N/A Although bone scan is a highly sensitive test for detecting bone metastasis, its findings are often limited in specificity and cannot be used for assessing the bone marrow. Bone marrow scintigraphy may provide useful information but previous experience with radiolabelled colloid has been disappointing. Recently, 99mTc labeled anti-granulocyte rnonoclonal antibody (anti-NCA-95 MAb) has been introdueed as a new bone marrow imaging agent. To evaluate the usefulness of 99mTc anti-NCA MAb bone marrow scans for detecting skeietal metastasis, bone marrow scans of 44 malignant tumor patients were evaluated and compared with bone scan fmdings. Bone scan showed abnormal lesions in 26(59%) cases, and 18 of these patients also had an abnormal bone marrow scan. Seven of the 8 patients who had normal bone marrow scan despite bone scan lesions were confirmed to be free from metastasis. There was one case with a marrow defect despite normal bone scan but the presence of metastasis was not determined due to loss of follow up. Bone scan demonstrated a total of 64 lesions while bone marrow scan showed 38 lesions. Fifty percent (32/64) of the bone scan lesions had matching marrow defects while the rernaining 50% did not. Most of these non matched lesions were suggested to be nonspecific 1esions such as rib fractures or degenerative change. Meanwhile bone marrow scan was able to detect 6 new lesions not detected by bone scan, but metastasis in each lesion was not confirmed. Bone marrow scan was also helpful in assessing equivocal bone scan lesions to be of metastatic nature in 10 patients by demonstrating a matched marrow defect. Thus 99mTc anti-NCA MAb bone marrow scan can help exclude metastasis in patients with nonspecific bone scan lesions and may be able to detect metastatic lesions not seen with bone scan. It appears useful as a complementary study to bone scan in evaluating malignant tumor patients.

The newly developed diagnostic method with application of $^{113}Sn-^{113m}In$ cow system ($^{113}Sn:\;T\frac{1}{2}$ 118 days, $^{113m}In:\;T\frac{1}{2}$ 1.7 hrs, 390 Kev, Single ${\gamma}$) has the remarkable advantages such as increased diagnostic ability by single large dose administration of $^{113m}In$ with no subsequent radiation hazard and shortened examining time. We reformed the research of following scope with the use of developed $^{113}Sn-^{113m}In$ cow (25 mCi) generator: The sizes of particles produced under various conditions were investigated, and possibility for application to the scannings of various organs such as brain, liver, lung, bone marrow and blood pool etc. were studied. Results: $^{113m}InCl_3$ solution eluted from diluted HCl solution (pH 1.5) passed through $^{113}Sn-^{113m}In$ generator, and there can be produced various sized particles of colloidal indium. And there observed the state of distribution of $^{113m}In$ in each organ which showed many differences according to the particle sizes of colloidal indium. The results are stated as follows: 1. The adjustment of pH is the most important factor in making the desirable particle size of colloidal indium. The colloid for blood pool showed the highest level as 7.1%/gm blood, at pH 1.7, the colloid of pH 3.5 for liver scanning showed the highest level, 88.4%, in the liver, the colloid pH 6 showed the highest level, 3.1%, in the spleen, and the colloid of pH 11.0 showed the highest level, 85.3%/gm, in the lung. 2. The colloid for liver scanning made with NaCl-NaOH system showed the highest liver uptake at pH 7.2, and at either higher or lower pH than 7.2 showed decrease of liver uptake more or less. 3. The activity of $^{113m}In$ eluted through $^{113}Sn-^{113m}In$ generator indicated over 90% in the initial 4 ml, and particularly 88.1%-86.0% in the initial 2 ml. 4. The incubation time, tempertaure and mechanical irritation related to colloid formation and coating of colloid were not the definite condition of influence.

N/A Dipyridamole is an agent that may be used to noninvasively evaluate coronary artery disease. The effect of dipyridamole infusion its generally related to its induced peripheral vasodilatory effect. In normal person, heart rate is generally increased slightly while blood pressure decrease, but the achieved double product and related myocardial oxygen consumption have no significant change. The purpose of this study is to examine the effect and side effect of dipyridamole, and to compare different response to dipyridamole among the patients. We evaluated 847 patients who underwent dipyridamole stress myocardial SPECT. 93.6% of them had induced hypotension 0.9% showed no change of blood pressure, 5.5% had increased blood pressure 8.3% had no change of pulse rate more than 10% of basal pulse rate. Among diabetes, 16.9% was not change of pulse rate, 6.7% in non-diabetes. There was no significant correlation between age and rate pressure product rest(RPPr), in patients without perfusion defects on SPECT(y=7.1x+48.4r=0.13 p>0.01). As increasing age, RPPs/RPPr was declined(y=-11.6x+68.9 r=0.17 p〈0.01), similar results were obtained in patients with perfusion defect. The size of perfusion defect on myocardial SPECT have no correlation between RPPr and RPPs/RPPr. The side effects of dipyridamole included chest pain and chest tightness, headache, abdominal pain, dizzness, nausea, and dyspnea. As increasing age, dipyridamole-induced cardiac work at rest was increased, cardiac response to dipyridamole was decreased.

