        Histologic Disorderliness in the Arrangement of Tumor Cells as an Objective Measure of Tumor Differentiation

        Background: Inter-observer and intra-observer variation in histologic tumor grading are well documented. To determine whether histologic disorderliness in the arrangement of tumor cells may serve as an objective criterion for grading, we tested the hypothesis the degree of disorderliness is related to the degree of tumor differentiation on which tumor grading is primarily based. Methods: Borrowing from the statistical thermodynamic definition of entropy, we defined a novel mathematical formula to compute the relative degree of histologic disorderliness of tumor cells. We then analyzed a total of 51 photomicrographs of normal colorectal mucosa and colorectal adenocarcinoma with varying degrees of differentiation using our formula. Results: A one-way analysis of variance followed by post hoc pairwise comparisons using Bonferroni correction indicated that the mean disorderliness score was the lowest for the normal colorectal mucosa and increased with decreasing tumor differentiation. Conclusions: Disorderliness, a pathologic feature of malignant tumors that originate from highly organized structures is useful as an objective tumor grading proxy in the field of digital pathology.

        Pharmacokinetics and extensive intestinal first-pass effects of apigenin and its active metabolite, apigenin-7-O-glucuronide, in rats

        Purpose This study investigated the metabolism and pharmacokinetics of apigenin and its metabolite, apigenin-7-O-glucuronide (A7G), using a newly developed UPLC-MS/MS method. Methods A simple and sensitive UPLC-MS/MS method was developed for simultaneous quantification of apigenin and A7G, and their pharmacokinetic properties were examined using an in vivo rat model. Moreover, stability under various conditions, protein binding, blood distribution, and metabolic studies were performed in vitro. Results Apigenin showed poor stability in simulated intestinal fluid, whereas A7G was stable for 24 h. An in vivo pharmacokinetic study demonstrated that apigenin had a very low oral bioavailability (F) of 0.708% and was mainly metabolized to A7G. Notably, systemic exposure (Cmax and AUC) of apigenin after oral administration of A7G was markedly higher (2.62- and 14.3-fold, respectively) than that after oral administration of apigenin. Apigenin and A7G were significantly metabolized in both hepatic and intestinal S9 fractions. Based on the well-stirred and QGut model concepts, they were classified as compounds with low EH (0.0167–0.0389) and moderate-to-high EG (0.626–0.979) in rats, indicating that the intestine had a greater contribution than the liver to pre-systemic elimination of both phytochemicals. Conclusion The low F of apigenin could be attributed to its poor stability in the gastrointestinal lumen and extensive intestinal first-pass effect, which could be improved by oral administration of A7G, demonstrating the potential of A7G as a natural prodrug for improving the low F of apigenin.

      • 거골 체부에 발생한 양성 종양

        Purpose: Benign bone tumor of the talar body have rarely been evaluated according to the therapeutic methods because of the scarcity of their incidence. Here, we report our experience of 8 cases who were treated by using of posterior approach and curettage through the posterior process of the talus. Materials and Methods: Between February 1986 and October 2001, we experienced 8 cases of benign bone tumor occurring in the talar body. They included two osteoid osteomas, two giant cell tumors, one capillary hemangioma, one chondroblastoma, one simple bone cyst, and one osteochondroma. Their mean age was 22.1 years (ranging from 10 to 41 years). Mean follow-up period was 7.7 years (ranging from 1 to 16 years). All patients were treated by using of posterior approach. Two osteoid osteomas and one osteochondroma were treated by excision of tumors. Other cases were treated with curettage through the cortical window on the posterior process of the talus. Results: There was no recurrence during the follow-up period. one infection occurred. Except this case, all patients had no pain in weight-bearing, and complete range of movement at the ankle joint was reserved in each case. Conclusion: In this study, we suppose that posterior approach to the talar body may be a safe method with minimal damage of normal tissues and sufficient of curettage is capable through the cortical window on the posterior process of the talus.

