피부섬유종과 융기성 피부섬유육종에서 Ezrin과 Endothelin-1의 발현에 관한 연구

Background: Distinction of dermatofibroma (DF) from dermatofibrosarcoma protuberans (DFSP) is occasionally difficult. Histologically, DF shows fibrohistiocyte growth, and DFSP shows myofibroblast proliferation. This difference is usually targeted by several immunohistochemical staining to differentiate between these two entities. Ezrin, a member of ezrin-radixin-moesin family of protein, has a critical role in oncogenic transformation. And endothelin-1 (ET-1) is a family of endothelin-derived peptides that promotes myofibroblast contraction and migration. Objective: The purpose of this study was to evaluate the usefulness of ezrin and endothelin-1 in differentiating between DF and DFSP. Methods: Formalin-fixed and paraffin-embedded tissues from 20 DFs and 10 DFSPs were immunohistochemically stained with ezrin and ET-1 monoclonal antibodies. Results: No DF case showed ezrin expression, whereas 3 of 10 DFSP cases showed weak positive reaction for ezrin. This difference was not statistically significant. In DF, 5 cases showed strong positive, 3 showed moderate positive, 4 were weak positive and 8 cases showed negative by ET-1 staining. In DFSP, 6 cases were strong positive, 1 was moderate positive, 2 were weak positive and 1 showed negative by ET-1. Conclusion: Ezrin and endothelin-1 are not useful in distinguishing between DF and DFSP. Further studies are needed to determine the factors which differentiate DFSP from DF. Background: Non-alcoholic fatty liver disease (NAFLD) is the most frequent liver disease. Evidence supporting a strong relationship between psoriasis and NAFLD exists. NAFLD is significantly higher in psoriatic patients than in matched controls and psoriatic patients with NAFLD have more severe forms of psoriasis than those without NAFLD. Objective: To evaluate the prevalence and clinical features of NAFLD among Korean psoriatic patients with abnormal liver function tests. Methods: We evaluated the laboratory results of psoriatic patients who visited the Department of Dermatology, National Medical Center, between September 2012 and June 2017. Those who had abnormal liver function tests were consulted by a hepatologist to confirm the diagnosis of NAFLD using ultrasonography. Results: A total of 307 psoriatic patients underwent liver function tests (LFTs), and 46 patients (15.0%) had abnormal LFT values. A hepatologist consulted psoriatic patients with abnormal LFTs, and hepatic ultrasonography was performed; 34 patients (73.9%) were diagnosed with NAFLD. Among psoriatic patients with abnormal LFTs, those with a Psoriasis Area and Severity Index (PASI) ≥10 had a significantly higher rate of NAFLD than psoriatic patients with PASI <10 (87.5% vs 59.1%). Conclusion: Among psoriatic patients with abnormal LFTs, 34 patients (73.9%) were diagnosed with NAFLD. Psoriatic patients with PASI ≥10 had a significantly higher rate of NAFLD than those with PASI <10. (Korean J Dermatol 2020;58(10):650∼655)
수용성 고무 아스팔트계 도막방수재의 실외 적용을 위한 기본 물성 연구

국내 건축 구조물 방수재료 중 하나인 수용성 고무 아스팔트계 도막 방수재는 주로 실내(화장실, 주방, 배란다 등)에 많이 사용되고 있다. 일반적으로 아스팔트계열의 재료는 검은색의 기본 채도를 벗어나지 못하여 노출용보다는 비노출용으로 많이 쓰이고 있으며, 수용성 고무 아스팔트계 도막방수재의 경우는 기본적으로 낮은 물성 때문에 실내로 한정하여 적용하고 있다. 이에 인장 및 신장성 향상을 위하여 실란커플링제와 무기필러, 프로세서 오일 넣어 물성을 향상시켰고, 이에 따라 실외 도막방수재 품질기준과의 기초 물성을 분석하였다. 그 결과 노출용 품질기준과 비교한 수용성 고무 아스팔트 도막방수재는 시험 평가가 진행된 전 항목에서 실리콘 고무계의 기본 물성 품질기준을 상회하는 결과가 나타났으나, 우레탄 고무계 1류의 인장강도 및 인열강도 클로로프렌 고무계 인열강도 품질기준 대비 성능이 약 34.2%에서 약 40.8% 정도 부족한 것으로 나타났다. Water-soluble rubber asphalt-based waterproofing material, which is one of the waterproofing materials for building structures, is mainly used indoors (toilet, kitchen, balcony, etc.). In general, asphalt-based materials are used for non-exposed installation, rather than as exposed type as they do not deviate from their usual basic black pigmentation, and water-soluble rubber asphalt-based coating waterproofing materials are basically limited to indoors because of their low physical properties. Accordingly, in order to improve the tensile and elongation properties, a silane coupling agent, an inorganic filler, and a processor oil were added to improve the physical properties, and accordingly, the basic physical properties of the outdoor coating waterproofing material quality standard were analyzed. As a result, the water-soluble rubber asphalt coating waterproofing material compared with the exposure quality standard showed a result that exceeded the basic physical property quality standard of silicone rubber in all items under test evaluation, but the tensile strength and tear strength of the first class of urethane rubber were chloroprene. It was found that the performance compared to the quality standards of rubber-based tear strength was about 34.2% to about 40.8%.

Self-excitations of edge flame were studied in laminar lifted free- and coflow-jet as well as counterflow flames diluted with nitrogen and helium. The self-excitations, originated from variation of edge flame speed and found in the above-mentioned configurations, are discussed. A newly found self-excitation and flame blowout, caused by the conductive heat loss from premixed wings to trailing diffusion flame are described and characterized in laminar lifted jet flames. Some trials to distinguish Lewis-number-induced self-excitation from buoyancy-driven one with O(1.0 Hz) are introduced, and then the differences are discussed. In counterflow configuration, important role of the outermost edge flame in flame extinction is also suggested and discussed.

Pilonidal sinus is one of a follicular occlusion tetrad, and is a chronic inflammatory disease that is associated with hair; a midline hairy patch or pit in the sacral region with a sinus orifice in the bottom, or a cyst beneath it, in which hair is growing. The exact pathogenesis of pilonidal sinus remains unknown, but hair insertion may be the essential cause of the disease. It is usually treated by radical excision, marsupialization or incision and curettage. However, these surgical options often lead to a high recurrence rate, especially for hirsute patients. Wide excision and simple primary closure of the pilonidal sinus is an easy, simple method and the recurrence rate is low. We report here the case of an 18-year-old male patient with recurrent pilonidal sinus mimicking inflammatory epidermal cyst. The pilonidal sinus was treated by wide excision and primary closure. Lots of Coal power plants (about 30) using bituminous coals are being run in Korea. The use of high volatile low grade sub-bituminous coal is increasingly extended because of imbalance between the worldwide coal supply and demand. Mill-fire has been an important issue since the use of such sub-bituminous coal. In existing coal plants of Korea, shutdown of coal and air supplies could be only a way, and an alternative has not been found in suppressing the mill fire. The inside fowfield in the mills has a highly fuel-rich, low temperature, and high velocity and non-reactive such that it could be a nonreactive system essentially. Nevertheless, occasional fire-occurrence could be attributed to the existence of an ignition source. However it has not been so far investigated in detail. The current work has a focus on suppressing the mile fire via some parametric experimental study such as effects of temperature, residence time, ignition source, and inert gas mixing. The results show that an small amount of CO2- or N2-mixing with air is very effective in suppressing fire formation even at high temperatures or flying sparks. The results suggest that exhaust gas recirculation into the mill should be an alternative to suppress mill fire.
