A 31-year-old woman was transferred because of dyspnea that occurred immediately after cesarean section. Initial echocar-diography showed akinesia in apex to mid left ventricular (LV) wall with severe LV systolic dysfunction, which was pre-sumed to be stress-induced cardiomyopathy. Because the patient developed cardiogenic shock, we initially planned on inser-tion of an intra-aortic balloon pump. However, due to aggravation of cardiogenic shock and pulmonary edema, we had to resort to extracorporeal membrane oxygenation (ECMO) for cardiopulmonary support. In addition, an LV pigtail catheter was applied for decompressing the LV. Finally, she was successfully weaned from ECMO after recovery of LV function.

Objectives: This study was conducted to find out if the dietary iron intake of Koreans estimated by 2 different methods (iron content sources) using the food intake data from the Korea National Health and Nutrition Examination Survey (KNHANES) are comparable. One method was based on the KNHANES's Food & Nutrient Database (FND) derived mainly from the Korean Food Composition Table and the other used the iron content (IC) of food samples processed in the Korean Total Diet Study (KTDS). Methods: Dietary intake data from the 2013-2016 KNHANES was used to select representative foods (RFs) in KTDS for iron analysis. Selection of the RFs and cooking methods for each RF (RF × cooking method pair) was performed according to the ‘Guidebook for Korean Total Diet Studies’ and resulted in a total of 132 RFs and 224 ‘RF × cooking method’ pairs. RFs were collected in 9 metropolitan cities nationwide once or twice (for those with seasonality) in 2018 and made into 6 composites each, based on the origin and season prior to cooking. Then, the RF composites prepared to a ‘table ready’ state for KTDS were analyzed by Inductively Coupled Plasma-Mass Spectrometry (ICP-MS). Dietary iron intake of the Korean population was estimated using only RFs’ intake data based on the 2 sources of iron content, namely FND-KNHANES and IC-KTDS. Results: RFs in KTDS covered 92.0% of total food intake of Koreans in the 2016-2018 KNHANES. Mean iron intake of Koreans was 7.77 mg/person/day by IC-KTDS vs 9.73 mg/person/day by FND-KNHANES. The major food groups contributing to iron intake were meats (21.7%), vegetables (20.5%), and grains & cereals (13.4%) as per IC-KTDS. On the other hand, the latter source (FND-KNHANES) resulted in a very different profile: grains & cereals (31.1%), vegetables (16.8%), and meats (15.3%). While the top iron source was beef, accounting for 8.6% in the former, it was polished rice (19.2%) in the latter. There was a 10-fold difference in the iron content of polished rice between 2 sources that iron intakes excluding the contribution by polished rice resulted in very similar values: 7.58 mg/person/day by IC-KTDS and 7.86 mg/person/day by FND- KNHANES. Conclusions: This study revealed that the dietary iron intake estimated by 2 different methods were quite comparable, excluding one RF, namely polished rice. KTDS was thus proven to be a useful tool in estimating a ‘closer-to-real’ dietary intake of nutrients for Koreans and further research on various nutrients is warranted.

The success of cell-based therapies for treatment of liver disease hinges on the phenotypic stability of isolated hepatocytes; however, primary hepatocytes rapidly lose many liver-specific functions following enzymatic isolation from the liver. Previous studies have demonstrated that liver-specific functions can be stabilized by cocultivation with non-parenchymal cells. Although the precise mechanisms underlying these effects have not yet been elucidated, it is believed that gap junctional intercellular communication plays an important role in cell-cell communication. In this study, indirect immunofluorescence was used to identify connexin 32 and 26 as homotypic gap junction proteins that were present between hepatocytes during co-culture with 3T3 fibroblasts, but not in pure hepatocyte cultures. Additionally, dye transfer of Lucifer yellow revealed that these gap junctions were functionally coupled. Although previous studies reported abundant connexin 43 expression in 3T3 fibroblasts and functional coupling by dye transfer, we did not observe heterotypic gap junction (connexin 43) expression or its functional coupling by either immunofluorescence or dye transfer. Incubation with a gap junction inhibitor, 18β-glycyrrhetinic acid, in hepatocyte co-culture diminished the level of liverspecific markers such as albumin production and P450 activity relative to control co-cultures. Overall, these findings suggest that homotypic gap junctions are present in hepatocytes co-cultured with fibroblasts and might play a functional role in homotypic hepatocyte communication. In conclusion, homotypic gap junctional communication may be important to phenotypic stability of isolated hepatocytes in co-culture and crucial to the design of tissue-engineered therapies for treatment of liver failure.
Porcine Ryanodine Receptor 유전자의 Mutation 을 확인하기 위한 Double Tube Allele Specific Pimer PCR 의 이용

A double tube allele specific PCR assay was developed for a rapid and accurate detection of the porcine Ryanodine receptor genotypes using the allele specific pimer PCR. The assay, a modification of Zinovieva` single tube allele specific(STAS) PCR, reduced significantly primer dieter artificial band and determined allelic variations in simple, accurate and cost-effective ways. PCR primers were designed as followed ; two allele-specific `internal` primers[Int 1 for the normal allele N 5`-AGTAATGAGATCTTGGTTGGA(G→T)CG-3`, Int 2 for the mutant allele n : 5`-GTGCAATGGTGTGGCCG (T→C)GT-3`] were recognized with their 3`nucleotide the allelic variation. In order to increase the allele specific annealing an additional mismatch at position 3 from the 3` end of each primer was introduced. The common `external` primers(Ext 1, Ext 2) were located at different distances from the point mutation(nt 1843, C→T), so that different fragment lengths were generated for each allele. In the case of the gene, in normal(NN) and mutant (nn) homozygotes, only one fragment of 533 by or 823 bp, respectively, was amplified, whereas in heterozygotes (Nn) both fragments were detected. Independently from the genotype, an additional product(1312 bp) of the common `external` primers was amplified in. the single tube method. The use of the allele specific PCR for detecting allelic nucleotide exchanges offers some obvious advantages the presence of restriction endonuclease sites is not required and the allelic variations are visualized by simple agarose gel electrophoresis. As compared with the PCR-RFLP this modified method is simple, reliable and time and cost effective and thus can be applied in large scale to establish the program for testing and removal of the genes associated with the breeding of swine.
