Familial adenomatous polyposis (FAP) is a disease characterzed by the development of 100 to several thousand colorectal adenomas, of which one or more will degenerate into carcinoma unless prophylactic colectomy is performed. Small bowel obstruction in pregnancy is a rare critical condition with high morbidity and mortality. The diagnosis of small bowel obstruction in pregnancy is often delayed because the symptoms such as nausea, vomiting, and abdominal pain mimic typical pregnancy-associated complaints. So, a high level of suspicion especially in patients with a history of previous abdominal surgery is essential for early diagnosis. Once the diagnosis is made, surgery should be performed immediately. We present the first case of small bowel obstruction during pregnancy in Korea with a brief review of the literature.

Purpose: To evaluate the effect of repeated debulking surgery for high-grade pseudomyxoma peritonei (PMP) originating from the appendix. Methods: Between January 1998 and December 2014, fifty patients, who underwent debulking surgery for high-grade PMP originating from the appendix, were obtained from a prospectively collected database and retrospectively analyzed. Two groups according to the number of operations were divided and analyzed. Results: A total of 118 operations were performed. Thirty-one patients received more than two operations. The median interval between operations was 18.2 months (range, 2–170 months). Complications developed after 26 operations (22.0%), including ileus (n=10), intra-abdominal fluid collection (n=7), surgical site infection (n=5), and others. There were two mortalities within 30 days after operation. Between two groups of patients who received one operation only and patients who received more than two operations, transfusion, diversion operation, and postoperative complication rate showed statistically significant differences. Two groups of patients had no differences in overall survival rates. Conclusion: Our results indicate that the number of operations does not affect the survival rate of high-grade appendiceal PMP, in which repeated debulking surgery is vital to relieve symptoms of the tumor burden.

Purpose: The studies on the impact of obesity on survival for colorectal cancer (CRC) in Korea are still insufficient. The aim of this study is to evaluate body mass index (BMI) as a prognostic factor of CRC. Methods: From January 2006 to December 2008, a total of 1873 patients with CRC who underwent surgery at Seoul National University Hospital were included in this study. Study participants were divided into five BMI groups: underweight (<18.5 kg/m2), normal weight (18.5– 22.9 kg/m2), overweight (23–24.9 kg/m2), obese (25–29.9 kg/m2) and morbidly obese (≥30 kg/m2). Clinicopathological variables and survival data were reviewed retrospectively. Overall survival was analyzed using log-rank test and Cox regression analysis. Results: Overall survivals of underweight, normal weight, overweight, obese, and morbidly obese groups were significantly different (5-year overall survivals rates: 54.4%, 73.2%, 78.6%, 77.2%, and 53.8%, respectively; P<0.001). In multivariable analysis for overall survival, BMI was an independently significant risk factor. Normal weight, overweight, and obese groups had lower hazard ratios for overall survival than underweight group (hazard ratio [HR], 0.626; 95% confidence interval [CI], 0.462–0848, P=0.002; HR, 0.530, 95% CI, 0.377–0.744, P<0.001; HR, 0.511, 95% CI, 0.470–1.675, P<0.001, respectively). The hazard ratio of morbid obese group was not significantly different from that of underweight group (HR=0.887, 95% CI=0.470–1.675, P=0.712). Conclusion: BMI can be a prognostic factor for CRC in Korea. Underweight and morbidly obese were associated with poor overall survival. Large multicenter studies in the Korean population is needed for a definite conclusion.

Chronic colonic pseudo-obstruction is a rare disease that results in colorectal dilatation without any obstructing lesions. Colonic dilatation does not usually cause colonic wall thickening and colonic visceral myopathy with muscular hypertrophy has not been reported in Korea. A 31-year-old female patient was transferred for treatment of refractory constipation accompanied by megacolon. She had suffered from recurrent attacks of severe abdominal pain with the sensation of a mass in the left lower quadrant. An abdominal CT revealed a large luminal dilatation of the sigmoid colon where massive stool was impacted. There was no obstructing lesion or luminal dilatation of the rectum. To relieve her refractory symptoms, a total colectomy with an ileorectal anastomosis was performed. Pathologic examination of the sigmoid colon revealed that the muscle layers were dysplastic and hypertrophied, and the innervations into the muscle fibers were markedly decreased. She was discharged without any intra-abdominal symptoms. (Intest Res 2008;6:145-149)

Aganglionosis is a rare form of neuronal intestinal malformation, which includes hypoganglionosis, neuronal intestinal dysplasia A (NID type A), B (NID type B) or combined type. Classic aganglionosis, or Hirschsprung’s disease is a neurogenic form of neonatal bowel obstruction characterized by a congenital absence of ganglionic cells in the submucosal and myenteric plexus, and presents in childhood with symptoms of constipation, colonic obstruction or sepsis due to enterocolitis. However, aganglionosis can be diagnosed first in adult due to short segment Hirschsprung`s disease with mild symptom and maybe also due to acquired process. We experienced a case of adult type aganglionosis assumed to be acquired type without malignancy or other neurological involvement. (Korean J Gastroenterol 2001;37:470-474)

Encapsulating peritoneal sclerosis (EPS) is a rare but fatal complication of continuous ambulatory peritoneal dialysis (CAPD). There are some reports on the effect of immunosuppressant therapy including steroid, but the results have not always been promising. Recently, owing to the advance of surgical techniques, there are some reports of the EPS cases significantly improved after successful surgery. A 30-year old man developed EPS after 9 years of peritoneal dialysis, and switched to hemodialysis. In spite of repetitive conservative management and immunosuppressant therapy, there was no improvement. His body weight decreased from 50 kg to 40 kg (BMI 14.2 kg/m2) due to severe malnutrition, so we decided to perform surgery. Total intestinal enterolysis was done successfully without concomitant enterectomy, and his general condition improved dramatically. Four months after surgery, the serum albumin concentration increased from 3.1 g/dL to 4.3 g/dL, cholesterol from 92 mg/dL to 208 mg/dL, and hemoglobin from 9.2 g/dL to 12.5 g/dL. His body weight increased to 61 kg (BMI 21.6 kg/m2), and there was not any fluid collection or bowel obstruction seen on the abdominal CT scan. We experienced a case of EPS which was significantly improved after enterolysis. We report this case with review of the literature.
