        Five-Year Community Management Rate for Dementia Patients: A Proposed Indicator for Dementia Policies

        Background and Purpose There is no specific indicator for monitoring dementia management. We propose an auxiliary indicator called the community management rate, defined as the proportion of dementia patients who receive informal care from close caregivers or themselves within their community population. The 5-year community management rate is the percentage of dementia patients who are receiving community management at 5 years after they were diagnosed. The aim of this study was to identify how the community management rate has changed over time and how the 5-year community management rate differs according to age, sex, income, residence area, and comorbidities. Methods We analyzed customized research database of the Korean National Health Insurance Services from 2003 to 2018. The 5-year community management rate was calculated annually with newly diagnosed dementia patients, and compared among subgroups according to age, sex, income, residence area, and comorbidities. Results This study analyzed 549,297 patients. Among those newly diagnosed with dementia in 2003, the mean duration of community management during the 15-year follow-up was 5.98 years. The community management rate decreased rapidly from 2003 to 2006, after which it increased. A low 5-year community management rate was associated with older age, higher comorbidity burden, nonmetropolitan residence, and low income. Conclusions The community management rate seems to reflect diverse patient factors. Efforts are needed to reduce the comorbidity burden and differences in the 5-year community management rate according to residence area and income. This study indicates the need for further investigations into the use of this indicator to monitor the management of dementia patients.

        “영감” 개념에 따른 플라톤의 시 이해: 『국가』의 시 논의와 일치하는 그것의 비판적 입장에 관하여

        Die Dichtungsauffassung, welche Platon unter dem Begriff des “Enthousiasmos” vertritt, ist bisher nicht selten als ein Lob der Dichtung interpretiert worden. Aus einer solchen Interpretation aber ergibt sich die Schwierigkeit, wie Platons ganze Dichtungsauffassung als innerlich konsequent verstanden werden könnte. Denn es ist eine unumstrittene Tatsache, dass seine repräsentative Dichtungsdiskussion, die unter dem Begriff der “Mimēsis” in der Politeia stattfindet, sich mit der Dichtung in kritischer Hinsicht beschäftigt. Da diese Dichtungsdiskussion sogar die Dichtung verbannt, gibt es keinen Weg, die Schwierigkeit zu lösen. Eigentlich ist aber die Schwierigkeit nicht aus Platons Dichtungsdiskussion selbst entstanden, sondern aus derjenigen Interpretation, die in seiner unter dem Begriff des “Enthousiasmos” stattfindenden Dichtungsdiskussion ein Lob der Dichtung sehen will. Dies ist nun dadurch festzustellen, all die zentralen Texte sorgfältig zu überprüfen, in denen diese Dichtungsdiskussion stattfindet. Durch eine solche Überprüfung wird also die Tatsache sichtbar, dass Platon unter dem Begriff des “Enthousiasmos” die Dichtung im allgemeinen kritisch bewertet, indem er ihr weder ‘Technē als Wissen’ noch ‘Erkenntnis der Ideen’ zuerkennt, dass er zugleich aber nur die zur Philosophie orientierte seltene und ideale Dichtung schätzt, indem er sie als eine solche Dichtung versteht, welche eine ‘wahre Meinung’ vorbringt. Diese ganze Tatsache stimmt im Grunde damit überein, was Platon in seiner unter dem Begriff der “Mimēsis” in der Politeia stattfindenden Dichtungsdiskussion als seine evaluative Ansicht bezüglich der Dichtung zeigt. Auf diese Weise also bewertet er unter beiden Beriffen die Dichtung im allgemeinen kritisch, schätzt aber nur die zur Philosophie orientierte seltene und ideale Dichtung. Dadurch erkennt er all den bis zu seiner Zeit vorliegenden Dichtungen keine erzieherische Rolle zu, schreibt aber der Philosophie die Rolle zu, alles im Leben Wichtige zu lehren. Dies ist Platons Weise, auf welche der alte bis zu seiner Zeit dauernde Wettstreit zwischen der Dichtung und der Philosophie zugunsten der letzteren gelöst wird. 플라톤이 “영감” 개념에 따라 제시하는 시 논의는 종래에 드물지 않게 시를 찬미하는 것으로 해석되곤 했다. 그러나 그것이 이렇게 해석될 경우 그의 시 논의 전체가 어떻게 일관적으로 이해될 수 있을지에 대한 난제가 생겨난다. 『국가』에서 “미메시스” 개념에 따라 이루어지는 그의 대표적인 시 논의가 시에 대한 부정적 평가에 기여한다는 것은 이론의 여지가 없는 사실이기 때문인데, 이 시 논의가 ‘시인 추방’을 선언하는 것인 한, 이 난제는 해결될 길이 없다. 그런데 이 난제는 본래 플라톤의 시 논의 자체에 의해 생겨나는 것이 아니라, “영감” 개념에 따른 그의 시 논의를 시에 대한 찬미로 보는 해석에 의해 생겨나는 것이다. 이는 이 시 논의가 이루어지는 곳으로서 본질적인 비중을 갖는 모든 텍스트가 면밀히 검토될 때 확인된다. 이러한 검토가 이루어질 때 드러나는 것은 플라톤이 “영감” 개념 아래에서 시 일반에 대해 ‘앎으로서의 기술’과 ‘이데아 인식’을 인정하지 않으면서 비판적 입장을 견지하고 있으며, 철학에 정향된 드물고 이상적인 시만을 ‘참된 의견’을 제시하는 것으로서 긍정적으로 평가하고 있다는 사실인데, 이것은 『국가』에서 “미메시스” 개념에 따라 이루어지는 시 논의와 전적으로 일치하는 점이다. 이와 같이 그는 두 개념 모두에 의해 시 일반을 비판하면서 철학에 정향된 드물고 이상적인 시를 긍정적으로 평가하는데, 이를 통해 당대까지의 모든 시에 대해 교육적 역할을 부정하면서 삶에서 중요한 모든 것을 가르치는 역할을 철학에 귀속시킨다. 이것은 당대까지 지속되던 ‘시와 철학 사이의 오랜 다툼’을 철학의 편에서 해결하는 그의 방식이다.

