      • Techzyme® HBsAg, Anti-HBs 시약의 유용성 평가

        배경:B형 간염은 우리나라에서 만연하고 있는 질환중의 하나이며, 이에 대한 혈청학회 검사는 B형 간염 진단에 중요하다. 최근 개발된 국산시약인 Techzyme®(Asan, Korea)을 이용하여 B형 간염의 표면 항원(HBsAg ), 항체 (Anti-HBs)의 임상적 유용성을 검토하고자 하였다.<br> 방법: 2001년 1월부터 3월 사이에 충남대학교 병원 임상병리과에 의뢰된 150검체를 대상으로 민감도와 특이도를 검사하였으며 Techzyme®음성대조 물질과 양성 대조물질을 이용하여 검사 중 정밀도와 검사간 정밀도를 구하였다. 고농도 환자 혈청을 단계 희석한 후 직선성을 구하였다.<br> 결과:정밀도에서 음성 표준물질인 경우 일간 재현성 검사와 일일 재현성 검사에서 HBs Ag은 변이계수가 7.48%와 11.2%이고 Anti-HBs는 7.02%와 14.6%이었다. 양성표준물질인 경우 일간 재현성 검사와 일일 재현성 검사에서 HBsAg은 변이계수가 2.34%와 4.14%이고 Anti-HBs는 2.43%와 4.97%이었다. 직선성의 평가에서는 HBsAg의 경우 R<sup>2</sup>=0.9903이고 Anti-HBs는 R<sup>2</sup>=0.9977이었다. 항원, 항체 검사에서 94%, 92%의 일치율을 보였고 민감도와 특이도는 항원 검사에서는 100%, 100%이고 항체 검사는 각각 95.6%,96.3%이었다.<br>결론: 새로 개발된 국산시약 Techzyme®은 우수한 정밀도와 직선성을 보였고 민감도와 특이도가 높아 임상적으로 유용하게 사용될 수 있으리라 생각된다. Background: The hepatitis B virus was one of the most prevalent infectious agent in Korea. Serologic assays of the detection for hepatitis B surface antigen (HBsAg) and hepatitis B antibody(Anti-HBs) were useful markers for the diagnosis of viral hepatitis. Some of domestic hepatitis enzyme immunoassay (EIA) reagents have been developed. We analyzed one of them, Techzyme® ELISA 3.0 HBsAg and Anti-HBs (Asan, Korea), to evaluate the clinical usefulness.<br> Methods: The precision, linearity, sensitivity and specificity of Techzyme® HBsAg and Anti-HBs were evaluated with 150 clinical specimens from January to March, 2001.<br> Results: The within-run and between-run coefficient of variance (C.V.) HBsAg were 7.48% and 11.12% in negative control and were 2.34% and 4.14% in positive control, respectively. The within-run and between-run CV for Anti-HBs were 7.02% and 14.6 in negative control and 2.43% and 4.97% in positive control. On linearity test, the R<sup>2</sup> for HBsAg was 0.9903 and the R<sup>2</sup> for Anti-HBs was 0.9977. The concordant results were 94% and 92% for HBsAg and Anti-HBs with conventional reagents, respectively. The sensitivity and specificity for HBsAg were 100%, both. And the sensitivity and specificity for Anti-HBs were 95.6% and 96.3%, respectively.<br> Conclusions:In conclusion, because of high sensitivity, specificity and high linearity, Techzyme® ELISA 3.0 HBsAg and Anti-HBs appeared to be suitable for detecting HBs Ag and Anti-HBs in clinical laboratories.