N/A 99mTc-MAG, was synthesized, and biodistribution and blood clearance rate were compared with those of 131I-OIH in mice and rats respective]y. Finally renal image was obtained from a normal male volunteer before and after prescription of probenecid. The results obtained were as follows: 1) The uptalce of Tc-MAG, by kindey was higher than that of 131I-OIH in mice 10 mins after injection (n=6, p<0.05), but slightly lower uptakes were found in all organs (kindney, blood, stomach, mtestinge and liver) 2 hrs after injection. 2) For ' Tc-MAG, t1/2 a=2.4-+0.0 min, t1/2B=44.3+7.4 min, and blood clearance=3.4+0.5ml/min, and for 131I-OIH t1/2=1.8=0.2 min, t1/2=69.1+9.5 min, and blood clearance=1.3+O.l min were found in rats. 3) From the renogram of normal male volunteer, we could find that tmax=130 sec and t1/2=430sec before probenecid prescriptiori, and tmax 150-170sec and t1/2=810-1,170sec after probenecid prescriprion. From these results we concluded that Tc-MAG, can be used instead of 131I-OIH for obtaining renal irnage.

N/A Pulmonary embolism demands rapid and accurate diagnosis. And ventilation imaging has greatly improved the diagnostic accuracy of pulmonary embolism in addition to perfusion imaging. Agents currently used include xenon-133, krypton-8lm and technetium-99m radioaerosols. However radioactive gases are compromised by availability and cost for krypton-8lm, radiation dose, gamma energy and non-physiologic behaviour for xenon-133. Radioaerosols of technetium-99m componds are rapidly cleared from the lung after inhalation, and their relative low effeciency (specific radioactivity) and wide distribution of particle sizes make them also suboptimum. A new ventilation agent, Technegas is a suspension of structured graphite ellipsoids with diameter below 20nm, labelled with Tc-99m in a carrier gas of Argon. This report describes the authors' clinical experience with Technegas. This is the first reported clinical study of this agent in Korea. A comparison of Technegas and Tc-99m-DTPA aerosol was performed in 12 patients with various pulmonary diseases such as COPD, pulmonary tuberculosis and pleural effusion. All patients were studied with Tc-99m-DTPA aerosol inhalation and Technegas ventilation. In both studies image quality was assessed (1) semiquantitatively by scoring bronchial and gastric activity, (2) subjectivly by direct visual comparisan of peripheral lung images and (3) quantitatively by computing the peripheral penetration index(PI) for each lungs. The bronchial activites were seen in 7 out of 12 cases with Tc-99m-DTPA aerosol and in 5/12 with Technegas. The gastric activities were seen in 5/12 and 1/12 cases respectively. The average values of PI were 61.26% with Tc- 99m-DTPA aerosol and 69.20% with Technegas (p〉0.05). Using Tc-99m-DTPA aerosol, COPD patients showed deposition in the central airways with poor visualization of the peripheral areas of the lungs. In Technegas studies these phenomena were less prominent, and the examination is well tolerated by patient and requires only a minimum of patient cooperation. With superiority of easy availability and handling, better physical characteristics and favorable image quality, Technegas is a promising agent for lung ventilation scanning.

N/A To evaluat.e the diagnostic usefulness of quantitative sacroiliac joint scintigraphy and its clinica1 significance in the ankylosing spondylitis, we measured SIS ratio (Sacroiliac Joint/Sacrum Uptake ratio) by region of interest method in 52 patients with ankylosing spondylitis. As compared with radiological grade, SIS ratio was significantly elevated in the eariy stage of ankylosing spondylitis and returned to normal range in the late stage. Scintigraphic finding of focaI cptakes of radiotracer at multiple spinal apophyseal joints was charactnistic and proximal large joints were niore involved than distaI joints. The incidence and extent of the arthropathy revealed by scnitigraphy exceeded that which was clinically evident, and scintigraphy was more sensitive to detect syrnptomatic lesion than radiography. Therefore, it is suggested that bone scan is useful for the early diagnosis and the assessment of clinical activity of involved joints of ankylosing spondylitis.
