        요추 추골체간 골유합술 술식별 결과에 대한 임상적 고찰

        Lumbar interbody fusion provides the most logical solution to diseases of the lumbar spine instability, such as spondylolysis, spondylolisthesis and retrolisthesis. The 33 cases of lumbar interbody fusion done after Cloward method, Wiltberger method, method using bovine bone employing Cloward technique, and anterolateral approach were analyzed. Posterior interbody fusion after Wiltberger technique using dowel bone graft is a more simplified method, providing accurate approximation of graft and host bone and preventing extrusion of graft then Cloward technique. Immediate correction of extruded graft it any after postoperative CT checking is essential for successful fusion.

        영아기의 원시 신경외배엽성 종양 2례 보고

        The authors are reporting two cases of huge primitive neuroectodermal tumors involving cerebral hemisphere. These are highly malignant neoplasm, both clinically and histologically. They are rapidly growing tumors, with a brief duration of symptoms and a rapidly progressive course. The patients were discharged after subtotal resection of tumor mass with an uneventful course, but one was dead within 2 months after the operation. These tumors usually occur in children and young adult. We have experienced two cases surgically treated in infancy.

        윌슨병의 진단과 분자유전학적 검사

        Wilson disease (WD) is an autosomal recessive disorder of copper metabolism that results in accumulation of copper primarily in the liver, the brain and the cornea. Mutations in the WD gene, ATP7B cause failure of copper excretion from hepatocyte into bile and a defective synthesis of ceruloplasmin. More than 370 mutations are now recognized, scattering throughout the ATP7B gene. Since WD has protean clinical presentations, awareness of WD in clinical practice is important for the early diagnosis and prevention of accumulated copper toxicity. None of the laboratory parameters alone allows a definite diagnosis of WD. There are numerous pitfalls in the diagnosis of WD. Low serum ceruloplasmin concentrations, increased 24 hour urinary copper excretion, increased hepatic copper concentrations and the presence of Kayser-Fleischer rings in the cornea are major diagnostic points. A combination of any two of these 4 laboratory findings is strong support for a diagnosis of WD. Molecular methods are now being used to aid diagnosis. Molecular genetic testing has confirmed the diagnosis in individuals in whom the diagnosis is not clearly established biochemically and clinically. Siblings should be screened for WD once an index case has been diagnosed. Discrimination of heterozygotes from asymptomatic patients is essential to avoid inappropriate lifelong therapy for heterozygotes. Genetic testing, either by haplotype analysis or by mutation analysis, is the only reliable tool for differentiating heterozygote carriers from affected asymptomatic patients. Currently, genetic testing is of limited value in the primary diagnosis. However, genetic testing will soon play an essential role in diagnosing WD as rapid advancement of biomedical technology will allow more rapid, easier and less expensive mutation detection.

        Helicobacter pylori 감염과 소아 복통

        The relationship between H. pylori(Hp) infection and recurrent abdominal pain(RAP) in children is not clear. Current data in the literature regarding a causal relationship between Hp infection and childhood RAP are conflicting. However, meta-analysis and most of the recently published studies have not supported an association between Hp infection and an increased prevalence of abdominal pain. Most published studies have some problems and have been criticized on methodological grounds. Most of the existing studies have not been designed to include double-blind placebo controls. And, because of the low incidence of Hp infection in children, they have a problem of obtaining sufficient numbers of the Hp infected children. There is also a concern about the heterogeneity of testing methodology in the studies and diagnostic accuracy problems of the non-invasive tests such as low sensitivity of the serology tests and false positive results of the UBT in children. And conflicting results may be explained, in part, by the poor definition of RAP or dyspepsia for the study subjects. To date, no consensus exists for the treatment of Hp-infected children with RAP. Some authors suggest the clinical efficacy of Hp. But others suggest that eradication of Hp is not associated with improvement of RAP. The problem of the most treatment studies is that they had usually no control groups and the number of the subjects was small. In order to make reasonable decisions about treatment of Hp infection in children with RAP, long-term, randomized, double blind, well controlled studies in large numbers of children will be required.

