Accuracy of cardiac output measurements during off-pump coronary artery bypass grafting: according to the vessel anastomosis sites

        Sung Yong Park,Dae Hee Kim,Han Bum Joe,Ji Young Yoo,Jin Soo Kim,Min Kang,Yong Woo Hong 대한마취통증의학회 2012 Korean Journal of Anesthesiology Vol.62 No.5

Background: During beating heart surgery, the accuracy of cardiac output (CO) measurement techniques may be influenced by several factors. This study was conducted to analyze the clinical agreement among stat CO mode (SCO), continuous CO mode (CCO), arterial pressure waveform-based CO estimation (APCO), and transesophageal Doppler ultrasound technique (UCCO) according to the vessel anastomosis sites. Methods: This study was prospectively performed in 25 patients who would be undergoing elective OPCAB. Hemodynamic variables were recorded at the following time points: during left anterior descending (LAD) anastomosis at 1 min and 5 min; during obtuse marginal (OM) anastomosis at 1 min and 5 min: and during right coronary artery (RCA) anastomosis at 1 min and 5 min. The variables measured including the SCO, CCO, APCO, and UCCO. Results: CO measurement techniques showed different correlations according to vessel anastomosis site. However, the percent error observed was higher than the value of 30% postulated by the criteria of Critchley and Critchley during all study periods for all CO measurement techniques. Conclusions: In the beating heart procedure, SCO, CCO and APCO showed different correlations according to the vessel anastomosis sites and did not agree with UCCO. CO values from the various measurement techniques should be interpreted with caution during OPCAB.

1997 Clinical Endoscopy Vol.17 No.3

Background: Use of antispasmodic medication prior to colonoscopy is controversial but someone believes antispasmodic may improve visualization of colonic mucosa and ease colonoscope insertion. So, we designed a study to assess the effect of premedication with the antispasmodic, hyoscine-N-butyl bromide(Buscopan') on the performance of colonoscopy. Methods: This study was prospective, double blinded, randomized, controlled study, One hundred three consecutive patients were randomized to receive intravenous buscopan lml(n=52) or placebo(n=51) combined with our standard initial medication(me- peridine 50 mg and midazolam 2 mg). Insertion of colonoscopy was timed, and 100 mm visual analogue scales (VAS) were used for asscssing difficulty of procedure, colonic motility, frequency of positional change, frequency of external compression, difficulty of assistance and degree of discomfort experienced by the patients. Results: There were no significant differences of intubation time between buscopan group(mean time, 7.23 min., range 2~15) and placebo group(7.07 min., range 3-25), (p=0.83) and withdrawal time between buscopan group (6.46 min., range 2-22) and placebo group(6.76 min., range 2 25), (p=0.69). Also, there was no significant differences in intubation time between males and females(buscopan; males 7.00 min., females 7.60 min., p=0.34, placebo; males 7.0~5 min., females 7.08 min., p 0.44). The VAS scores checked by endoscopist(p=0.29), assistant(p=0.32) and patient (p=0.15) were not significantly different in both groups. There were no significant differences in intubation time, VAS scores nf endoscopist, assistant, and patients. Conclusion: Premedication with intravenous bu.opan has no advantage on colonoscopy procedure. Use of antispasmodic medication prior to colonoscopy was not considered as recommendable agent.

Dong-Seok Park¹, Ji-Hoon Kim¹ and Byung-Il Min, Soo-Myung Oh,³¹Department of Acupuncture and Moxibustion, College of Medicine, ²Department of Physiology, College of Medicine, And ³East-West Medical Research Institute, Kyung Hee University,Seoul, Korea. Analgesic Effect of Manual Acupuncture and Electroacupuncture combined with Twirling Method. Proceeding of International Symposium on East-West Medicine Seoul. 187-200, 1999.-Twirling the needle (TN) is one of several needing methods frequently used for acupuncture in the oriental medicine. The present study was conducted to see if TN enhanced the antinociceptive effect produced by plain-or electroacupuncture in the rat tail flick test. Rats (Sprague-Dawley, 200-300g)lightly anesthetized with thiopental sodium(40mg/kg, i.p.) were used in the tail-flick test. The basal reaction time for tail-flick was 2 sec. For "plain" acupuncture(PA), a needle was inserted for 20 min into a Chock-samni (ST36) acupuncture point which (is located at the anterior tibial muscle and about 10mm below the knee joint). For electroacupuncture(EA), train-pulses(3 Hz,0.3-ms pulse width, 0.2-0.3mA) were applied to the inserted needle for the period of insertion. For combining TN with either PA or EA, TN was performed during the each acupuncture period. Two methods for TN were used; twirling 3 times for 1 second persisted for 1 min in every 5 mins(long-duration and long-interval;LDLI) and twirling 3 times for 1 second persisted for 10 sec in every 1 min(short-duration and short-interval;SDSI). PA and EA produced an increase in tail flick latency (TFL); peak increases were 21.3±5.1% and 42.1±17.3% of the pre-acupunture control, respectively, and occurred immediately after cessation of 20min-acupuncture. Performing TN combined with PA increased TFL more than PA itself by showing a greater peak increase in TFL when performed with a SDSI-TN than with a LDLI-TN(60.6±12.5% and 42.7±22.7% of the pre-acupuncture control, respectively). Performing TN combined with EA also increased TFL more than EA itself and a greater peak increase in TFL was observed with a SDSI-TN as compared to a LDLI-TN(77.5±13.8% and 67.3±14.0% of the pre-acupuncture control, respectively). These results indicate that TN enhances both PA-and EA-produced antinociception where an greater enhanced antinociception than a LDLI-TN. It is suggested that performing a SDSI-TN combined with EA seems to be one of the most effective ways to produce analgesia.

