        Acute Aortic Dissection in Pregnancy with the Marfan Syndrome

        Acute aortic dissection (AAD) during pregnancy can be fatal to both the pregnant mother and the baby, particularly in patients with the Marfan syndrome. We report a case of the modified Bentall procedure in surgery for AAD in a 31-year-old pregnant woman at 24 weeks of gestation with the Marfan syndrome. The patient recovered well after the operation, but unfortunately, the fetus could not be saved.

        A Rare Case of an Endobronchial Lipomatous Hamartoma Growing into the Peribronchial Pulmonary Parenchyme

        A 40-year-old man was admitted with dry cough of two months’ duration. Radiologic examination revealed an endobronchial mass obstructing right middle lobar bronchus and poststenotic pneumonia. We performed a right middle lobectomy because of suspected malignancy and difficulty for bronchoscopic resection, despite of failure in bronchoscopic diagnosis. The histopathological diagnosis was a lipomatous hamartoma which was exophytic and endobronchial. We report a rare surgical case of endobronchial lipomatous hamartoma which had occlusive and exophytic growth across the bronchial wall.

        Purification, Characterization, and Cloning of Trimethylamine Dehydrogenase from Methylophaga sp. Strain SK1

        Trimethylamine dehydrogenase (TMADH, EC 1.5.99.7), an iron-sulfur flavoprotein that catalyzes the oxidative demethylation of trimethylamine to form dimethylamine and formaldehyde, was purified from Methylophaga sp. strain SK1. The active TMADH was purified 12.3-fold through three purification steps. The optimal pH and temperature for enzyme activity was determined to be 8.5 and 55oC, respectively. The Vmax and Km values were 7.9 nmol/min/mg protein and 1.5 mM. A genomic DNA of 2,983 bp from Methylophaga sp. strain SK1 was cloned, and DNA sequencing revealed the open reading frame (ORF) of the gene coding for TMADH. The ORF contained 728 amino acids with extensive identity (82%) to that of Methylophilus methylotrophus W3A1.

        내엽성 폐분리증에서 발생한 결핵 감염 −1예 보고−

        Pulmonary sequestration is a relatively rare malformation. Infection with common pyogenes is a frequent complication in this disease. We report here on a case of intralobar sequestration that was infected with Mycobacterium tuberculosis in the absence of any other site of tuberculous infection. A 40-year man presented with a small amount of hemoptysis, and the man had been previously diagnosed with bronchiectasis 3 years ago. Chest computerized tomography revealed bronchiectasis with pneumonia in the left lower lobe and there was a large feeding artery from the thoracic aorta. A lobectomy of the left lower lobe was conducted via thoracotomy and the final pathologic examination confirmed pulmonary tuberculosis limited to the intralobar sequestrated lung. The patient underwent antituberculous chemotherapy from the postoperative 7th day and he was discharged without any adverse event. 폐 분리증은 비교적 드문 기형적 폐 질환이다. 이 질환에서 일반적인 화농성균의 폐 감염은 흔한 합병증일 수 있다. 저자들은 내엽성 폐분리증의 폐에 국한된 결핵 균의 감염 1예를 치험하고 보고한다. 내원 3년 전에 기관지 확장증으로 진단받은 40세의 남자 환자가 반복되는 소량의 객혈을 주소로 내원하였다. 흉부 CT 촬영상 좌하엽의 염증을 동반한 기관지 확장증 소견과 흉부 대동맥에서 기시되는 커다란 영양동맥이 발견되었다. 환자는 개흉술을 통한 좌하엽 절제술을 받았고 최종 병리학적 검사상 폐분리증의 폐에 국한된 결핵감염이 확인되었다. 수술 후 7일째부터 환자에게 항 결핵요법을 시작하였고 퇴원 후 외래에서 추적 관찰 중이다.

        Purification and Characterization of a MethanolDehydrogenase Derived from Methylomicrobium sp. HG-1 Cultivated Using a Compulsory Circulation Diffusion System

        Methanotrophs are microorganisms that possess the unique ability to utilize methane as their sole source of carbon and energy. A novel culture system, known as the compulsory circulation diffusion system, was developed for rapid growth of methanotrophic bacteria. Methanol dehydrogenase (MDH, EC 1.1.99.8) from Methylomicrobium sp. HG-1, which belongs to the type I group of methanotrophic bacteria, can catalyze the oxidation of methanol directly into formaldehyde. This enzyme was purified 8-fold to electrophoretic homogeneity by means of a 4 step procedure and was found in the soluble fraction. The relative molecular weight of the native enzyme was estimated by gel filtration to be 120 kDa. The enzyme consisted of two identical dimers which, in turn, consisted of large and small subunits in an a2b2 confor-mation. The isoelectric point was 5.4. The enzymatic activity of purified MDH was optimum at pH 9.0 and 60oC, and remained stable at that temperature for 20 min. MDH was able to oxidize primary alcohols from methanol to octanol and formaldehyde.

