Mycobacterium abscessus에 의한 피부 및 연조직 감염 1예

A 36-year-old female who initially presented with a small erythematous and swollen abscess on her left anterior tibial area was found out to have a cutaneous Mycobacterium abscessus infection. She was first treated with incision and drainage, dressing, and antibiotics. The lesion began to be aggravated and dispersed. Neither aerobic nor anaerobic bacteria was grown on blood agar plate. After a few weeks, Mycobacterium grew on Ogawa media after 6 days, and was identified as M. abscessus by PCR-restriction fragment length polymorphism. She was then treated with clarithromycin, levofloxacin, and amikacin, and the skin lesion was resolved without further recurrence.
Pseudomonas aeruginosa 와 Acinetobacter baumannii 에서의 Metallo-β-lactamase의 유병률

Background: Metallo-β-lactamases (MBLs) have been reported in gram negative bacilli and are becoming increasingly important clinically because the enzymes hydrolyse almost all β-lactams, including carbapenems. Thus, the present study was conducted to determine the prevalence of MBL types in imipenem-nonsusceptible Pseudomonas aeruginosa and Acinetobacter baumannii isolated from a tertiary teaching hospital. Methods: Imipenem-nonsusceptible strains, 128 P. aeruginosa and 93 A. baumannii, were collected from clinical specimens. Identification and susceptibility tests were determined by Vitek GNI and GNS cards. MBL production was determined by modified Hodge test and imipenem-EDTA synergy test. Multiplex PCR amplification of MBL genes including blaIMP-1, blaVIM-1 and blaVIM-2were performed. Results: Thirty-one P. aeruginosa (24.2%) isolates and 3 A. baumannii (3.2%) were found to be MBL producers. In P. aeruginosa, 20 (15.6%) and 11 (8.6%)isolates were positive for blaIMP-1 and blaVIM-2, respectively whereas 1 (1.0%) and 2 (2.2%) isolates in A. baumannii, respectively. Conclusion: IMP-1 is more prevalent MBL type than VIM-2 among imipenem-nonsusceptible P. aeruginosa unlike in other studies. Larger numbers of isolates and sequential studies are strongly recommended for the useful evaluation and monitoring of MBL production in the hospital setting to infection-control. 건강한 사람에 있어 Salmonella enteritica serovar Senftenberg에 의한 감염은 드물며 특히 골수염의 원인균으로도 드물게보고되고 있다. 저자들은 평소 건강하던 소아에서 발생한 Salmonella enteritica serovar Senftenberg에 의한 Brodie 농양을경험하였기에 방사선학적 그리고 미생물학적 소견과 함께 보고하는 바이다. [대한임상미생물학회지 2010;13:132-134]