        회절 이중초점 인공수정체와 회절 삼중초점 인공수정체의 임상 결과 비교

        Purpose: We compared the short-term visual outcomes of patients fitted with trifocal diffractive intraocular lenses (IOLs) and bifocal IOLs 3 months after IOL implantation. Methods: We included 38 eyes undergoing IOL implantation at a single tertiary hospital. In all, 21 eyes received bifocal IOLs (TECNISⓇ 1 ZLB00; Abbott Medical Optics Inc., Santa Ana, CA, USA) and 17 received trifocal diffractive IOLs (FineVisionⓇ POD F; PhysIOL SA, Liège, Belgium). The primary outcomes were corrected and uncorrected visual acuities at 4 m, 50 cm, and 33 cm on postoperative weeks 1, 4, and 12, and the spherical equivalence values at those times. The secondary outcomes included contrast sensitivity measured using an Optec 6500Ⓡ instrument (Stereo Optical Co. Inc., Chicago, IL, USA) 4 weeks after surgery, the numbers of total and internal optical aberrations assessed using the iTraceⓇ (Tracey Technologies, Houston, TX, USA) instrument at 12 weeks, symptoms including glare or halo, and the need for spectacles when engaging in daily activity. Results: One week after surgery, eyes implanted with trifocal IOLs exhibited significantly better uncorrected near vision compared to eyes that had received bifocal IOLs, and at weeks 4 and 12, uncorrected intermediate vision was also better in the former eyes. The mean spherical equivalence did not significantly differ between the two groups, nor did the contrast sensitivity measured on postoperative week 4 or the numbers of optical aberrations evident at postoperative week 12. Eyes fitted with trifocal IOLs appeared to suffer less from glare or halo, but the between-group difference was not significant. The two groups did not significantly differ in terms of the need to wear supplementary spectacles. Conclusions: Trifocal diffractive IOLs seem to afford superior intermediate vision compared to traditional bifocal IOLs, and may therefore improve visual outcomes at varying distances in patients undergoing cataract surgery. 목적: 최근 소개된 회절 삼중초점 인공수정체의 단기간의 안전성과 유효성을 평가하기 위하여 회절 이중초점 인공수정체와 수술 후 3개월까지의 임상 결과를 비교해 보고자 하였다. 대상과 방법: 회절 이중초점 인공수정체(TECNISⓇ 1 ZLB00, Abbott Medical Optics Inc., Santa Ana, CA, USA) 및 회절 삼중초점 인공수정체(FineVisionⓇ POD F, PhysIOL SA, Liège, Belgium) 삽입술을 시행받은 각 21안, 17안을 대상으로 수술 후 1주, 1개월, 3개월간의 원거리, 중간거리, 근거리(4 m, 50 cm, 33 cm) 나안시력, 원거리 교정시력, 구면렌즈대응치를 측정하였다. 수술 후 1개월 뒤 Optec 6500Ⓡ (Stereo Optical Co. Inc., Chicago, IL, USA)를 이용한 대비감도를 비교하였으며, 수술 후 3개월 뒤 iTraceⓇ (Tracey Technologies, Houston, TX, USA)를 이용한 내부 수차를 비교하였다. 수술 후 눈부심과 달무리 증상에 대하여 설문하였으며, 안경의 필요 유무에 대하여 조사하였다. 결과: 수술 후 1주 뒤, 이중초점 인공수정체군보다 삼중초점 인공수정체군에서 근거리 나안시력(33 cm)이 유의하게 높았으며, 수술 후 1개월, 3개월 뒤에는 중간거리 나안시력(50 cm)이 유의하게 높았다. 두 군 간 구면렌즈대응치, 대비감도, 내부수차의 유의한 차이는 없었다. 수술 후 눈부심 및 달무리 증상은 삼중초점 인공수정체군에서 조금 더 적게 나타났으나 유의한 차이를 보이지는 않았으며, 안경 착용의 필요성도 두 군 간 유의한 차이가 없었다. 결론: 회절 삼중초점 인공수정체는 회절 이중초점 인공수정체보다 우수한 중간거리 시력을 보여, 백내장수술 후 환자의 다양한 거리에서의 시력 호전에 효과적일 것으로 기대된다.

      • 상변화 물질을 이용한 위성 탑재체 열제어기구 개발 및 환경시험

        상변화 물질을 이용한 열제어 장치는 상변화 물질이 갖는 잠열특성, 특정온도 상변화 성질을 활용한 온도제어 장치이다. 이는 첨단 열제어기술로 달탐사선, 화성탐사선 등에 사용되어 달과 화성의 극단적인 열환경에서도 시스템 정상임무 수행에 중요한 역할을 담당하였다. 본 연구에서는 기존 실용급 위성에 적용된 바 있는 위성 탑재체 열제어 장치를 상변화 물질 열제어 장치로 대체하여 경량화 및 열전달성능 개선을 목표로 하였다. 이를 위해 상변화 물질과 히트파이프(Heat Pipe)을 병렬로 연결한 새로운 열제어 시스템에 대한 개념 설계를 수행하였고, 핵심 부품인 상변화 물질 용기의 설계/해석/우주인증시험을 수행하여 개발된 장치의 신뢰성을 확인하였다. 본 연구결과로 제작된 열제어장치는 향후 개발 예정인 달탐사선 열제어에 적용될 수 있을 것으로 예상된다. TCDPCM(Thermal Control Device with Phase Change Material) is a thermal control device designed for spacecraft thermal control using phase change material. TCDPCM’s have been used in thermal control of several lunar rovers, Mars rovers and proved their thermal performance in spite of extreme thermal condition in Moon or Mars. In this study, a new thermal control device using phase change material and heat pipe is proposed and validated. Also, structural analysis and space qualification tests were performed for a PCM Container which is the most important component of the device. TCDPCM shows a good thermal/structural performance and can be used in thermal control device of real spacecraft.