        통년성 알레르기비염 환자에서 비강내 광치료

        Background and Objectives:Phototherapy has a profound immuno- suppressive effect and is widely used for the treatment of immune- mediated skin diseases. The purpose of this study was to investigate the effectiveness of intranasal phototherapy in treating patients with perennial allergic rhinitis. Materials and Methods:We conducted a study on 19 patients with perennial allergic rhinitis from January 2008 to July 2008. Each intranasal cavity was illuminated three times in the first week and then once a week the following five weeks. Symptom scores and Rhinoconjunctivitis Quality of Life Questionnaire (RQLQ) were assessed. Results:Phototherapy was well tolerated by the patients and resulted in a significant improvement of clinical symptoms for rhinorrhea, nasal obstruction, nasal itching, sneezing and total nasal score. In addition, significant improvements were achieved for itching of eyes, itching of the palate and total non-nasal symptom score except tearing and redness of eyes. After six weeks, the overall RQLQ scores significantly improved by 51% from the baseline in the treatment. Conclusion:These results suggest that phototherapy is an effective modality for treating clinical symptoms of perennial allergic rhinitis and represents an alternative treatment for perennial allergic rhinitis.         Causes, Risk Factors, and Clinical Outcomes of Stroke in Korean Young Adults: Systemic Lupus Erythematosus is Associated with Unfavorable Outcomes

