Adiponectin Concentrations: A Genome-wide Association Study

<P>Adiponectin is associated with obesity and insulin resistance. To date, there has been no genome-wide association study (GWAS) of adiponectin levels in Asians. Here we present a GWAS of a cohort of Korean volunteers. A total of 4,001 subjects were genotyped by using a genome-wide marker panel in a two-stage design (979 subjects initially and 3,022 in a second stage). Another 2,304 subjects were used for follow-up replication studies with selected markers. In the discovery phase, the top SNP associated with mean log adiponectin was rs3865188 in <I>CDH13</I> on chromosome 16 (p = 1.69 × 10<SUP>−15</SUP> in the initial sample, p = 6.58 × 10<SUP>−39</SUP> in the second genome-wide sample, and p = 2.12 × 10<SUP>−32</SUP> in the replication sample). The meta-analysis p value for rs3865188 in all 6,305 individuals was 2.82 × 10<SUP>−83</SUP>. The association of rs3865188 with high-molecular-weight adiponectin (p = 7.36 × 10<SUP>−58</SUP>) was even stronger in the third sample. A reporter assay that evaluated the effects of a <I>CDH13</I> promoter SNP in complete linkage disequilibrium with rs3865188 revealed that the major allele increased expression 2.2-fold. This study clearly shows that genetic variants in <I>CDH13</I> influence adiponectin levels in Korean adults.</P>

유선 인터넷 망의 Mail 서비스에는 POP3(Post Office Protocol 3)규약과 IMAP4 (Internet Message Access Protocol 4)규약을 기반으로 하는 Mail 등이 있다. 무선 인터넷을 이용한 모바일 메일 서비스를 위해서는, POP3 Mail의 경우 전용 모바일 메일 서버를 이용하거나 메일 변환을 거쳐야 하고, Web Mail로 이용되는 IMAP4 메일의 경우에는 원격 파일 서버와 같이 서버 측에 메일이 보관되므로 IMAP4 규약이 없는 모바일 클라이언트로는 모바일 메일 서비스의 이용이 곤란하다. 본 논문에서는 XML 기반의 마크업 언어 변환기를 설계하고, 최근 보편화되고 있는 IMAP4 기반의 Web Mail을 모바일에서 이용할 수 있도록 하는 모바일용 메일 추출 엔진을 구현함으로써, POP3 메일 서비스뿐만 아니라 IMAP4를 기반으로 하는 Web Mail을 모바일 환경에서 이용할 수 있도록 하는 모바일 메일 서비스 방법을 제시하였다.

<P>Background: Acute exacerbation and bacterial pneumonia are major life-threatening conditions in patients with interstitial lung disease (ILD). The rapid recognition of these 2 different conditions is important for their proper treatment. An elevated procalcitonin (PCT) level is commonly detected in patients with bacterial infections. This study assessed the usefulness of the serum PCT level as a biomarker for the differential diagnosis of acute exacerbation and bacterial pneumonia in patients with ILD. Materials and Methods: In this prospective observational study, we enrolled patients with ILD who had experienced recently progressive dyspnea and exhibited new infiltrations on chest radiographs. We classified these patients into an acute exacerbation group and a bacterial pneumonia group and compared their baseline characteristics and laboratory parameters, including the PCT level. Results: Of 21 patients with ILD, 9 patients had bacterial pneumonia. Both the groups showed similar baseline characteristics. The bacterial pneumonia group demonstrated a high PCT level. The PCT level in the acute exacerbation group was significantly lower than that in the bacterial pneumonia group (0.05 versus 0.91ng/mL, respectively; P < 0.001). Other parameters, such as the C-reactive protein level, leukocyte count and body temperature, were also lower in the acute exacerbation group. At a cutoff value of 0.1ng/mL, the sensitivity, specificity and negative predictive values of the serum PCT level were 88.9%, 100.0% and 92.3%, respectively. Conclusions: These findings suggest that the serum PCT level is useful in the differential diagnosis of acute exacerbation and bacterial pneumonia in patients with ILD.</P>

Background: The number of immigrants with tuberculosis (TB) increases each year in South Korea. Determining the transmission dynamics based on whole genome sequencing (WGS) to cluster the strains has been challenging. Methods: WGS, annotation refinement, and orthology assignment for the GenBank accession number acquisition were performed on two clinical isolates from Chinese immigrants. In addition, the genomes of the two isolates were compared with the genomes of Mycobacterium tuberculosis isolates, from two native Korean and five native Chinese individuals using a phylogenetic topology tree based on the Multiple Alignment of Conserved Genomic Sequence with Rearrangements (Mauve) package. Results: The newly assigned accession numbers for two clinical isolates were CP020381.2 (a Korean-Chinese from Yanbian Province) and CP022014.1 (a Chinese from Shandong Province), respectively. Mauve alignment classified all nine TB isolates into a discriminative collinear set with matched regions. The phylogenetic analysis revealed a rooted phylogenetic tree grouping the nine strains into two lineages: strains from Chinese individuals and strains from Korean individuals. Conclusion: Phylogenetic trees based on the Mauve alignments were supposed to be useful in revealing the dynamics of TB transmission from immigrants in South Korea, which can provide valuable information for scaling up the TB screening policy for immigrants.

Immunoglobulin (Ig) G4-related disease is a recently recognized systemic fibroinflammatory condition characterized by a lymphoplasmacytic infiltrate rich in IgG4-positive plasma cells with elevated circulating levels of IgG4. The disease can either be localized to one or two organs, or present as diffuse multi-organ disease. Furthermore, lesions in different organs can present simultaneously or metachronously. In the pulmonary manefestations, lesions associated with IgG4-related disease have been described in the lung parenchyma, airways and pleura, as well as the mediastinum. We report a case of IgG4-related disease presenting as massive pleural effusion and thrombophlebitis.
