Background: Ambulatory surgery has the advantages of reduced demand for postoperative medications, faster recuperation, less disruption to personal life and reduced hospital costs. In order to discharge the patients on the day of surgery, specialized cares and skills might be required for ambulatory anesthesia. We reviewed ambulatory anesthesia for orthopedic lower extremity surgeries to investigate the efficacy of the anesthesia for ambulatory surgeries. Methods: Ambulatory surgeries were reviewed from July 1996 to August 2001. Medical records of 365 cases of ambulatory anesthesia for orthopedic lower extremity surgeries were reviewed. Patient's demographic profile, coexisting systemic disease and anesthetic techniques were reviewed The incidence of postoperative complication and eases of canceled operation were also analyzed. Results: Outpatients for lower extremity orthopedic surgery were mainly classified as physical status 1 and 2 by American Society of Anesthesiology (99.4%). The most common surgery was arthroscopic surgery and examination (35.6%). The most common anesthetic technique was general anesthesia using LMA (Laryngeal Mask Airway) and propofol (58.7%). The incidence of postoperative complication was 11.7% of total cases. The majority included the minor complications such as sore throat, mild hypertension, nausea, vomiting and pain. Conclusions: The incidence of postoperative complication, canceled operation and unexpected admission due to untolerable side effects were low. Minor orthopedic surgical procedures can be efficiently and successfully managed using simple ambulatory anesthesia techniques.
Three Cases of Spontaneous Intracranial Hypotension (SIH) Treated with Epidural Blood Patch

Spontaneous intracranial hypotension, a syndrome of low CSF pressure, occurs without preceding events such as lumbar puncture, back trauma, operative procedure or medical illness. The most char- acteristic symptom is an occipital or frontal headache which is aggrevated in the erect position and relieved in the supine position. This syndmme usually resolves spontaneously or with strict bed rest. When the headach persists or is incapacitating, more aggressive treatment may be necessary. Autologous epidural blood patch is highly effective in the management of spontaneous intracranial hypotension. Epidural blood produces an organized clot which effectively tamponade any dural CSF leak The rapid relief of headache immediately after the infusion of blood occur by some other mechanism, such as an increase in subarachnoid pressure that is known to occur with infusion of fluid into the lumbar epidural Space. We report three cases of spontaneous intracranial hypotension successfully treated with epidural blood patch.

As modern people's interest in health grows, wearable healthcare-related technologies that can collect biometric information and receive health-related services regardless of time and place are developing. This paper proposes the healthcare system based on voice recognition function. Unlike conventional wearable healthcare systems, the proposed system provides real-time telemedicine and diagnosis and the direct connection with doctors is possible through it. And almost all functions of the application in it are operated with voice commands to lower user access barriers. Because it deals with personal information including medical information, encryption algorithms have been applied between all data movements. Finally, the healthcare system proposed in this paper is expected to increase convenience in health care and reduce the medical gap.
