충주 지역에서 10년간 급성심근경색의 임상관찰에 대한 연구

Background: The current treatments of acute myocardial infarct(AMI) Include noninvasive method using thrombolytics and invasive methods such as primary percutaneous transluminal coronary angioplasty(PTCA) and coronary artery bypass graft(CABG)surgery. Although there are many studies comparing the effectiveness of noninvasive and invasive methods, when large clinical centers, 20% of totar hospitals, were excluded, most middle-sized hospitals lacked personnels and facilities for the invasive methods. Thus they opted for the noninvasive methods. Therefore, in this study, the clinical characteristics of AMI patients and the results of thrombolytic & conservative treatment were observed. Method: 137 patients with acute myocardial infarct, who visited KonKuk University Medical Center, ChoongJu Hospital during Sept. 1990 to Sept. 1999 were analysed retrospectively using medical records. Result: The sexual compositions of patients were 63.5% male, and 36.5% female. The mean age for male patients were 56±12.9, and for female patients 68.7±9.6 (p<0.05). The mean age for female patients was significantly older than male patients. The patients in the thrombolytic treatment group had mortality rate of 6.25% which is lower than that of the conservative treatment group at 12% (p>0.05). Conclusion: Although in our study, small number of acute myocardial infarct patients caused the statistical insignificance, rapid revascularization by thrombolytic therapy within 6 hours of onset of symptoms improved mortality rate and early ambulation. And we had good results on AMI with thrombolytic agent in ChungJu including near rural area as primary emergency hospital. Therefore a prospective research with greater number of subjects is needed to develop better treatments for AMI.

