        Nosocomial Outbreak of COVID-19 from a Kidney Transplant Patient: Necessity of a Longer Isolation Period in Immunocompromised Patients

        Background: Determination of the release from isolation for coronavirus disease 2019 (COVID-19) in immunocompromised patients who need additional hospitalization for treatment of non-COVID-19 related disease is important to prevent nosocomial transmission. However, there is insufficient evidence for an extended isolation period. Materials and Methods: In September 2021, when the Delta variant was dominant, a nosocomial outbreak of COVID-19 occurred in the nephrology ward of a tertiary hospital in Gwangju, Korea. We conducted epidemiological investigations and whole-genome sequencing (WGS) of this virus. Results: A man who underwent kidney transplantation was admitted to our hospital for the treatment of acute kidney injury. He was diagnosed with asymptomatic COVID-19 infection during a pre-admission screening test on September 1, 2021 and underwent isolation. After 10 days of isolation in the COVID-19-designated ward, he was transferred to the general nephrology ward. He underwent steroid pulse therapy (September 17 to September 23, >60 mg/day prednisolone) due to acute T-cell rejection. On September 28, 2021, the first patient with COVID-19 was identified in the nephrology ward, and a rapid-response team was activated to identify additional patients with COVID-19 and prevent the spread of COVID-19. Epidemiological investigations revealed that 12 patients, two caregivers, and three healthcare workers from the nephrology ward were diagnosed with COVID-19. The WGS of specimens from 14 nosocomial outbreak samples and released an index patient exhibited the same Delta variant originating from the B.1.617.2 lineage. This hospital-acquired COVID-19 outbreak in the nephrology ward resulted in two (11.7%) deaths in patients who underwent kidney transplantation. Conclusion: We demonstrated that an immunocompromised patient can cause a nosocomial outbreak due to the prolonged shedding of infectious viruses. Prolonged isolation in patients under active immunosuppressive therapy may be necessary to prevent transmission, especially in the hospital setting.

        대학 교양체육 수업 참여자의 사회적 지지가 자아존중감 및 운동 지속에 미치는 영향

        This study aims to further revitalize university liberal arts classes by identifying the impact of social support on self-esteem and athletic persistence so that college students can enjoy a pleasant social life by presenting the ultimate measures to continue sports activities as lifelong sports. For this purpose, 356 male and female university students in Gyeonggi Province were analyzed, and the data processing concluded that Frequency Analysis, Cronbach's alpha were conducted with reliability analysis, descriptive analysis, t-test, correlation analysis, and multiple regression analysis. First, there were significant differences in social support, self-respect, and athletic persistence depending on the general characteristics of participants in university liberal arts and sports classes. Second, self-esteem has been shown to have a positive influence on social support. Thirdly, exercise continuation has been shown to have a positive influence on social support. Fourth, self-esteem has been shown to have a positive influence on exercise continuation. As suggested in this study, it has been shown to have a significant impact on social support, self-respect, and athletic persistence of participants in university liberal arts and physical education classes.

