Background: Most cases of Graves' disease have been treated by antithyroid drugs in Korea. The major drawbacks of antithyroid drugs are the relative high rate of relapse and the inconvinience of long term therapy. Measurement of TBII and TRH stimulation test have been considered as the most reliable tests predicting relapse, but the predictive power is not high enough to use in clinical situation. This study was performed to find good prognostic indicators in Graves' disease patients after the discontinuation of antithyroid drug therapy. Subjects and methods : We restrospectively evaluated 96 patients with Graves' disease who showed normal TRH test at the end of antithyroid drug therapy and were followed for more than one year. Serum T3, T4, T3/T4 ratio, TBII, Anti-TPO Ab and TGAb levels were measured at the time of diagnosis and at the end of therapy. Two to three months after withdrawal of antithyroid medication, serum TSH and free T3 responses to TRH were examined. These parameters were compared between the relapse group and the remission group. Results : Among the 96 patients, 22 patients(22.9%) relapsed(Group Ⅱ) and 74 patients(77.1%) remained in remmision(Group Ⅰ). No significant difference was observed between the relapse and the remission group in clinical parameters, serum T3, T4, TSH, T3/T4 ratio, TBⅡ and Anti-TPO Ab determined before and after treatment. Serum TSH and free T3 levels measured after TRH stimulation were also similar in both groups. Serum TGAb levels determined at the end of treatment were significantly higher in the remission group(p<0.05). Relapse following the discontinuation of therapy occured within 24 months in 72.7% of the relapsed cases. After 24 months, relapse rate was reduced significantly. Conclusion : These results suggest that high serum TGAb levels could be a favorable prognostic indicator for the long term remission of Graves' disease treated with antithyroid drugs, and may suggest that Hashimoto's thyroiditis is combined with Graves' disease in those patients.

Background: Histamine in food is known to cause food intolerance and chronic urticaria. Although we usually ingest histamine in cooked food, there are few studies about the influence of cooking method on the histamine level. Objectives: The purpose of this study was to determine the influence of cooking method on the concentration ofhistamine. Methods: The histamine level of foods was measured by using enzyme-linked immunosorbent assay (ELISA). Results: When fish was grilled, it resulted in the highest histamine level compared with raw or boiled fish. For meats, grilled meats increased histamine level, while it got decreased when boiling them. For eggs and fermented pastes (e.g., soybean and red pepper paste), there was not much difference in histamine level according to cooking methods. In vegetables, there was no distinct difference of histamine level depending on cooking methods. Interestingly, the histamine level of sea weed laver was increased about fourfold by roasting. For dairy products, cheese showed 20 times higher histamine levels than the ones of fresh milk.Conclusion: The highest histamine concentrations were found in grilled fish and meats. This is the first study that has evaluated the influence of cooking methods on histamine level in various Korean foods.

Atypical mycobacterium and Aspergillus, widely distributed in nature, are opportunistic infection strains, rare pathogens in immunocompetent patients. Recently, the increase of invasive cosmetic treatment and surgery, has increased the infection of both microorganisms. We report a case of cutaneous infection by mixed pathogens, Mycobacterium (M.) fortuitum, M. intracellulare and Aspergillus fumigatus, that occurred after fat graft in immunocompetent patients. A 62-year-old female patient presented painful, erythematous nodules and purulent discharge on her face after fat graft. The skin biopsy and culture demonstrated atypical mycobacterium and Aspergillus. On the Mycobacterium Other Than Tuberculosis identification Polymerase chain reaction-hybridzation (MOTT-PCR), M. fortuitum and M. intracellulare were found. She was treated with clarithromycin, rifampicin and ethambutol for 1 monthafter excision and drainage of pus, and then the skin lesion have improved. In cases of refractory cutaneous infection after invasive cosmetic procedure, atypical mycobacterium or Aspergillus should be suspected.

Xanthomas are plaques or nodules consisting of abnormal lipid deposition and foam cells. The clinical type of xanthoma with hyperlipoproteinemia is categorized into four types: tendinous, tuberous, eruptive, and plantar. Diffuse plane xanthomas are rare disorder presenting diffuse area of yellowish-orange discoloration on the skin, and usually not associated with dyslipoproteinemia. Tuberous xanthomas showed nodules that are frequently localized to the extensor surfaces, and are usually associated with familial hypercholesterolemia and broad beta disease. We report a rare case of diffuse plane xanthoma and tuberous xanthoma which show type Ⅱa hyperlipoproteinemia. A 51-year-old female patient presented yellowish-orange colored plaques on the face and yellowish papules on the arms for 2 years. Serum lipid profile showed abnormal finding, increased total cholesterol and low density lipoprotein (LDL). Furthermore, lipoprotein electrophoresis showed prominent beta lipoprotein portion. She has been treated with cholestyramine and statin for 2 years, then serum total cholesterol and LDL showed normal finding with improved skin lesion.

