투명캡을 씌운 내시경으로 치료한 바터 팽대부 Dieulafoy 병변 출혈 1예

Dieulafoy 병변은 반복적이며 대량으로 위장관 출혈을 일으킬 수 있는 드문 원인으로 주위 궤양이나 염증이 없는 작은 점막 결손 부위를 통해 점막하 세동맥이 위장관으로 노출되어 출혈을 유발하는 질환이다. Dieulafoy 병변은 주로 위에서 발생하지만 드물게 식도, 소장 또는 대장에서도 발생한다. 십이지장의 Dieulafoy 병변은 드물고, 또한 바터 팽대부에 발생한 Dieulafoy 병변에 대한 증례는 과거 국내에서 1예만 보고되었다. 저자들은 토혈과 흑색변을 주소로 내원한 환자에서 상부위장관 내시경을 시행하여 출혈의 원인을 바터 팽대부의 Dieulafoy 병변 출혈로 진단하고 내시경의 선단에 투명캡을 씌운 후 지혈 클립술과 에피네프린 국소 주입법으로 치료한 증례를 경험하여 문헌 고찰과 함께 보고한다. Dieulafoy’s lesion is a rare cause of repetitive and massive gastrointestinal bleeding, and this is characterized by an isolated arteriole protruding through a small mucosal defect. Dieulafoy’s lesion is generally found in the stomach within 6 cm of the gastroesophageal junction, and usually on the lesser curvature, but many lesions have been reported in extragastric locations, including the esophagus, small bowel and rectum. A Dieulafoy’s lesion in the ampulla of Vater is extremely rare, and only one such case has been reported in the Korean population. We experienced a rare case of Dieulafoy’s lesion in the ampulla of Vater with massive pulsatile bleeding, and this was successfully treated by transparent cap-assisted endoscopic hemoclipping. We report here on this case with a review of the relevant literature.