        비형식 학습으로의 대학 온라인 교육콘텐츠 활용에 대한 의미연결망 분석

        The purpose of this study, using semantic network analysis, we structurally grasp the experiences and perceptions of using university online learning educational contents as informal learning, and examine their meanings, thereby providing basic data for the implementation of learner-centered contents and efficient online educational contents management. The results showed that students associated online learning experiences with words such as diversity, applicability, search, interest, accessibility, knowledge, and various fields. Such findings demonstrate that university students experience and utilize their knowledge fields through online learning. They also perceived such learning methods to be useful for self-development, and trusting the content curation provided, thinking that it was helpful. Through semantic network analysis, this study, therefore, identifies connectivity and structural relationships among key words and quantitatively visualizes learners' qualitative experiences through them in a scientific manner. In particular, the study is meaningful for the development and management of online learner-centered contents. In the future, it will be more important to leverage such informal online learning observed in the study and to continuously improve on shortcomings to provide a learning experience that is tailored each and every individual learner. 본 연구에서는 비형식 학습으로의 대학 온라인 교육콘텐츠 활용에 대한 의미연결망 분석을 통해 학습자들의 경험과 인식을 구조적으로 파악하고 그 의미를 살펴봄으로써 학습자 중심의 콘텐츠 구현과 효율적 온라인 콘텐츠 운영을 위한 기초자료를 제공하고자 하였다. 연구결과, 온라인 교육콘텐츠 학습경험에 대한 주제어는‘다양함, 활용, 검색, 흥미, 접근, 지식, 분야’등으로 나타났다. 이는 대학생들이 온라인 교육콘텐츠 학습을 통해 흥미 있는 분야에 대한 다양한 지식을 경험하고 활용하는 것으로 볼 수 있다. 대학생들은 온라인 교육콘텐츠 학습을 자기계발을 위해 필요한 지식을 쌓는 것으로 인식하고 있었으며, 제공되는 콘텐츠 큐레이션을 신뢰하고 도움이 된다고 생각하고 있었다. 본 연구는 의미연결망 분석을 통해 주요 키워드들 사이의 연결성과 구조적 관계를 파악하여 학습자의 질적 경험과 의미를 과학적 방법으로 분석하고 시각화하였다. 특히 비형식으로 이루어지는 온라인 교육콘텐츠 학습경험에 대한 양적, 질적 자료가 부족한 현실에서 학습자 중심 콘텐츠의 개발과 운영을 위한 시사점을 제공했다는 점에서 의의가 있다. 향후 본 연구를 통해 나타난 비형식 학습으로의 대학 온라인 교육콘텐츠 활용 경험에 대한 유용성을 활용하고 한계점은 보완하여 학습자 맞춤형 온라인 교육콘텐츠를 제공할 필요가 있다.

        ADHD 청소년의 내재화 증상 감소를 위한 공예 중심 미술치료 프로그램의 효과

        The purpose of this study was to develop a craft-centered art therapy program and examine it’s effects on the symptoms of internalization of an adolescent with ADHD. The client, selected by CAPRS for this study, was a 15-years old middle school student. Estimated impulsivity score of the client was high and the symptoms of internalization were serious. She also had various behavioral problems. In this study, a 20-sessions craft-centered art therapy program was carried out from March the 1st, 2015 to August the 30th, 2015. The HTP, for the qualitative analysis, and YSR, for the quantitative analysis, were used to examine the effects of the program. The significant results of this study indicate that the craft-centered art therapy program is very effective to decrease of symptoms of internalization of a adolescents with ADHD. And based on this result, art therapy programs which is focused on craft are expected to be used to decrease the symptoms of internalization of adolescents with ADHD in the schools. 이 연구에서는 공예 중심 미술치료 프로그램을 개발하고 프로그램이 ADHD 청소년의 내재화 증상에 미치는 변화를 알아보고자 하였다. 연구대상인 내담자는 CAPRS를 통하여 선별한 중학교 2학년 여학생(만14세)이다. 대상자는 충동적인 성향이 강하고 내재화 증상이 심각하여 여러 가지 문제행동을 경험하고 있다. 2015년 3월 1일부터 8월 30일까지 프로그램을 실시하였고 총 20회기로 주 1~2회기씩 이루어졌다. 각 회기는 중학생의 성장 특성을 고려하여 60~70분으로 집중도 있게 진행하였다. 프로그램의 효과를 검증하기 위해 이 연구에서는 질적 분석 방법으로 HTP 사전‧사후 검사 결과와 회기별 작품의 변화 과정을 분석하였다. 양적분석 방법으로 YSR의 내재화 점수와 그 하위요인인 불안/우울, 위축/우울 점수의 사전‧사후 검사 결과를 분석하였다. 이 연구의 주요 결과로서, 공예 중심 미술치료가 ADHD 청소년의 내재화 문제를 감소시키는 데 효과가 있었다. 이렇게 볼 때, 이 공예 중심 미술치료 프로그램은 향후 ADHD 청소년의 내재화 문제 감소를 위해 학교장면에서 긍정적으로 활용할 수 있을 것으로 기대된다.

