Purpose: It has been reported recently that peritoneal protein clearance (Pcl) is a marker of endothelial dysfunction and cardiovascular disease in peritoneal dialysis patients. We evaluated whether baseline or longitudinal follow-up Pcl is a factor to consider in predicting the outcome in peritoneal dialysis patients for the follow-up period. Methods: Patients who initiated continuous ambulatory peritoneal dialysis at our center from September 1994 to January 2006 and had a baseline peritoneal equilibration test, measurement of dialysis adequacy, and 24-h dialysate Pcl (24hr dialysate protein loss/ [serum albumin/0.4783]) were included. Demography, comorbidities, and biochemical data were retrospectively collected. Follow-up was until death or the end of the period studied (November 2009). Results: A total of 203 patients (56% men, mean age 55.0±12.5; 55.2% with diabetes; 22.2% with cardiovascular disease) were included. The mean follow up period was 38 months (3-170 months). Baseline Pcl was 128.2 ml/day. Follow up data of Pcl were not changed from baseline period. Cox`s analysis revealed the predictors of mortality (and technical failure) were age and diabetes mellitus but not dialysate/plasma creatinine ratio (D/Pcr) and Pcl. On multivariate analysis, Pcl was negatively correlated with serum albumin and triglyceride and positively correlated with D/Pcr and peritoneal creatinine clearance. Conclusion: Our study showed that the changes in Pcl over time were not significant from baseline up to 53 months. Neither baseline nor mean Pcl during the follow-up period were independent predictors for technical or patient survival.
Improved accuracy in periodontal pocket depth measurement using optical coherence tomography

Purpose: The purpose of this study was to examine whether periodontal pocket could be satisfactorily visualized by optical coherence tomography (OCT) and to suggest quantitative methods for measuring periodontal pocket depth. Methods: We acquired OCT images of periodontal pockets in a porcine model and determined the actual axial resolution for measuring the exact periodontal pocket depth using a calibration method. Quantitative measurements of periodontal pockets were performed by real axial resolution and compared with the results from manual periodontal probing. Results: The average periodontal pocket depth measured by OCT was $3.10{\pm}0.15mm$, $4.11{\pm}0.17mm$, $5.09{\pm}0.17mm$, and $6.05{\pm}0.21mm$ for each periodontal pocket model, respectively. These values were similar to those obtained by manual periodontal probing. Conclusions: OCT was able to visualize periodontal pockets and show attachment loss. By calculating the calibration factor to determine the accurate axial resolution, quantitative standards for measuring periodontal pocket depth can be established regardless of the position of periodontal pocket in the OCT image.

Nontuberculous Mycobacteria, especially Mycobacterium avium complex (MAC) infection is a common opportunistic infection in patients with acquired immunodeficiency syndrome (AIDS). In patients with AIDS, MAC infection more frequently presents as disseminated form rather than localized infection. Disseminated MAC infection is associated with a high mortality rate in patient with AIDS. We report a case of disseminated MAC infection in an AIDS patient involving bone marrow, mediastinal lymph node and lung.
연구논문 : 교육프로그램 참가자 만족도 조사로 본 국립과천과학관의 비형식 과학교육프로그램 운영 방향 연구

이 연구의 목적은 국립과천과학관 교육프로그램 참가자에 대한 만족도 조사를 통해 비형식 과학교육기관의 교육프로그램 운영 방향에 대한 시사점을 알아보는 것이다. 본 연구에서는 국립과학관으로서 4,322 ㎡의 교육 공간을 따로 확보하고 있어 교육의 기능이 용이하며 최근 1년간 1만명 이상의 자체 교육프로그램 수강생을 유치한 경기도 과천시 소재의 국립과천 과학관을 연구 장소로 선정하여 2010년 5월 교육프로그램에 참가자 중 87명의 학생과 78명의 학부모를 대상으로 과학관 교육프로그램에 대한 만족도 및 인식을 조사하였다. 연구 결과, 비형식 과학교육기관이 과학적 소양 및 탐구 능력, 과학적 태도 향상 측면에서 학교교육에 대한 보완을 해 주기를 바라는 인식이 나타났다. 개선해야 할 점으로는 학생 활동에 대한 피드백, 학년별 세분화, 강사의 수업방식과 홍보에 대한 요구가 있었으며, 이에 대한 개선책으로 지속적으로 참여하는 학생을 위한 중장기 프로그램을 마련, 접수대상 제한, 수업 지도안 개발, 홍보 방식의 개선 등이 필요할 것으로 보인다. This study is to investigate the direction for management of informal science education center by survey of participants` satisfaction and implication. The place to study was Gwacheon National Science Museum in Kyung-gi province. 4,322㎡ for the education space can make education done effectively. And attendees of their own education program are over 10,000 per year. 87 students who attend education program and 78 of their parents joined the survey of participants` satisfaction. The results of this study show that most of the participants want informal science education center to be able to make up for school education in part of promoting scientific literacy, heuristic method and scientific attitude. Things to be improved were feedback about student activity, segmentation of the education program for each grade, public relations exercise of program and advanced method of teaching based on each subject of classes. As ideas for improvement, it seemed that making long term program for continuous participant, limiting participant appropriately, developing guide manual of teaching, improving publicity of program were required.
소형 수직축 풍력터빈에 대한 동특성 분석 및 동적모델 수정