      • 연부 조직 종양에서 PET의 유용성: 기존의 진단법과의 비교 연구

        Introduction: Currently, F-18 fluorodeoxyglucose positron emission tomography scans (FDG-PET) has been investigated in soft tissue tumor especially for tumor detection and noninvasive grading. However, the validity and the efficacy of FDG-PET are still unclear in clinical evaluation. The purpose of this study is to determine the efficacy of FDG-PET in compared to conventional diagnostic imaging studies currently used in the soft tissue tumor. Methods: Between March 2001 and March 2002, 29 patients (sixteen males, thirteen females, mean age, 47 years; a range from 4 to 73) diagnosed with soft tissue tumor were evaluated by both conventional diagnostic imaging and FDG-PET. Valid reference test of the local lesion was the histopathologic diagnosis, which was measured in all patients. The suspecting metastasis in the imaging studies was validated by pathology or follow up imaging for at least 6 months. Each imaging diagnosis was made independently. The accuracy of each diagnostic method was evaluated. The incremental cost accuracy ratio was determined in each diagnostic method. Results: For detection of local lesion, sensitivity, specificity, and accuracy for MRI and FDGPET scans were 91%, 57%, 83% and 95%, 43%, 83% respectively. For detection of distant lesion, sensitivity, specificity, accuracy for conventional diagnostic methods and FDG-PET scans were 77%, 89%, 87% and 92%, 94%, 93% respectively. The incremental cost accuracy ratio (ICAR) of FDG-PET for detection of distant lesion was 145,000won/%. According to ICAR for each tumor grade, PET strategy is most cost-effective at high grade tumors. Conclusions: For detection of local lesion such as recurrence or remnant tumor, FDG-PET scan was not more accurate than MRI. However, It was more accurate for detection of metastatic lesion than conventional methods. For detection of high grade tumor, PET was most costeffective than for detection of lower grade tumor.

        A Case of Postoperative Sphingomonas paucimobilis Endophthalmitis After Cataract Extraction

        Purpose: To report a case of an acute onset of delayed postoperative endophthalmitis that was caused by Sphingomonas paucimobilis. Methods: This case demonstrates an acute onset of delayed postoperative endophthalmitis at 3 months after uneventful cataract extraction and posterior chamber intraocular lens implantation. We performed vitrectomy, intraocular lens and capsular bag removal, and intravitreal antibiotics injection. On the smear stains from the aspirated vitreous humor, gram-negative bacilli were detected and S. paucimobilis was found in culture. Results: At three months after vitrectomy, the best corrected visual acuity was 20/300. Fundus examination showed mild pale color of optic disc and macular degeneration. Conclusions: Vitrectomy with intravitreal ceftazidime injection had contributed to the favorable result in case of an acute onset of delayed postoperatire endophthalmitis caused by S. paucimobilis.

        마이크로파 처리 유무에 따른 우엉뿌리의 항비만 효과 비교 연구

        Objective : The present study comparatively analyzed the anti-obesity effect of Arctium lappa L. roots with and without microwave processing. Methods : Four HFD groups except for the Normal group (n=8) were allocated: Control, microwave-processed dried Arctium lappa L. roots (MAL) extract 400 mg/kg/d (MAL400), MAL extract 800 mg/kg/d (MAL800), and the dried Arctium lappa L. roots (DAL) extract 800 mg/kg/d (DAL800). The efficacy of MAL and DAL was confirmed in terms of the serum biochemical index, protein expressions related to the synthesis of triglyceride (TG) and total cholesterol (TC) and β-oxidation, and histopathological staining. Results : Both MAL and DAL treatments significantly reduced final body weight and body weight gain. MAL800 treatment significantly reduced serum TG, TC, low-density lipoprotein cholesterol, and leptin levels, but the serum high-density lipoprotein cholesterol and adiponectin concentrations were dramatically increased. In particular, leptin in the MAL800 group was reduced by 14.1% compared with the DAL800 group. Moreover, the MAL800 treatment showed an effect superior to the DAL800 treatment in the reduction of serum TC, the sirtuin 1 (Sirt1) activation, and the inhibition of 3-hydroxy-3-methyl glutaryl coenzyme A reductase (HMGCR) gene expression. In particular, unlike the DAL800 treatment, MAL treatment significantly led to the activation of peroxisome proliferator-activated receptor alpha (PPARα). Subsequently, PPARα meaningfully regulated downstream proteins associated with β-oxidation. Conclusion : These findings suggest that Arctium lappa L. roots with microwave processing effectively ameliorate obesity through the regulation of leptin and TC and the promotion of β-oxidation compared with Arctium lappa L. roots without microwave processing.