        Lung Ultrasonography for the Diagnosis of Respiratory Distress Syndrome in Late Preterm Infants: Changing Incidence – A Single Center Experience

        Purpose: Ultrasonography is non-ionizing, easy to operate, and performed at bedside in neonatal intensive care unit (NICU). We investigated the incidence of respiratory distress syndrome (RDS) with or without using lung ultrasound (LUS) in late preterm infants with postnatal respiratory difficulties. Methods: We retrospectively reviewed medical records of 494 late preterm infants born at 34–36 weeks’ gestation at Keimyung University Dongsan Medical Center. Fifty infants with postnatal respiratory difficulties were admitted to the NICU between May 2015 to October 2015 (period I), and forty-one were between November 2015 to February 2016 (period II). The diagnosis of RDS was based on chest radiography in period I. LUS was additionally performed at bedside in period II. All infants with RDS were received exogenous surfactant therapy. Results: The overall incidence of RDS with surfactant replacement therapy was decreased in period II period II (9.4%, 20/212) compared to period I (14.5%, 41/282) (P=0.088). In terms of infants with postnatal respiratory difficulties, the incidence of RDS in period II (48.8%, 20/41) was significantly lower than that in period I (82.0%, 41/50) (P=0.001). There are no difference in the rate of reintubation, repeated doses of surfactant, oxygen demand at 48 hours after birth, air leak syndrome, pulmonary hemorrhage, persistent pulmonary hypertension of newborn, and mortality (P> 0.05). Conclusion: We could decrease the incidence of RDS with surfactant replacement therapy by using LUS in late preterm infants with postnatal respiratory difficulties. Further prospective studies are needed to apply LUS clinically to diagnose RDS.

        급성백혈병 완전 관해 골수에서 항원형 이상 세포의 분포

        Background : The aberrant, leukemia-associated antigen expression patterns allow us to discriminate leukemic blasts from normal precursor cells. Our major goal was to determine a guideline for the detection of minimal residual disease using CD20+/CD34+ and myeloid Ag+/CD19+ combination in the bone marrow of acute leukemia in complete remission (CR) after chemotherapy. Methods : Bone marrow samples from 117 patients with acute leukemia in complete remission after chemotherapy and from 22 healthy controls were immunophenotyped by triple staining and measured by flow cytometry . Results : The CD20+/CD34+ cells in the large lymphocyte gate (R1) ranged from 0% to 3.24% (0.8±0.82%, P=0.000) in CD20+/CD34+ B-lineage ALL CR (N=31), from 0.03% to 4.2% (0.7±0.83 %, P=0.000) in CD20-/CD34- B-lineage ALL CR (N=66), from 0.1% to 0.96% (0.45±0.32%, P=0.016) in T-ALL CR (N=10), and from 0.02% to 0.48% (0.18±0.15%, P=0.776) in AML CR (N=10). The CD13,33+/CD19+ cells in R1 gate ranged from 0% to 2.69% (0.37±0.48%, P<0.001) in CD13,33+/ CD19+ B-lineage ALL CR (N=31), from 0% to 1.8% (0.31±0.28%, P<0.001) in CD13,33-/CD19+ B-lineage ALL CR (N=65), from 0.02% to 0.64% (0.29±0.22%, P=0.071) in T-ALL CR (N=9), and from 0% to 0.17% (0.07±0.09%, P=0.341) in AML CR (N=3). Conclusions : Using an immunophenotypic method for the detection of early relapse or minimal residual disease of B-lineage ALL bone marrow in CR after chemotherapy, different cutoff values should be applied according to antigen combination and gating. When the proportion of aberrant antigen combination was less than 5% in large lymphocyte gate, the results should be interpreted with caution. (Korean J Lab Med 2008;28:1-7)

        전공영어강의에 대한 교수들의 인식 및 운영방안 탐구

        The Journal of Modern British & American Language & Literature. 30.2 (2012): 1-25. The present study investigates Korean university professors' perceptions of English-medium instruction(EMI), attempting to provide practical recommendations for future EMI implementation. An on-line questionnaire was administered to 82 professors in diverse disciplines at a private university in Korea, regardless of whether they have offered EMI or not prior to participating in the study. The data was analyzed quantitatively using a t-test and ANOVA. The survey results indicate that most of the professors were well aware of the importance and necessity of EMI in light of the global competitiveness of the university and its students, and that most felt they were ready to offer English-medium classes. The professors were also shown to have a high level of efficacy in their use of English, and they thought positively of future linguistic support from the university. The professors were however concerned that EMI may not be as effective as Korean lectures and that it will serve to lower the overall quality of university education given the difficulties students might experience due to their inadequate level of English proficiency. In facilitating the introduction and the promotion of EMI at the institutional level, we suggest that systematic support measures both for professors and students should be in place; i.e., workshops can be organized for professors to introduce an EMI-specific teaching methodology, and students can be assisted by offering preparation courses to undertake EMI and language support.
