인터벤션 시술환자에 있어 체질량 지수별 방사선 차폐 보조기구 사용 유무에 따른 방사선 방어 효과 비교

목 적 인터벤션 시술환자에 있어 체질량 지수별 방사선 차폐 보조기구 사용 유무에 따른 방사선 방어 효과에 대하여 알아보고자 하였다. 대상 및 방법 2010년 7월부터 8월까지 DAP와 피부선량 측정이 가능한 Siemens사의 AXIOM artis dTA 장비를 이용하여 중심정맥 카테타 삽입술을 시행한 환자 60명 (M:F=29:31, Age:50.6 (Min:15, Max:85), BMI:22.6(Min:12.9, Max:35.9))을 대상으로 갑상선을 차폐한 쪽 30case를 실험군으로 다른 30case는 갑상선을 차폐하지 않은 대조군으로 설정 후 체질량을 기준으로 방사선 피폭선량 측정 후 비교, 검토하였다. 결 과 체질량지수(BMI)에 따른 DLP와 피부선량의 결과값은 과체중으로 갈수록 DLP와 피부선량 값이 증가하는 것을 알 수 있었으며 단지 방사선 차폐 보조기구의 사용 유무에 따른 DLP와 피부선량의 결과값은 차폐를 하였을 때가 약간 더 많은 피폭선량을 받는 것으로 나타났다. 또한 BMI을 기준으로 방사선 차폐 보조기구의 사용 유무에 따른 DLP와 피부선량의 결과값은 BMI에 따른 DLP와 피부선량 값은 차이가 거의 차이가 없는 것으로 나타났다. 결 론 방사선 차폐 보조기구 사용 유무에 따른 피폭선량 역시 보조기구를 사용 할 때와 사용하지 않을 때 별 차이가 없는 것으로 나타났다. 따라서 DSA 투시장비의 자동선량제어 장치 영향으로 효과적인 피폭 선량 최소를 위해서는 Collimator 사용과 최소 투시시간이 중요한 것을 알 수 있었다.
간호학 문제 중심 학습 효과 평가 - 일개 교과목을 중심으로

Purpose: The purpose of this research was to evaluate the application of the Problem-Based Learning(PBL) in nursing with regard to the learning process and learning outcomes. Method: PBL modules were provided to the students so that they could identify the nursing problems related to nutrition, regulation, and elimination, and implement appropriate nursing interventions according to the problems. PBL was also used to develop ability to use self-evaluation for nursing intervention outcomes. The modules were developed by the authors for the course, Nursing 1(3 credits) at Y university, and used during one semester, March to June 2003. Evaluation of the learning process and learning outcomes was done by self-report questionnaires and a semi-structured self -report developed by the authors. Result: The PBL evaluation included role of the facilitators, group work process, and self-directed learning attitude. The students evaluated the facilitator's role positively in fostering positive interactions and cooperative study among students and stimulating students to apply various problem-solving strategies. Students evaluated their own group work performances as a good opportunity to improve their group work participation, contribution, cooperation, and leadership through the PBL session. Their responsibility for their own learning and develop self-directed learning attitudes to connect present learning to long-term goals. Conclusion: Critical thinking, problem solving skills, self-directed learning and group work accomplishments in undergraduate student nurses were fostered and improved through the PBL program.
복식호흡이 조기진통 임부의 불안과 자궁수축 빈도에 미치는 효과

Purpose: This pilot study was aimed to examine the effect of abdominal breathing on the frequency of uterine contraction and anxiety of pregnant women with preterm labor. This study utilized pre-experimental design and nonequivalent control group pre-post test quasi-experimental design. Method: Subjects were 35 women in total; 5 in experimental group and 30 in control group. Following the protocol by Shim Joung-Ohn(2005), pregnant women who were admitted to the hospital for preterm labor were assigned to the experimental group. They were instructed to perform abdominal breathing 25 times at the start of uterine contraction, which took 5 minutes. Three sets of 5-minutes abdominal breathing(25 times) were performed daily for three days. Before and after the abdominal breathing of the experimental group, Visual Analogue Scale was employed in order to assess the psychological anxiety level of the experimental group. Blood pressure, pulse, and skin temperature were measured to obtain data on the physical anxiety level. Frequencies of uterine contraction were collected using tocodynamometer. For the women in the control group, nursing and medical records were reviewed. The collected data were analyzed using the SPSS program. Results: After abdominal breathing, there was a statistically significant decline in the psychological anxiety level of the experimental group(t=3.11, p=.01). With regard to the physical anxiety level, no statistically significant drop was detected in blood pressure and pulse, skin temperature rose significantly(t=-5.98, p=.00). There was no statistically significant reduction in the frequency of uterine contraction(t=1.94, p=.06). Conclusion: The findings of the study confirmed that abdominal breathing reduced the psychological anxiety of pregnant women with preterm labor and raised their skin temperature, one of the indicators of relaxation response. Besides, the frequency of uterine contraction tended to decrease, although the degree of the decrease was not statistically significant. This suggests that abdominal breathing has a positive effect on uterine muscle relaxation.

Purpose: The purpose of this study is to clearly classify the roles of Physician assistants and to identify obstacle factors and necessary factors for the operation of the system when they are performing the roles of Physician assistants. Methods: This is a descriptive research study to analyze the roles of Physician assistants and to understand the expected roles of Physician assistants according to each medical personnel (doctor, nurse, Physician assistants). Results: The obstacles to work are the lack of legal protection, and unclear work guidelines, and the matter necessary for the operation of the system is the legal protection. There were statistically significant differences in health collection ability, physical examination, drug response monitoring, and reporting in terms of the differences in role expectations considered by medical professionals regarding the roles of Physician assistants. Conclusion: Physician assistants need to develop various educational programs and materials, cultivate the ability to perform Physician assistants, and develop an advisory role through effective problem-solving between departments and increase the quality and satisfaction of Nursing care.