최근 우리 나라는 열두조충과 (Diphyllobothridae)에 속하는 조충류의 인체 감염예가 빈번하게 보고되고 있다. 이는 본 조충류의 감염매체인 어류를 날 것으로 생식할 기회가 많으며, 또한 본 조충류의 제1중간숙주인 물벼룩이 음료수를 통한 인체 감염이 원인일 것으로 추정되고 있다. 조충류의 진단은 충체에 있어 두적과 편절의 형태학적 특징을 감별하여 種의 분류와 진단을 하는 것이 기본적 순서가 될 것이나, 1차적으로 가능한 진단은 환자의 증상 참작과 분변으로부터 충란을 발견 감별하게 된다. 특히 광절열두조충류의 충란에 의한 種鑑別은 환자로부터 편절이나 충체의 확실한 감별을 위한 사전 정보를 가늠하는 뜻에서도 중요성이 있다. 이에 저자들은 환자의 치료와 함께 수집된 Diphyllobothrium latum, D. latum parvum 그리고 Spirometra erinacei의 편절과 충란을 재료로 형태 및 진단적 결과를 분석한 바 그 성적은 다음과 같다. 1. 환자의 분변에서 수집된 충란 50개씩을 각각 선택하여 형태와 크기에 대한 계측을 실시하였다. D. latum의 충란은 난개가 있고 난원형 또는 타원형의 모양으로 관찰되었으며 D. latum parvum은 D. latum에 비하여 더 난원형이면서 작게 관찰되었다. S. erinacei의 충란은 폭이 비대칭적이며 길쭉한 모양으로 관찰되었다. 2. D. latum, D. latum parvum 그리고 S. erinacei의 충란의 크기에서 충란의 평균 길이와 폭은 각각 61.4X41.7㎛, 55.9X41.4㎛ 그리고 66.7X36.4㎛이었다. 3. 각 조충의 편절을 고정한 다음, 첫째 포매하고 microtome으로 절편을 만들어 hematoxylin-eosin 염색을 실시하였고, 둘째 고정과 함께 semicon's aceto-carmine 염색표본을 작성하여 현미경 관찰을 한 결과 D. latum과 D. latum parvum은 생식선이 편절의 중앙선상에 위치하고 개구된 자궁구가 관찰되었다. 그리고 편절의 양측에 난황이 follicle로 관찰되었으며 전형적인 rosette모양을 형성하고 있었다. 그러나 D. latum parvum은 D. latum에 비하여 아주 작은 크기로 관찰되었다. S. erinacei의 편절은 자궁이 나선형으로 5∼7회 이상으로 말려서 관찰되었고 음경낭안에서 저정낭이 이어져 연결되고 있었다. 이로써 본 연구에서는 편절의 형태특징과 함께 충란 크기에 대한 계측과 분석을 하고자 하였으며, 공학현미경에 의한 계측치로 세 종류의 조충 감별에 유의한 참고치를 제기하는 바이다. Recently there have been frequent reports on human infection caused by the Diphyllobothridae in Korea. The adequate opportunities for Koreans to eat raw fish, the primary infection medium of cestodes and the human infection through drinking water by cyclops, the first intermediate host are believed to be main reasons for the infection. The first task of this study was to classify and diagnose the species by differentiating morphological characteristic between scolex and proglottids of cestodes. However, the initially available diagnosis was done with the patient's symptoms and the eggs obtained from his stool. It is important to differentiate the species by the eggs of Diphyllobothrium latum especially in that it can help get advance information for a more reliable analysis in the near future. The morphological and diagnostic results from proglottids and eggs of Diphyllobothrium latum, Diphyllobothrium latum parvum and Spirometra erinacei are as follows; In each kind of cestodes from the patient's stool, the shape and size of 50 eggs were measured. Eggs of Diphyllobothrium latum had an operculum and were ovoidal or ellipsoid to elliptical in shape. Eggs of Diphyllobothrium latum parvum were more ovoidal in shape and smaller in size than Diphyllobothrium latum. And eggs of Spirometra erinacei were asymmetrical in width and long and slender in shape. The average lengths and widths of Diphyllobothrium latum, Diphyllobothrium latum parvum and Spirometra erinacei were 61.4X41.7㎛, 55.9X41.4㎛ and 66.7X36.4㎛, respectively. After the segments of each cestode were fixed, embedding and hematoxylin-eosin dyeing on a microtome-made specimen were done. The micrographs of the semicon's aceto-carmine dyed specimen showed that Diphyllobothrium latum and Diphyllobothrium latum parvum had a centrally-located genital gland and an opened uterine pore. The yolks were observed on both sides of proglottids and had a typical rosette pattern. Yet, Diphyllobothrium latum parvum was shown smaller than Diphyllobothrium latum in the micrograph. Proglottids of Spirometra erinacei displayed that the uterus was rolled spirally more than five to seven times, and connected successively to the seminal vesicle in the cirrus sac. Shown above, this study was performed to measure the size of eggs and analyze the morphological characteristics of proglottids and provided the measurements of three types of cestodes obtained by a light microscope.

<B>ABSTRACT</B><P>The objectives of the current studies were to determine the roles of key enzymes in central carbon metabolism in the context of increased production of antibiotics in <I>Streptomyces coelicolor</I>. Genes for glucose-6-phosphate dehydrogenase and phosphoglucomutase (Pgm) were deleted and those for the acetyl coenzyme A carboxylase (ACCase) were overexpressed. Under the conditions tested, glucose-6-phosphate dehydrogenase encoded by <I>zwf2</I> plays a more important role than that encoded by <I>zwf1</I> in determining the carbon flux to actinorhodin (Act), while the function of Pgm encoded by SCO7443 is not clearly understood. The <I>pgm</I>-deleted mutant unexpectedly produced abundant glycogen but was impaired in Act production, the exact reverse of what had been anticipated. Overexpression of the ACCase resulted in more rapid utilization of glucose and sharply increased the efficiency of its conversion to Act. From the current experiments, it is concluded that carbon storage metabolism plays a significant role in precursor supply for Act production and that manipulation of central carbohydrate metabolism can lead to an increased production of Act in <I>S. coelicolor</I>.</P>
고식적 체외수정시술시 3 일째 배아이식술의 효용성에 관한 연구