The dynamic characteristics including natural frequencies and excitation frequencies were evaluated for a small 10 kW vertical axis wind turbine for investigating the root cause of excessive vibration of a structure under 30RPM of rotor speed. Acceleration responses were measured at 12 distributed locations for ambient vibration tests during non-operational and operational conditions. The natural frequencies for the lowest 2 bending modes were estimated and the excitation frequencies, i.e. 1P, 2P, 4P, were also estimated according to the rotational speed using the responses under operational conditions (i.e. power generation condition). It was verified that the excessive vibration was due to the resonance between the 4P excitation and the 1st vibration modes. To resolve the resonance issue, dynamic model modification was carried out and concrete filling inside the steel tower up to 7 m height was the most efficient way to increase the 1st natural frequency about 20% by numerical simulation. After the concrete filling, the acceleration responses were measured again and it was observed that the 1st natural frequency was increased as amount of 21% by concrete filling.
Validation of the oxford classification of iga nephropathy: A Single-Center Study in Korean Adults

Background/Aims: The recently published Oxford classification of IgA nephropathy (IgAN) proposed a split system for histological grading, based on prognostic pathological features. This new classification system must be validated in a variety of cohorts. We investigated whether these pathological features were applicable to an adult Korean population. Methods: In total, 69 adult Korean patients with IgAN were analyzed using the Oxford classification system at Soonchunhyang University Hospital, Seoul, Korea. All cases were categorized according to Lee`s classification. Renal biopsies from all patients were scored by a pathologist who was blinded to the clinical data for pathological variables. Inclusion criteria were age greater than 18 years and at least 36 months of follow-up. We excluded cases with secondary IgAN, diabetic nephropathy combined other glomerulopathies, less than 36 months of follow-up, and those that progressed rapidly. Results: The median age of the patients was 34 years (range, 27 to 45). Mean arterial blood pressure was 97 ± 10 mmHg at the time of biopsy. The median follow-up period was 85 months (range, 60 to 114). Kaplan-Meier analysis showed significant prognostic predictions for M, E, and T lesions. A Cox proportional hazard regression analysis also revealed prognostic predictions for E and T lesions. Conclusions: Using the Oxford classification in IgAN, E, and T lesions predicted renal outcome in Korean adults after taking clinical variables into account.
공무원 총액인건비제에서 공공도서관 사서직 인력에 대한 고찰

본 연구의 목적은 공무원 총액인건비제가 시행되는 시기에 새로 공공도서관을 개관한 기초자치단체의 사서직 공무원 정원 변화를 분석하는데 있다. 연구방법으로 2008년에서 2010년 사이에 공공도서관을 2개관 이상 개관한 전국의 15개 기초자치단체를 선정하여, 이들 기초자치단체의 공무원 정원과 사서직 정원을 "자치법규정보시스템"과 해당 기초자치단체 홈페이지에서 데이터를 수집하여 분석하였다. 연구결과 총액인건비제가 실시된 시기에 조사대상의 15개 기초자치단체의 공무원 정원은 증가하였으나, 사서직 정원의 증가는 극히 미약하였다. 또한 복수직렬을 이용하여 행정직원 수를 증가시킨 결과를 보여주고 있었다. 결과적으로 총액인건비제 때문에 사서직 정원을 증가시킬 수 없다는 것은 사실이 아니다는 것이 밝혀졌다. The purpose of this study is to identify the influence of the change on librarian number of local governments on the lump-sum personnel expenditure system. Data were collected and analyzed from the Enhanced local laws and regulations information system and local government home pages of 15 local governments where opened more than two public libraries from 2008 to 2010. Results show that the number of 15 government employees was increased but the number of librarian was slightly increased. Also, the number of administrative positions was increased using plural series position system. It shows that the number of librarian can be increased under the lump-sum personnel expenditure system.
