made, The average number of overdentures was 3.6 in maxilla and 3 in mandible, Stud type attachments were used in 12 cases (1 maxilla, 11 mandible), and bar type attachments were used in 41 cases (13 maxilla, 28 mandible). The rates of maxilla and mandible were 22.5% and 77.5%, respectively. Four cases of failure of the implant fixture were identified. Failure of osseointegration was found in two of the cases, and fracture of the implant fixture was found in one case. Both types of failure were found in another case. Fracture of the clip, O-ring replacement, and falling off the magnet or locator were identified as prosthetic complications. Conclusion: Implant overdenture treatment is a very efficient and reasonable treatment option for edentulous patients. However, to achieve good treatment results and longevity, precise diagnosis, ideal implant placement, stable occlusion, and periodic follow-up are required.
직접방식 Flat Panel Detector를 이용한 Tomosynthesis의 유용성 평가

Purpose : To conduct Digital Tomosynthesis by using 17” FPD-loaded body scanner, and to determine the possibility of application and the extent in clinical practices through evaluating its usability. Materials and Methods : We conducted Tomosynthesis to the 30 patients who has undergone examination or ongoing treatment in our hospital and other 9 volunteers depending on the objective part, and we selected 13 for chest and 11 for orthopedic implant-inserted knee joint among them as the object. We used CR System as the simple photography equipment, and Digital RIF System which loaded 17”direct conversion FPD was used for Tomosynthesis and we made burst shots then reconstructed the images with Filtered Back Projection. Results : The Tomosynthesis of chest showed comparatively higher visibility than simple photography at both main brochus, lung placed at the rear of heart, lung placed lower part at the rear of diaphragm, and the lower part ribs (p<0.01, p<0.05, p<0.01, p<0.001). There was no remarkable differences between Tomosynthesis and simple photography regarding the implant positioning of implant-inserted knee joint (p=1.000). However, in terms of cement positioning, bone- cement interface and cement- stem interface, the visibility level of Tomosynthesis was higher than simple photography (p<0.05, p<0.01, p<0.05). Conclusion : We found that Tomosynthesis would be able to clearly check the whole lung field, and would be the possible inspection with low amount of radiation dose in comparatively free pose without Artifact by metallic material. Therefore, we expect that the method will be able to be used for the orthopedic, respiratory organs and oral cavity fields.
편마비 환자의 견관절 능동적 외전 동작분석을 위한 방사선투시 동영상촬영의 유용성

Purpose : To evaluate clinical effectiveness of fluoroscopy guided motion analysis of active abduction of hemiplegic shoulder joint by analyzing motion of scapula and humeral head with fluoroscopy, and to provide assistance to elementary research on pain associated with hemiplegic patients’ shoulder abduction. Materials and Methods : From acute phase stroke patients who were admitted to Ewha Womans University Mokdong Hospital’s rehabilitation department, 19 patients with ability to abduct shoulder joints were selected as subjects. Depending on degree of active motion recovery, subjects were divided into two groups to compare differences during abduction of shoulder joints. Standard resolutions, partitioned by video processing part of an image control equipment, was simultaneously received by a computer to be recorded and analyzed. Results : By utilizing the fluoroscopy obtained by the computer capturing, values for variables at abduction angle of 30, 60, and 90 degrees could be determined, and the distance between scapula and thoracic vertebra (D1, D2, D3) could be measured for unaffected and affected side. Group 1 showed statistically significant difference(p<0.05) between unaffected and affected side for the distance between scapula and thoracic vertebra(D), while Group 2 did not show any significant difference. Trajectory of humeral head of the affected side was significantly superiorly translated than that of the unaffected side, and also showed tendency for medial translation(p<0.05). Conclusion : Motion analysis on abduction of shoulder joint using video fluoroscopy provides better quantitative analysis compared to previous medical imaging evaluation methods.
