우리나라 100세 이상 고령자의 인지기능장애 수준과 관련요인

본 연구는 우리나라 100세 이상 고령자의 인지기능장애 수준을 알아보고 그의 관련요인을 규명해 보고자 시도하였다. 연구대상은 2016년부터 2017년까지의 기간 동안에 국민건강보험공단에서 시행하는 건강검진 및 장기요양 인정조사를 받은 주민등록상의 100세 이상 고령자 268명(남성 132명, 여성 136명)으로 하였다. 연구결과, 조사대상 100세 이상 고령자의 인지기능장애수준은 남성보다 여성에서(p=0.000), 혼자 산다는 군보다 배우자나 자녀와 함께 산다는 군에서(p=0.019), 가정의 경제 상태가 낮을수록(p=0.001), 비만도가 낮을수록(p=0.000), 비흡연군보다 흡연군에서(p=0.000), 비음주군보다 음주군에서(p=0.013), 규칙적인 운동을 한다는 군보다 하지 않는다는 군에서(p=0.020)에서 유의하게 낮았다. 조사대상 100세 이상 고령자의 확정적 인지기능장애가 나타날 위험비는 남성보다 여성(ORs=9.85, 95% CI=2.88~33.59)에서 유의하게 증가하였고, 저체중군보다 비만군(ORs=0.25, 95% CI=0.07~0.86)에서, 흡연군보다 비흡연군(ORs=0.30, 95% CI=0.10~0.83)에서 유의하게 감소하였다. 조사대상 100세 이상 고령자의 인지기능장애에 영향을 미치는 요인으로는 성, 거주상태, 경제상태, BMI, 흡연상태가 유의한 변수로 선정되었다. 이상과 같은 연구결과를 보면, 조사대상 100세 이상 고령자의 인지기능장애 수준은 여러 인구사회학적 및 건강관련행위특성 변수에 따라 유의한 차이를 보이고 있었다. 특히 흡연, 음주 및 규칙적인 운동 등과 같은 건강관련행위특성이 좋지 않은 군에서 인지기능 수준이 저하됨을 시사하고 있다. This study was conducted to determine the levels of cognitive impairment (MMSE-K) among centenarians, and to reveal their association with related variables. The study subjects were 268 centenarians who received medical check-ups from the National Health Insurance Corporation during the period of 2016-2017. The levels of cognitive impairment were significantly reduced among females as well as those living with spouses and children, lower monthly income, lower BMI and individuals who engaged in smoking and alcohol consumption without regular exercise relative to their respective counterparts. The risk of definitive cognitive impairment in study subjects was significantly higher among women (ORs=9.85, 95% CI=2.88-33.59) than men and was lower in obese group (ORs=0.25, 95% CI=0.07-0.86) than in low weight groups, as well as in non-smoking (ORs=0.30, 95% CI=0.10-0.83) than smoking groups. The factors influencing the cognitive impairment of study subjects were sex, living status, monthly income, BMI and smoking status. Taken together, these results suggest that the levels of cognitive impairments among centenarians differed significantly in response to socio-demographic characteristics and health related variables. Especially, the levels of cognitive impairments decreased among those with poor health related variables such as smoking, alcohol consumption and regular exercise.

최근의 사회는 매우 빠르게 변화하고 있으며 이로 인해 비장애인뿐만 아니라 장애인의 삶을 더욱 어렵게 하고 있다. 장애영역 확대로 인한 장애인구의 증가와 가족구조의 변화 등에 따라 장애인 복지 수요가 지속적으로 증가하고 있으며, 장애개념에 대한 변화와 자립생활에 대한 확산 그리고 무엇보다 장애인 인권의식 신장 등 장애인복지를 둘러싼 환경과 패러다임이 급속히 변화하고 있다. 노동권은 인간의 삶에 있어 생계는 물론이고 사회참여와 자아실현이라는 삶의 가치부여에 있어 아주 중요한 부분이며 장애인복지 패러다임인 자립생활을 가능하게 하는 핵심이기도 하다. 이에 본 연구는 청주시 중증장애인의 근로욕구 및 실태를 조사하여 향후 중증장애인의 욕구와 청주시특성에 맞는 일자리창출 및 중증장애인 고용 기회 확대 방안을 제시하고자 한다. The latest society is very quickly changing so that it is placed in difficult life of not only the non-disabled people but also disabled people. The demand on welfare of the disabled people is continually increasing by the increasing in population of the disabled people by the expansion of the disabled realm and changing of family structure. The changing about the concept of the disabled, the spreading about the independence life, expecially the expanding about the awareness of the disabled human right, and the circumstance and the paradigm of the surrounded by the disabled welfare is rapidly changing. The right to work is related to the living, and is the important part of value endowment in the social participation and the self-realization, and is the core of the independence life. Therefore, this study is researched into the desire for work of the severely-disabled people in Cheong-ju City and their actual condition, so I present the desire of severely-disabled people and the suitable creation of the employment in Cheong-ju City, and the plan of the expansion of the disabled employment opportunity in my study.