Objective: To investigate the positive or negative effect of delaying embryo transfer(ET) one day in IVF-ET. Methods: From May to July, 1997, a total of 64 patients was emolled in this prospective randomized case-controlled study. When the timing of oocyte retrieval was decided, random allocation of patients was made to one of the two groups: day 2 transfer or day 3 transfer. In day 3 transfer group, embryos were cultured in M3 media(Medi-Cult) for further 24 hours. Results: There were no significant differences in age of patients, infertility factor, basal serum FSH level, and serum E2 level on hCG day between two poups, but number of previous IVF-ET cycles was significantly higher in day 3 transfer group(p 0.042). Number of oocytes retrieved, fertilization rate, cleavage rate, and number of embryos transferred had no significant difference, but cumulative embryo score(CES) was significantly higher in day 3 transfer group(p 0.0001). Clinical pregnancy and implantation rates were bigher in day 3 transfer group, but without significance(34.4% vs. 21.9%; 8.7% vs, 5.4%). There were also no significant differences in spontaneous miscarriage and multiple pregnancy rates. Especially in patients over 35 years of age, clinical pregnancy and implantation rates were more higher in day 3 transfer group, but without significance(41.7% vs, 8.3%; 8.5% vs. 1.6%). Conclusion: Considering the higher number of previous cycles in day 3 transfer group, it is at least likely that delaying ET one day may be clinically beneficial in IVF-ET, especially in patients with old age or repeated failure of previous cycles.
폐쇄성 무정자증 환자에서 정자 채취 후 난자 세포질내 정자 주입술의 결과 : 결핵성 부고환염의 영향

Objective: To investigate the intluence of previous tuberculous epididymitis in infertile males with obstructive azoospermia on the outcome of sperm retrieval and intracytoplasmic sperm injection(ICSI) in IVF-ET propam. Methods: Retrospective analysis was paformed in 26 patients with obstructive azoospermia undergoing sperm retrieval and ICSI at Seoul National University Hospital from January, 1996 to August, 1997; 12 cycles in 5 patients with tuberculous obstructive azoospermia(Group A), and 40 cycles in 21 patients with non-tuberculous obstructive azoospermia(Group B). Results: There was no significant difference in the clinical pregnancy rate(PR) per fresh transfer between Groups A and B(20.0%[2/10] vs. 26.8%(11/41)). The rates of embryo implantation and clinical miscarriage were also comparable in both groups(6.3% vs. 11.1%, 50.0% vs. 9.1%). This tendencies were also similar after including five cryopreserved-thawed transfer cycles. Conclusion: Embryo quality and pregnancy outcome in sperm retrieval and ICSI were comparable in both the tuberculous and non-tuberculous obstructive azoospermia patients. Our results suggest that previous history of tuberculous epididymitis in patients with obstructive azoospermia does not affect the outcome of sperm retrieval and ICSI.
Resveratrol Induces Glioma Cell Apoptosis through Activation of Tristetraprolin

Tristetraprolin (TTP) is an AU-rich elements (AREs)-binding protein, which regulates the decay of ARE-scontaining mRNAs such as proto-oncogenes, anti-apoptotic genes and immune regulatory genes. Despite the low expression of TTP in various human cancers, the mechanism involving suppressed expression of TTP is not fully understood. Here, we demonstrate that Resveratrol (3,5,4'-trihydroxystilbene, Res), a naturally occurring compound, induces glioma cell apoptosis through activation of tristetraprolin (TTP). Res increased TTP expression in U87MG human glioma cells. Res-induced TTP destabilized the urokinase plasminogen activator and urokinase plasminogen activator receptor mRNAs by binding to the ARE regions containing the 3' untranslated regions of their mRNAs. Furthermore, TTP induced by Res suppressed cell growth and induced apoptosis in the human glioma cells. Because of its regulation of TTP expression, these findings suggest that the bioactive dietary compound Res can be used as a novel anti-cancer agent for the treatment of human malignant gliomas.
