동맥경유방사선색전술(TARE) 시 발생하는 Yttrium-90 방사성 폐기물 차폐용기 제작 및 유용성 평가

목적: 본 연구는 Yttrium-90 방사성 폐기물 차폐용기의 개발과 그 성능 평가에 대한 결과를 제시하며, 동맥경유방사선 색전술(TARE)의 시술 중 안전성을 향상시키고 방사성 폐기물 관 리의 중요성을 강조하고자 한다. 대상 및 방법: 방사성 폐기물을 수납하는 내부 공간의 크기를 의료폐기물 전용용기의 크기에 맞게 설계 및 제작하였다. 본 연구에서는 방사성 폐기물 표면과 방사성 폐기물을 수납한 차폐 용기 표면에서 각각 10cm, 100cm, 200cm 떨어진 위치에서 방사선량률을 측정 및 분석하였다. 결과: 차폐용기를 적용한 경우, 폐기물을 근거리에서 다루는 방사선 작업종사자를 고려하여 설정한 10cm 거리에서는 선량감소율이 97%로 나타났다. 또한, 시술실 내에서 시술의와 핵의 학과로 이동하는 과정에서 내원객을 고려하여 설정한 100cm와 200cm 거리에서는 77~97% 의 선량감소율을 확인할 수 있었다. 결론: 방사성 폐기물에 차폐용기를 적용하였을 때 방사선 작업종사자 및 시술의, 내원객이 받 는 피폭에 대한 선량감소 효과가 뛰어남을 확인할 수 있었다. 따라서, 본 연구는 TARE의 안전 성 향상을 위한 방사성 폐기물 관리 방안 지침에 기여할 수 있을 것으로 기대된다. PURPOSE: This study presents the results of the development and performance evaluation of Ytrium-90 radioactive waste shielding containers, and aims to improve safety during the procedure of Transarterial radioembolization (TARE) and emphasize the importance of radioactive waste management. In this study, the shielding container was designed and manufactured to fit the size of the container dedicated to medical waste. METHOD: The radiation dose rate was measured and analyzed after installing a survey meter at 10cm, 100cm, and 200cm away from the surface of the radioactive waste and the surface of the shielding container where the radioactive waste was stored. RESULTS: When the shielding container was applied, the dose reduction rate was more than 97% at the distance of 10cm set in consideration of the distance from the radiation worker. In addition, a dose reduction rate of 77% to 97% was confirmed at the distance of 100cm and 200cm set in consideration of the exposure of visitors during the process of moving to the medical staff and the nuclear medicine department in the procedure room. CONCLUSION: When the shielding container was applied, it was confirmed that the dose reduction effect on exposure to radiation workers and visitors was excellent. Therefore, this study is expected to contribute to the guidelines for radioactive waste management measures to improve the safety of TARE.

The purpose of this study is to develop the curriculum of power transmission and distribution fields in department of electricity by job analysis approach in order to bring up technicians with the adaptive power of industrial field in a rapidly changing industrial society. The method of this study was to carry out the research of questionnaire for teachers of department of electricity in order to analyze the demands of school education. The various actual data from this study were analyzed to make a basic plan, and then job approach was executed by the verification method through the interview and observation with engineers of Korea Electric Power Co. in Cheonnam province, and finally task training data which could be reflected to curriculum were extracted.The main results of this study were as follows: (1) Using the verification method about the power transmission and distribution field, job approach was executed, and task training data were extracted. (2) Analyzing quantitatively importance, frequency, and difficulty about thirteen tasks, the tasks able to reflect curriculum of the department of electricity were selected. (3) Trying to find out the problems of current curriculum, and suggested new curriculum to include the required ability for the students in future industrial society based on the questionnaire and field interview.
녹내장 여과 수술 후 발생한 중심장액성 맥락망막병증 1예

