심장정지 환자에 대한 일반시민 심폐소생술 수행능력 향상방안 연구

우리나라에서 심장정지 환자는 매년 30,000건 이상 지속적으로 발생하여 연간 사망자수의 10%를 점유한다. 그러나 심장정지 발생 후 골든타임 내에 현장에서 일반 시민에 의한 심폐소생술은 24.7%에도 미치지 못하는 상황으로 일본의 48.4%비해 큰 격차를 보이고 있다. 현장에서 심폐소생술을 수행하면 생존율과 뇌기능 회복률은 1.9배-6.2배로 높아진다. 현장에서 시민의 심폐소생술 수행능력 저하는 심장정지환자의 생존율과 회복률 저하로 이어져 심각한 사회문제를 야기 시킨다. 따라서 본 연구는 일반시민의 현장 심폐소생술 수행능력 향상을 위한 대안제시에 목적이 있다. 이를 위해 문헌 및 정부기관 보도자료 등 선행연구와 온라인과 오프라인으로 진행한 소방재난안전교육에서 참여자 집단별로 설문조사, 질의응답과 이메일 조사 및 전화인터뷰방식으로 조사하였으며, 조사대상 집단별 1,000명으로 7개 집단 7,000명을 모집단으로 하였다. 연구 결과 우리나라에서 일반시민은 심장정지 환자 발생 시 현장에서 골든타임 내에 응급처치인 심폐소생술을 환자에게 신속하게 수행하기 위해서는 교육이수경로의 다양화, 교육의 장 편리한 접근성(수월성) 확보, 교육이수자에 대한 인센티브 제공 등의 법과 제도적 측면에서의 정책변화를 요구하고 있는 것으로 조사 분석되었다. 결론적으로 일반시민을 심폐소생술 학습의 장으로 유인하기 위해서 동기부여가 되는 핵심변수는 경제적 요인의 제공으로 귀결된다. 이는 교육이수자에게 마일리지제도 등을 통해 국민건강보험료와 사적영역에서의 개별 보험가입자에게 보험료에 대한 일정금액을 감면하여 줌으로서 자발적 참여 집단의 확대를 유인하는 것이 핵심동력이라 하겠다. 이러한 정책변화는 보험시장의 재정상 건전성 확보에도 유의미한 결과로 나타날 것이다. 남성에 비해 여성이 심폐소생술 교육이수 및 체험기회의 접근성(수월성)이 상대적으로 제한되어 결과적으로 심폐소생술 수행능력이 부족한 것으로 분석되었다. 따라서 가정경제에 민감성이 높은 여성의 심폐소생술 체험과 교육이수의 장으로 유인하기 위한 법과 제도적으로 유연한 정책변화가 요구된다. In Korea, more than 30,000 cases of cardiac arrest continue to occur every year, accounting for 10% of the annual death toll. However, cardiopulmonary resuscitation by ordinary citizens is less than 24.7 percent within Golden Time, a wide gap compared to 48.4 percent in Japan. Cardiopulmonary resuscitation in the field increases survival rates and brain function recovery rates by 1.9 to 6.2 times. In the field, citizens ability to perform CPR leads to a decrease in survival rate and recovery rate of cardiac arrest patients, causing serious social problems. Therefore, this study aims to present alternatives to improve citizens ability to perform on-site CPR. To this end, prior research such as literature and government press releases, and fire and disaster safety education conducted online and offline were conducted by survey, question and answer, e-mail survey, and telephone interview. Seven groups of 7,000 people were surveyed. According to the study, ordinary citizens in Korea demand changes in law and institutional policies such as diversifying education channels, securing convenient accessibility, and providing incentives for education recipients in order to quickly perform CPR, which is a first aid, in case of cardiac arrest. In conclusion, key variables that motivate ordinary citizens to enter the field of CPR learning result in the provision of economic factors. This is a key driver to induce the expansion of voluntary participation groups by reducing a certain amount to individual policyholders in the national health insurance premiums and private areas through the mileage system. This policy change will also have significant consequences for securing the financial health of the insurance market. Compared to men, it was analyzed that women lacked the ability to perform CPR due to the limited access route (Suwolseong) of CPR training courses and experience opportunities. Therefore, laws and institutional flexible policy changes are required to attract women who are highly sensitive to the home economy to the cardiopulmonary resuscitation experience and education courses.

소방특별사법경찰의 수사에 관한 직무집행능력 향상 방안연구

본 연구는 소방특별사법경찰을 중심으로 소방행정영역에서 소방법령위반자에 대한 수사주체로써 전문성 확보 수단인 인사 및 채용제도 개선, 통일된 수사전담직제 신설, 교육훈련체계 강화로 수사에 관해 직무집행능력향상방안을 제시하는데 목적이 있다. 연구범위는 소방특별사법경찰의 수사직무 수행과정에서 제기되는 문제점을 개선하기 위한 전문성 확보 수단인 인사 및 채용제도 개선, 통일된 수사전담직제 신설, 교육체계 개선에 한정하였다. 연구방법은 관련 선행연구논문, 소방청 등 통계연보, 미국 등 외국의 운용사례, 정부 각종 보고서, 우리나라 정부부처홈페이지, 언론보도 등을 분석하였다. 연구분석결과 수사직무전담인력은 소수에 불과하고 대부분은 타 직무와 겸직을 하고 있고, 특별사법경찰 수사직제는 과 단위 직제도없이 독립성이 확보하지 못하고 있으며, 전문성 확보를 위한 일선 수사직무수행 소방장 이하에 대한 공채 또는 특채 채용제도가 없을 뿐만 아니라 직무교육을 위한 교육과정개설도 미흡하였다. 결론적으로 소방특별사법경찰수사분야를 신설하여 공채 또는 특채로 채용하는 제도 시행, 현재의 순환보직 인사제도를 지양하고 소방특별사법경찰 직무를 전문분야로 지정하여 직무 연속성과 전문성 확보, 소방특별사법경찰 직무를 담당하는 통일된 직제를 신설하여 소방청과 소방본부에 과 단위 직제와 일선 소방서에 팀 단위 직제 편재 운영, 전문성을 담보할 수 있는 직무교육과정을 개발하여 최소한 6개월 이상의 전문교육을 이수하는 교육제도 도입 등이 조속히 시행되어야 할 것이다. The purpose is to present. The research method was an exploratory analysis of related previous research papers, statistical yearbooks such as the National Fire Agency, operation cases in foreign countries such as the United States, various government reports, homepages of Korean government ministries, and media reports. As a result of the analysis, there are only a very small number of personnel dedicated to investigative work, most of them hold other positions concurrently, the investigative position does not have a division-level structure, so independence is not secured, there is no public recruitment or special recruitment system for fire chiefs or lower to perform front-line investigative work, and no training for job training. Course opening was also insufficient. In conclusion, avoid the new investigative field recruitment or special recruitment system and rotational personnel system, designate the fire and special judicial police duties as a specialized field, ensure job continuity, establish a unified organization, develop a job training course, and complete at least 6 months of professional training. The introduction of an education system should be implemented quickly.