        Comparative Analysis on Blood Fatigue Variables after Isokinetic and Isotonic Exercise Training in Elite Athletes

        PURPOSE: This study examined the changes in the blood fatigue variables caused by isokinetic and isotonic exercise training. METHODS: Ten healthy adult males with at least one year of athletic experience participated. The participants performed the isokinetic circuit exercise program first, followed by an isotonic circuit exercise program. A two-hour break was allowed between the isokinetic circuit exercise program and the isotonic circuit exercise program. The circuit exercise program consisted of four items (Squat, Deadlift, Shoulder press, and Bench press). The blood samples were analyzed for the LDH, CPK, and Cortisol levels. RESULTS: The LDH level in the isokinetic group was significantly different from the isotonic group. In particular, the change in LDH level in the isokinetic group was 33.30% lower than that of the isotonic group. The serum CPK level of the isokinetic group showed a 10.03% lower decrease than the isotonic group, but the difference was not significant. The Cortisol level was relatively unchanged in the isotonic group, but it decreased in the isokinetic group. On the other hand, the Cortisol level did not show a significant difference between the two groups. CONCLUSION: The isokinetic group showed alleviation of the three indices, unlike the isotonic group. Further studies associated with the changes in blood fatigue variables through various exercise programs and exercise intensity will be needed.

        본 논문에서는 WCDMA 통신용 송신기에 적용 가능한 12비트 1GS/s 전류구동 방식의 혼합형 DAC를 설계하였다. 제안된 DAC는 혼합형 구조로써 하위 4비트는 이진 가중치 구조, 중간비트와 상위비트는 4비트 온도계 디코더 구조로 12비트를 구성하였다. 제안된 DAC는 혼합형 구조에서 발생되는 지연시간에 따른 성능 저하를 개선하기 위해 지연시간보정 회로를 사용하였다. 지연시간보정 회로는 위상주파수 검출기, 전하펌프, 제어회로로 구성되어 이진 가중치 구조와 온도계 디코더 구조에서 발생하는 지연시간을 감소시킨다. 제안한 DAC는 CMOS 0.18㎛ 1-poly 6-metal n-well 공정을 사용하여 제작되었고 측정된 INL/DNL은 ±0.93LS/ 0.62LSB 이하로 나타났다. 입력 주파수 1㎑에서 SFDR은 약 60㏈로 측정되었고, SNDR은 51㏈로 측정되었다. 단일 DAC의 전력소모는 46.2mW로 나타났다. This paper describes a 12 bit 1GS/s current mode segmented DAC for WCDMA communication. The proposed circuit in this paper employes segmented structure which consists of 4bit binary weighted structure in the LSB, and 4bit thermometer decoder structure in the mSB and MSB. The proposed DAC uses delay time compensation circuits in order to suppress performance decline by delay time in segmented structure. The delay time compensation circuit comprises of phase frequency detector, charge pump, and control circuits, so that suppress delay time by binary weighted structure and thermometer decoder structure. The proposed DAC uses CMOS 0.18㎛ 1-poly 6-metal n-well process, and measured INL/DNL are below ±0.93LSB/±0.62LSB. SFDR is approximately 60dB and SNDR is 51dB at 1㎑ input frequency. Single DAC's power consumption is 46.2mW.