        Background and Purpose The incidence of ischemic stroke (IS) in young adults is increasing, and the associated large socioeconomic impact makes understanding IS in young adults important. We investigated the causes of and risk factors for IS in young adults, and their impact on outcomes. Methods The Stroke in Korean Young Adults (SKY) study is a standardized multicenter prospective study involving eight medical centers of the Republic of Korea. First-ever IS patients aged 18 years to 44 years were prospectively included in this study within 7 days of stroke onset. Their outcomes at 3 months were analyzed. Results This study enrolled 270 patients from April 2014 to December 2018, most (67.8%) of whom were male. About 41.5% of the patients had one or more vascular risk factors from among hypertension, diabetes mellitus, and dyslipidemia. However, only half of them had received regular treatment. Arterial dissection was more common in males, and systemic lupus erythematosus (SLE) and Moyamoya disease were more common in females. The outcome was favorable (modified Rankin Scale score of 0 or 1) in 81.9% of the patients at 3 months after stroke onset. More severe initial symptoms, higher initial glucose level, and SLE as a comorbidity were associated with unfavorable outcomes. Conclusions Young adult IS patients in Korea exhibit low awareness and poor management of their risk factors. Although the short-term outcome was relatively favorable in those patients, having SLE was associated with unfavorable outcomes. More attention needs to be paid for improving awareness and controlling risk factors in this population.

        아리스토텔레스의 ‘카타르시스’에 대한 재고

        The present paper is a reconsideration of Aristotle’s Catharsis. For this task it is above all needed to reconsider thoroughly the passage in the Politics about the musical Catharsis; for this passage has been often misunderstood. As the result of this reconsideration it is revealed that the view this passage represents about the Catharsis can be as a whole accepted for the interpretation of tragic Catharsis on the basis of the Poetics. In consequence the tragic Catharsis can be interpreted as follows: It is such a function of the tragedy that by arousing pity and fear intensely in the mind of the spectator through its plot understood by him, the tragedy makes him feel such emotions, without vagueness that they would have more or less in real life, in refined form. The Catharsis as the tragedy’s function that consists of this refined emotional experience is not in itself anything that forms moral virtue, but can contribute to this cultivation. On the other hand, it brings the spectator’s mind to pleasurable relief by its termination, because this emotional experience as intense also is, when terminated, subsiding. 본고는 아리스토텔레스의 카타르시스를 재고하는 작업이다. 이를 위해 종래에 빈번히 잘못 이해되어 온 음악의 카타르시스에 대한 『정치학』 구절이 면밀히 재고되어야 한다. 이 재고에 따르면, 이 구절이 카타르시스에 대해 대변하는 견해는 그것 전체가 『시학』에 따라 비극의 카타르시스에 대한 해석에 수용될 수 있다. 그 결과, 비극의 카타르시스는 다음과 같이 해석될 수 있다. 그것은 비극이 플롯에 의해 이를 이해하는 관객의 마음에 연민과 공포를 강렬하게 환기함으로써, 관객으로 하여금 이러한 감정들을, 현실에서라면 그것들이 다소간 가졌을 수 있는 혼연함 없이, 정련된 상태로 느끼게 하는 것이다. 카타르시스는 이러한 정련된 감정 경험으로 이루어지는 것으로서 그 자체가 덕성을 형성하는 것은 아니지만 이에 기여할 수 있다. 다른 한편으로 카타르시스는 그것의 종료에 의해, 이 감정 경험이 또한 강렬한 것으로서 종료되면 가라앉기에, 관객의 마음에 즐거운 경감을 가져다준다.

        『古事記』における「神による神のまつり」

        The myth of “Kojiki,” thus far, has been considered to be a festival myth. It is thought that myths develop and redevelop alongside rituals, and as these systematized, they became “Kojiki” or “nihonshoki.” However, religious services for “the deities of heaven and earth” that have been performed are continuous events to be repeated and carried out to prevent evil. In contrast, for this ritual, a contagious disease was prevalent at the age of the Emperor Sujin, and many people died of “Kojiki.” In other words, it became a festival for “peace and people to prosper around the world” with God. In a sense, where there was a crisis, this ritual was performed, and so it is not continuous but temporary. There is some part of the ritual that has been followed continuously because Amaterasu gave an order to “always worship this mirror like me” and applied this to “Kojiki.” However, the crisis for the country, which the Emperor rules over, is not accepted there. This means that the country that the Emperor rules over has been guaranteed limitlessly by Amaterasu.
