As the population ages, senile dementia of Alzheimer's type is increasing and becoming as a major public health problem called the disease of the century. Its exact etiology is uncertiain and therapeuric regimen has been unsatisfactory so far. Several lines of evidence suggest that estrogen deficiency of postmenopausal state contributes to neurodegenerative changes associated with Alzheimers disease. Recent increasing number of epidemiologic studies have examined and proved that postmenopausal hormone replacement therapy can prevent development of Alzheimer's disease and can be used for the treatment of the postmenopausal women with established Alzheimers disease. The action mechanism of estrogens are suggested to be inhibition of B-amyloid production, supression of apolipoprotein E, reduction of interleukin-6, antidepressive effect, increased cerebral blood flow and better cognitive functions and so on. Cleary, more prospective randomized research trials are needed to confirm these promissing clinical findings.