Effect of Jaeumkanghwatang (JEKHT), a Polyherbal Formula on the Pharmacokinetics Profiles of Tamoxifen in Male SD Rats (1) - Single Oral Combination Treatment of Tamoxifen 50 mg/kg with JEKHT 100 mg/kg within 5 min -

Kwak, Min A,Park, Soo Jin,Park, Sung Hwan,Lee, Young Joon,Ku, Sae Kwang The Society of Korean Medicine 2016 대한한의학회지 Vol.37 No.2

Objectives: The objective of this study was to elucidate the effect of Jaeumkanghwatang (JEKHT) on the plasma concentration and pharmacokinetics of tamoxifen in combination therapy as a process of the comprehensive and integrative medicine against breast cancer. Methods: After 50 mg/kg of tamoxifen treatment, JEKHT 100 mg/kg was orally administered within 5 min. The plasma were collected at 30 min before administration, 30min, 1, 2, 3, 4, 6, 8 and 24 hrs after end of JEKHT treatment, and plasma concentrations of tamoxifen were analyzed using LC-MS/MS methods. PK parameters of tamoxifen ($T_{max}$, $C_{max}$, AUC, $t_{1/2}$ and $MRT_{inf}$) were analysis as compared with tamoxifen single administered rats. Results: JEKHT did not influenced on the plasma concentrations and pharmacokinetics of tamoxifen after single oral co-administration, within 5min except for some negligible effects on plasma concentration. The $T_{max}$, $C_{max}$, AUC, $t_{1/2}$ and $MRT_{inf}$ of tamoxifen in co-administered rats were quite similar to those of tamoxifen single treated rats. Conclusions: Based on the results of the present study, JEKHT did not influenced on the oral bioavailability of tamoxifen, when they were single co-administered within 5min. However, more detail pharmacokinetic studies should be tested to conclude the possibilities that can be used as comprehensive and integrative therapy with JEKHT and tamoxifen for breast cancers, when they were co-administered, like the effects on the pretreatment of JEKHT and after repeat co-administrations.

The purpose of this study is to quantitatively evaluate the amount of carbon storage for trees in forest ecosystem to support the foundation for carbon neutrality implementation in Korea National Park. It targeted 22 national parks designated and managed as national parks in Korea, and conducted research on forest trees in the terrestrial ecosystem among various natural and ecological carbon sink. The survey and analysis method followed the IPCC guidelines and the National Greenhouse Gas Inventory in Korea. The amount of tree carbon storage in the forest ecosystem of Korea National Park was confirmed to be about 218,505 thousand CO2-ton and the amount of carbon storage per unit area was 570.8 CO2-ton per hectare. Compared to 299.7 CO2-ton per hectare, the average carbon storage per unit area of the entire Korean forest, it was found that about twice as much carbon was stored when assuming the same area. In other words, it means that the tree carbon storage function of the national park is about twice as high as that of the average tree carbon storage function of entire Korean forest. It has great implications in Korea National Park not only provides biodiversity promotion and exploration services as a national protected area, but also performs excellent functions as a carbon sink.

Poly(vinylbenzyl chloride-glycidyl methacrylate)/Polyethylene Composite Anion Exchange Membranes for Vanadium Redox Battery Application

        Park, Min-A,Shim, Joonmok,Park, Se-Kook,Jeon, Jae-Deok,Jin, Chang-Soo,Lee, Ki Bong,Shin, Kyoung-Hee Korean Chemical Society 2013 Bulletin of the Korean Chemical Society Vol.34 No.6

Anion exchange membranes for a vanadium redox flow battery (VRB) were prepared by pore-filling on a PE substrate with the copolymerization of vinylbenzyl chloride (VBC) and glycidyl methacrylate (GMA). The ion exchange capacity, water uptake and weight gain ratio were increased with a similar tendency up to 65% of GMA content, indicating that the monomer improved the pore-filling degree and membrane properties. The vanadium ion permeability and open-circuit voltage were also investigated. The permeability of the VG65 membrane was only $1.23{\times}10^{-7}\;cm^2\;min^{-1}$ compared to $17.9{\times}10^{-7}\;cm^2\;min^{-1}$ for Nafion 117 and $1.8{\times}10^{-7}\;cm^2\;min^{-1}$ for AMV. Consequently, a VRB single cell using the prepared membrane showed higher energy efficiency (over 80%) of up to 100 cycles compared to the commercial membranes, Nafion 117 (ca. 58%) and AMV (ca. 70%).