        양측 유미흉을 동반한 림프관평활근종증 -1예 보고-

        림프관평활근종증은 드문 질환이며, 임상경과상 악성으로 분류할 수 있다. 복통을 주소로 입원한 21세 여자 환자가 골반 내시경을 통해 난소 황체 출혈을 지혈하였고, 그 후 유미성 복수로 개복하여 후복막에 위치한 낭성 종양을 절제하여 림프관평활근종증으로 조직 진단 받았다. 홀몬 치료 중 유미흉이 발생하여 흉부외과에서 개흉하여 림프액 누출부위를 봉합결찰하였고, 10% 포타딘 관주로 유미흉은 호전되었으나 유미성 복수가 재발하였으며 진단 6개월에 전신 쇠약과 호흡 부전으로 사망하였다. We had expierienced pulmonary lymphangioleiomyomatosis(LAM) with bilateral chylothorax and chylous ascites. A twenty-one-year-old lass with chief complaint of abdominal pain was admitted through the emergency room. She received emergent pelvicoscopic surgery for the rupture of the right corpus luteum. We aspirated 1000ml of the uncoagulated blood. The bleeding point was cautherized electrically. LAM was diagnosed with tissue from the retroperitoneum. Chylous ascites and bilateral chylothorax were occurred despite of various treatments. On thoracotomy, bullous changed lung and lymphatic leakage from visceral and parietal pleura were observed. She died of respiratory insufficiency and general weakness after 6 months from admission.

        심방중격결손을 통해 좌심방으로 유입되어 있는 혈전을 동반한 급성 폐동맥 혈전색전증의 치험 -1예 보고-

        Though acute pulmonary thromboembolism is usually managed medically with the use of thrombolytics or anticoagulants, an emergent life-saving surgery would be required. In a case of acute pulmonary thromboembolism with acute severe right heart failure and deferment of it could result in fatal outcomes in a short time. In addition, the mortality is raised considerably if it is combined with right heart thrombi. Despite paradoxical thromboembolism via patent foramen ovale was reported, few report might be presented, in which showed the thrombus in right atrium has traversed atrial septal defect into left atrium and left ventricle like this case as the evidence of paradoxical thromboembolism. We report a case of acute pulmonary thromboembolism with acute right heart failure arising from deep vein thrombosis, developed immediately after low anterior resection for colon cancer in a 63-year-old male, who was managed successfully by emergent thromboembolectomy with cardiopulmonary bypass. 급성 폐동맥 혈전색전증은 대부분 혈전용해제나 항응고제 등을 이용하여 내과적으로 치료하나, 폐동맥 혈전색전증에 의한 급성 우심실 부전이 발생하는 경우에는 신속히 수술을 하지 않으면, 단시간 내에 치명적인 결과를 가져올 수 있으며, 우심혈전이 동반되어 있는 경우 그 사망률은 훨씬 높아지고, 개방형 난원공을 통한 역리성 색전증(paradoxical embolism)이 보고되고 있다. 그러나 본 증례에서와 같이 우심방의 혈전이 심방중격결손을 통해 좌심방으로 유입되는 것이 관찰되는 예는 극히 드물다. 직장암으로 저위전방절제술을 받은 63세 남자 환자에서 수술 직후 심부정맥혈전증에 의해 생긴 우심실부전을 동반한 급성 폐동맥 혈전색전증을 응급개심술을 통해 좋은 결과를 얻었기에 보고하는 바이다.

