Rapidly progressive Glomerular nephritis(GN) has features of a number of different diseases and this heterogeneity of it has confounded many clinical classification and trials of therapy. Especially, the discovery of antiglomerular basemembrane ( Anti-BBM) antibody as demonstrated pathogenicity and antineutrophilic cytoplasmic antibody(ANCA) associated with vasculitis that allowed development of a rational form of treatment. Subgroup is being recognized to have both ANCA and anti-GBM antibodies-so called double-positive group. This subgroup has different manifestation of clinical disease, which occurs with pure anti-GBM disease or ANCA positive group. These considerations are important in making a therapeutic decision. We report a first case of double positive patient in Korea with review of literatures.

Acute gastric dilatation is sudden and excessive distention of the stomach by fluid and gas, often accompanied by vomiting, dehydration, and peripheral vascular collapse. Most cases of acute gastric dilatation have occurred as a postoperative complication and other predisposing causes which have been documented include chronic illness, acute infections, emotional stress, physical injuries, diabetes mellitus, progressive muscular atrophy, excessive food intake, and body cast attachment. Early recognition of this condition is important, since the mortality is high if treatment is delayed. Decompression by continuos suction, together with careful restoration and maintenance of water and electrolyte balance, constitutes the essence of the therapy. We report a case of acute gastric dilatation developed after trauma with review of literatures.

대상포진은 수두대상포진 바이러스의 재활성화 감염에 의한 것으로 임상적으로 드문 병은 아니다. 이것은 수포성 피부병변과, 통증 등을 특징으로 하며 비교적 예후는 좋은 편이다. 신경학적 합병증으로는 뇌막염, 뇌염, 신경통, 단일신경병증, 다발신경병증, 지각해리, 운동부전마비 등을 포함한다. 대상포진성 척수염과 뇌염은 나이든 사람이나 면역기능이 떨어진 사람에서 더 흔하게 발생한다. 우리는 적은 용량의 methotrexate로 치료 받고 있던 류마티스양 관절염 환자에서 발생한 드문 경우의 수두대상포진 바이러스에 의한 척수염에 대해 보고하는 바이다. 류마티스양 관절염으로 고생하던 64세의 여자가 양측 하지마비의 평가와 치료를 위해 입원하였다. 환자는 T11-T12 피부절제도상에 수포성 발진과 양측 하지마비 소견을 보이고 있었다. 척수 자기공명영상과, 뇌척수액 검사를 포함한 임상 검사결과, 대상포진 바이러스에 의한 척수염으로 진단되었다.

Chylous mesenteric cyst in adult is extremely rare and do not hale any specific signs or symptoms to facilitate their clinical diagnosis. Most cysts are noticed as a palpable abdominal mass or by nonspecific abdominal symptoms such as pain, nausea and vomiting. In some cases, they are detected incidentally by routine abdominal examination, but in symptomatic patient, the possibility of complication must be considered. The diagnosis can be done by radiologic studies such as sonography and computerized tomography but confirmative diagnosis is done by surgical excision. We describm a case of large mesenteric cyst which was cured by surgical excision in a 31-year-old man.

RADS(Reactive Airways Dysfunction Syndrome) is a clinical pathologic entity characterized by exposure to a toxic or irritant chemical, negative history of obstructive symptoms prior to exposure, persistence of obstructive symptoms after exposure, objective evidence of obstructive airway disease and/or nonspecific bronchial hyperresponsiveness, and abnormal bronchial biopsy result. We report a case of typical RADS after a single exposure to an irritant gas. He had worked as a specialist in a factory of copying-press for 30 years with no previously respiratory symptoms. Immediately after a exposure to the gas, he had eye irritation, rhinorrhea, cough and shortness of breath. He had no personal and familial history of allergy and respiratory disease. Allergoc skin prick test revealed negative response. Total IgE level by PRIST was 49 IU/ ml and eosinophil count was 235/mm3. Methacholine bronchial provocation test revealed a positive response. Bronchial biopsy showed chronic bronchitis with mild mononuclear cell infiltration and neither eosinophilic infiltration nor thickening of basement membrane.
