다중 중합효소연쇄반응 ( Multiplex PCR ) 방법을 이용한 한국인 자궁경부암 환자에서의 HPV 고 위험군 및 저 위험군의 동시 분석

목적 : 다중 중합효소연쇄반응 (Multiplex PCR) 방법을 이용하여 자궁경부암 환자에서 인유두종부 바이러스 (human papilloma virus; HPV)의 고 위험군과 저 위험군을 동시에 검색하고, 이를 진단 및 예후판정에 이용하고자 시도하였다. 연구방법 : 1995년 6월부터 11월까지 분당 차병원 산부인과에 내원하여 생검이나 수술적 적출술을 수행한 71명의 환자를 대상으로 실시하였다. Multiplex PCR 방법으로 한번의 RCR 수행으로 HPV 감염여부 및 고 위험군, 저 위험군을 동시에 typing하였다. 이때, HPV의 L1, E6, E7 DNA에 대한 3 set의 primer를 동시에 반응시켜 결과를 얻었다. 결과 : 총 68명의 자궁경부암 환자를 대상으로 HPV 감염여부 및 고 위험군과 저 위험군여부를 검색하였다. 이들을 자궁경부암의 진행정도에 따라 상피내종양 (cervical intraepithelial neoplasia; CIN)과 침윤암 (Invasive carcinoma; ICa)으로 구분하였다. 이들은 CIN I이 75% (9/12), CIN II가 75% (9/12), CIN III가 92.0% (23/25), 그리고 ICa가 94.7% (18/19)로 각각 나타났다. 또한, 고 위험군 (HPV16/18), 저 위험군 (HPV6/11) 및 기타 type이 각각 50.0%(34/68), 4.4%(3/68), 32.4%(22/68)로 나타났다. 전체적으로는 86.8%(59/68)의 HPV 감염 양성율을 나타냈다.결론 : 이러한 연구결과로 보아 다중 중합효소연쇄반응 (Multiplex PCR)은 자궁경부암 환자에서 HPV 고 위험군 및 저 위험군의 동시 분석에 매우 효과적인 방법으로 임상진단 및 예후판정에 널리 활용될 수 있을 것으로 기대된다. Objective : To investigate simultaneous detection of high and low-risk type of HPV in Korean cervical carcinoma using multiplex PCR method. Methods : HPV typing in cervical carcinoma was performed by multiplex PCR using three set of consensus primers for L1, E6, E7 of HPV DNA. Results : A total of 68 cervical carcinomas were examined. Histological examination revealed a specimen of carcinomas, which were classified as CIN I, CIN II, CIN III and invasive carcinoma (ICa). Of 68 cervical carcinomas, 75.0% (9 of 12) of CIN I, 75.0% (9 of 12) of CIN II, 92.0% (23 of 25) of CIN III and 94.7% (18 of 19) of invasive carcinoma (ICa) were positive for at least one type of HPV by the multiplex PCR. HPV 6/11, HPV 16/18 and other HPVs (general) were detected in 86.8% (59/68) were detected in 4.4% (3/68), 50.0% (34/68) and 32.4% (22/68) of cervical carcinomas of patients from Korea, respectively. Conclusion : We also confirmed that multiplex PCR is a simple, convenient, and cost-beneficial technique in simultaneous detection of high and low-risk type of HPV for genetic diagnosis in Korean cervical carcinoma patients.