      • UV-TiO₂광촉매 반응기를 이용한 미생물의 살균효과

        UV-TiO_(2) 반응기를 이용하여 Escherichia coli와 Aspergillus oryzae var. oryzae의 살균효과를 측정하였다. 254 ㎚에서 최대 14 watt의 자외선 방출량을 내는 램프를 원형 Pyrex유리관 중앙에 설치하였고 TiO_(2)는 석영관에 박막증착(Thin Film Coating)된 형태와 슬러리 형태로 나누어 회분식으로 살균정도를 측정하였다. E. coli에 대한 살균력은 1.7× 10^(7) cells/㎖에 대해 5분간 자외선 조사를 하였을 경우 2.0× 10^(2) cells/㎖으로 감소하였고, 자외선 조사와 함께 슬러리 형태의 TiO_(2)를 첨가하였을 경우에는 3.4× 10 cells/㎖으로, 자외선 조사와 함께 TiO_(2)가 석영관에 박막증착된 경우에는 7.6× 10^(2) cells/㎖으로 감소하였다. 한편 위와 같은 조건에 유리관 하부에서 기포를 주입후 11분 동안 자외선을 조사시킨 경우에는 1.3× 10^(2) cells/㎖으로, 자외선 조사와 함께 슬러리 형태의 TiO_(2)를 첨가하였을 경우에는 1× 100 cells/㎖으로, 자외선 조사와 함께 TiO_(2)가 박막증착된 경우에는 7.9× 10 cells/㎖을 나타내었다. 결국 UV-TiO_(2) 반응기에 사용되는 TiO_(2)가 슬러리 형태일 때 최대 살균효과를 나타내었으나 기포가 첨가되었을때는 오히려 살균에 장애를 받는 것으로 나타난 반면 석영관에 박막증착된 경우에는 기포가 첨가되는 것이 살균에 효과적인 것으로 관찰되었다. The killing effect of UV-TiO_(2) photocatalytic system on the Escherichia coli DH5-慣 and Aspergillus oryzae var. oryzae (KCTC 6095) was studied. The UV lamp which emits maximum 14 watts at 254 nm was set on the center of pyrex round glass tube. Two types of TiO_(2), one of which is slurry and another which is thin film coated form, were used to determine the killing effect. When UV was irradiated to 1.7 * 10^(5) cells/??of E. coli for 11 min, the living cell number decreased to 4.0 * 1.0^(0) cells/?? The effect of UV system together with slurried TiO_(2) was less than 1 cells/?? whereas that of UV-coated TiO_(2) system decreased to 7.1 * 10^(3) cells/?? To study the effect of bubble on the killing of microorganisms, air was bubbled to the bottom of glass tube. When 1.7 * 10^(5) cells/??were exposed to UV for 11 min in combination with air bubble, the living cell number decreased to 1.3 x 10^(2) cells/?? In the same condition except the addition of slurried TiO_(2), the living cells were 1 * 10^(2) cells/?? However, more cells could be killed by the system which consists of UV, coated TiO_(2), and air bubble (7.9 * 10^(1) cells/??. From these results, it was found that UV-slurried system is the most effective one, but its killing effect is not stimulated by air bubble. However, bubbling was very effective in the UV-coated TiO_(2) system.

        개폐식 대공간 구조물의 파라메트릭 설계와 풍하중 적용

        The purpose of this study is to model and analyze retractable large spatial structures by applying parametric modeling techniques. The modeling of wind loads in the analysis of typical structures including curved surfaces can be error-prone, and the processing time increases dramatically when there are many types of variables. However, the method based on StrAuto that was developed in previous research, facilitates the efficacious assignment of wind loads to structures and the rapid arrival of conclusions. As a result, it is possible to compare alternatives with various loads, including wind loads, to determine an optimal alternative much faster than the existing process. Further, it is almost impossible to directly input the wind load by calculating the area of an irregularly curved surface. However, the proposed method automatically assigns the wind load, which allows for automatic optimization in a structural analysis system. The approach was applied and optimized using several models, and the results are presented. 본 연구의 목적은 개폐식 대공간 구조물의 풍하중 산정 및 구조해석의 과정을 자동으로 수행하는 컴포넌트를 개발하는 것이다. 설계한 파라메트릭 모델링을 StrAuto를 통해 구조해석 자동화단계를 거쳐 구조해석용 모델로 변환하는 과정을 실시간으로 연동하여 구조해석 결과를 자동으로 도출하는 과정으로부터 본 연구에서는 추가로 구조물의 풍하중을 형상에 따라 상세히 할당하는 기능을 개발하였다. 이와 같은 과정을 통해 풍하중에 대한 최적화를 수행하여, 기존 설계된 구조의 물량을 줄이고, 구조적 안정성은 유지하는 방향으로 결론을 도출하였다. 추후에는 본 예제 모델을 통해 진동제어 최적화를 위한 제진장치 설치위치의 자동탐색이 가능하게 되는 연구를 진행할 계획이다.