Purpose: To report a case of central serous chorioretinopathy development after glaucoma filtering surgery and spontaneous resolution in a patient with a history of central serous chorioretinopathy in the contralateral eye. Case summary: A 46-year-old male with a history of chronic uveitis in both eyes presented with uncontrolled intraocular pressure (IOP) in his left eye. Initial IOP was 34 mm Hg in his left eye. On preoperative evaluation, central serous chorioretinopathy, which was diagnosed in another clinic 1 month prior, was observed in his right eye. Slightly pale optic disc and retinal nerve fiber layer defects were noted in the left eye. However, macular abnormalities were not observed in the left eye. Trabeculectomy and peripheral iridectomy using mitomycin C were performed in the left eye. The patient was prescribed triamcinolone 8 mg daily for 4 days to reduce the post-surgical inflammation. On postoperative day 4, IOP in the left eye was 7 mm Hg and newly developed central serous chorioretinopathy was noted. On follow-up, IOP was maintained at 7-10 mm Hg and central serous chorioretinopathy disappeared 7 months postoperatively. Conclusions: When clinicians consider performing an ophthalmological procedure in a patient with a history of central serous chorioretinopathy in the contralateral eye, careful observation of central serous chorioretinopathy development is recommended.J Korean Ophthalmol Soc 2014;55(12):1895-1900
백내장을 동반한 녹내장 환자에서 백내장 수술이 시야 결손에 미치는 영향

Purpose: To determine the effect of cataract extraction on visual field damage in glaucoma patients with cataracts. Methods: Sixty-two eyes of fifty glaucoma patients (primary open-angle glaucoma, angle-closure glaucoma, and normal-tension glaucoma) were divided into three groups according to the MD (mean deviation) value from the Humphrey automatic visual field test. The PSD (pattern standard deviation) values were analyzed before and after the cataract surgery. Additionally, the visual acuity, intraocular pressure and the number of antiglaucomatous agents were compared. Results: All of the three groups, showed significant improvement in visual acuity. However, there was only a significant decrease in intraocular pressure at three months postoperatively. When comparing the mean MD values of each group, there was a statistically significant difference between the mild defect group and the severe defect group, but no statistically significant difference in the moderate defect group. There was also no statistically significant difference in PSD values in the any of the groups before and after cataract surgeries. Conclusions: In glaucoma patients with cataracts, there were no significant differences in PSD values before and after cataract surgery. Additionally, to perform an adequate cataract operation in advanced glaucoma patients without the progression of visual field damage, lowering intraocular pressure and improving visual acuity is helpful. 활락단(活絡丹)이 항염(抗炎), 진통(鎭痛) 및 항혈전(抗血栓)에미치는 영향(影響)에 대(對)한 연구(硏究)

This study was designed to elucidate the anti-inflammatory, cardiovascular, anti-thrombotic, and analgesic effect of Whalrakdan. The anti-inflammatory effects was measured by the method of carrageenin induced edema, protein leakage test using CMC-pouch, and the effect of Whalrakdan on the cardiovascular system was observed by the change of flow rate of Ringer solution in the vascular system in the ear of rabbit. and the contraction and dilatation of rat tail artery. Death rate, platelet aggregation, plasma coagulation activity, antithrombin activity was observed for the measurement of the anti-thrombotic effect of Whalrakdan, and the analgesic effect was measured by the acetic acid method and hot plate method. The result was as follows: 1. After 2 or 3hour of Whalrakdan administration, carrageenin induced edema and CMC-pouch protein leakage was significantly decreased. 2. The slight anagesic effect of Whalrakdan extract was confirmed by the observation of writhing syndrome, paw licking time, and escape time. 3. The droplet of Ringer solution increased according to the increase of concentration of Whalrakdan extract, and the vasoconstriction decreased dependantly to the concentration of Whalrakdan extract. 4. The anti-thrombotic effect of Whalrakdan was observed by the decrease of death rate, the inhibition of platelet aggregation, and the increase of anti-thrombin activity.