Discoid lupus erythematosus (DLE) is a chronic form of cutaneous lupus which can cause permanent scarring. Recent studies reported that pulsed dye laser, CO2 laser and intense pulsed light (IPL) have been used for the cosmetic treatment of DLE. It is well known that IPL works in the treatment of telangiectatic lesion andQ-switched 1064-nm Nd:YAG laser works in the treatment of dermal melanosis. We report a case of DLE scar that was successfully treated with IPL and Q-switched 1064-nm Nd:YAG laser. A 42-year-old female patient with a 20-year history of DLE scar presented erythematous and telangiectatic plaques on the face. She had been treated with systemic corticosteroid andhydroxychloroquin for systemic lupus erythematosus for 20 years. She suffered from scarring lesion of DLE, which is refractory to the systemic medicine and topical steroid. She has been treated with IPL and Q-switched 1064-nm Nd:YAG laser for 6 months, and the skin lesion showed marked improvement.

Background: Polychlorinated biphenyls (PCBs) and 2,3,7,8-tetrachlorodibenzo-p-dioxin (TCDD) are major environmental pollutants, and their effects on the human body depend critically on the aryl hydrocarbon receptor (AhR). Objectives: The aim of this study was to evaluate the significance of the AhR and its ligands (TCDD and PCBs) in chronic inflammatory skin disease such as atopic dermatitis (AD) and psoriasis. Methods: We took the skin biopsy samples for real-time PCR and immunohistochemistry staining, and the blood samples for isolation of CD4+ T cell from AD, psoriasis patients and healthy controls. Differentiation of the cells was analyzed using FACS, ELISA, and real-time PCR. Results: The expression of AhR-related mRNA was increased in lesional skin from patients with AD and psoriasis. After treatment of normal human epidermal keratinocytes with TCDD or PCBs, IL-6 and IL-8 production were increased. In the CD4+ T cells with TCDD treatment, CCR10+ cells were significantly increased in psoriasis patients and CD161+CCR10+ cells (CCR10+: expressed in Th22 cells, CD161+: expressed in Th17 cells), levels of IL-4 were significantly increased in AD patients. In the TCDD-treatedgroup, levels of IFN-γ, IL-17, and IL-22 were significantly higher in psoriasis patients and levels of IL-13, IL-17, and IL-22 were significantly higher in AD patients compared to those of the healthy controls. Conclusion: AhR might be highly expressed in patients with AD and psoriasis.

Background: Ingestion of foods that contain much histamine can provoke not only signs and symptoms of histamine poisoning such as nausea, vomiting, and diarrhea, but also allergic reactions such as rash and pruritus. Objectives: The purpose of this study is to present a list of Korean traditional foods containing great amounts of histamine. Methods: The histamine level of foods was measured by using enzyme-linked immunosorbent assay (ELISA). Results: Fermented pastes, such as soybean paste (1164 ng/g) and red pepper paste (1448 ng/g), showed high histamine levels. There wasn’t no difference in histamine level depending on cooking methods, such as boiling. Fermented cabbage (378 ng/g) and radish (500 ng/g), such as kimchi, showed 30 to 50 times higher histamine levels than the ones of fresh cabbage (11 ng/g) and fresh radish (10 ng/g). Salty fermented anchovies showed high histamine levels (1157 ng/g), but salty fermented shrimps (27 ng/g) showed low histamine levels, as did fresh shrimps (24 ng/g). Conclusion: We suggest that this study is used as a guideline for educational dietary interventions for patients with chronic urticaria or histamine intolerance.

Discoid lupus erythematosus (DLE) is a chronic form of cutaneous lupus that can cause permanent scarring. Treatment of DLE includes protection from sunlight and artificial sources of ultraviolet light, as well as systemic and topical medications. The first-line standard therapies are antimalarials and topical steroids. Other systemic therapies include systemic steroid, azathioprine, dapsone, and immunosuppressive agents. Topical tacrolimus and pimecrolimus have also been evaluated. Recent studies reported that several treatments, including pulsed dye laser, CO2 laser, intense pulsed light (IPL), and 1,064-nm long-pulse neodymium- doped yttrium aluminum (Nd:YAG) have been used for the cosmetic treatment of DLE. Here, we report a case of a DLE scar that was successfully treated with a combination therapy of IPL and Q-switched 1,064-nm Nd:YAG laser. (Ann Dermatol 29(3) 331∼333, 2017)