        초등교사의 갈등관리유형과 갈등요인 및 학교조직효과성과의 관련성 탐색

        본 연구는 초등교사의 갈등관리유형과 갈등요인 및 학교조직효과성과의 관련성을 탐색하는데 그 목적이 있다. 초등교사가 학교에서 가장 크게 갈등을 느끼고 있는 두 대상인 학교행정가와 동료교사와의 갈등관리유형을 세분화하기 위해 본 연구에서는 잠재프로파일분석을 실시하였다. 교사와 학교행정가 간의 갈등에서 ’행정가 의존형‘은 갈등을 거의 보이지 않은 반면에 ’업무상요인과 의사소통요인‘ 에서 ’개인욕구충족형‘이 ’다수의견수용형‘보다 갈등이 나타날 확률이 높았다. 또한 교사와 동료교사 간의 갈등에서는 ’개인적인 요인‘에서 유일하게 ’무관심형‘이 갈등 확률이 높게 나타났다. 초등교사의 갈등관리유형과 학교조직효과성과의 관련성은 교사와 학교행정가 사이에서는 개인욕구충족형>행정가의존형>다수의견수용형 순으로 직무성과, 직무만족, 조직적응이 모두 높게 나타났으며, 교사와 동료교사 사이에서는 ’무관심형‘이 ’주인의식 강조형‘이나 ’개인욕구충족형‘보다 직무만족과 직무성과가 높게 나타나 유의미한 차이를 보였다. 이러한 연구결과는 향후 잠재집단별 특성과 집단의 의미를 이해하는 데 필요한 기초도구로 잠재프로파일 분석이 활용될 수 있을 것이다. The purpose of this study is to investigate the relationship between elementary school teachers" conflict management types, conflict factors and school organization effectiveness. In order to disaggregate the types of conflict management between teachers, school administrators, teachers and fellow teachers, two subjects whose primary teachers are most at odds in school, the study conducted a Latent profile analysis. Through this, "conflict was intended to overcome the limitations shown by existing research by confirming that there was one or more types of features. As a result, the conflict between teachers and school administrators showed little conflict between "administration-dependent" while in "business-related and communication factors", "personal desire-referring" was more likely than "multiple dog-acceptance". Also, in the conflict between teachers and fellow teachers, the only personal factor was that "non-interesting" was more likely to conflict. The association between conflict management type of elementary school teachers and effectiveness of school organizations is that among teachers and school administrators, the individual desire fulfillment type is high in the order in which the majority of teachers and school administrators are satisfied with their job performance, job satisfaction, and organizational adaptability are all higher, and among teachers and fellow teachers, the "non-interest type" is higher than the "main"s emphasis" or "personal motivation." These findings could be used as a basic tool for understanding the characteristics of each potential group and the meaning of the group.

        Source Analysis and Effective Control of a COVID-19 Outbreak in a University Teaching Hospital during a Period of Increasing Community Prevalence of COVID-19

        Background: South Korea has been experiencing a third wave of coronavirus disease 2019 (COVID-19) since mid-November 2020. Our hospital in Gwangju metropolitan city experienced a healthcare-associated COVID-19 outbreak early in the third wave. The first confirmed COVID-19 patient was a symptomatic neurosurgery resident with high mobility throughout the hospital. We analyzed the transmission routes of nosocomial COVID-19 and discussed infection control strategies. Methods: We retrospectively analyzed the severe acute respiratory syndrome coronavirus 2 reverse transcription-polymerase chain reaction (RT-PCR) testing results according to time point and evaluated transmission routes. Results: Since COVID-19 was first confirmed in a healthcare worker (HCW) on 11/13/2020, we performed RT-PCR tests for all patients and caregivers and four complete enumeration surveys for all HCWs. We detected three clusters of nosocomial spread and several sporadic cases. The first cluster originated from the community outbreak spot, where an asymptomatic HCW visited, which led to a total of 22 cases. The second cluster, which included patient-to-patient transmission, originated from a COVID-19 positive caregiver before diagnosis and the third cluster involved a radiologist and a banker. We took measures to isolate Building 1 of the hospital for 17 days and controlled the outbreak during a period of increasing community COVID-19 prevalence. Universal screening of all inpatients upon admission and resident caregivers was made mandatory and hospital-related employees are now screened monthly. Conclusion: Infection control strategies to prevent the nosocomial transmission of emerging infectious diseases must correspond with community disease prevalence. Our data reinforce the importance of multi-time point surveillance of asymptomatic HCWs and routine surveillance of patients and caregivers during an epidemic.

        The Immediate Effects of Ankle Restriction Using an Elastic Band on Ground Reaction Force during a Golf Swing

        Purpose: The purpose of this study was to analyze the immediate effects of ankle restriction with an elasticband on ground reaction force during a golf swing.Method: There were five subjects who were teaching pros with an average golf score of 75. A force platform(9281B, Switzerland) was used. The independent variable was the presence of an elastic band. The dependentvariables were three-dimensional ground reaction forces to analyze the transfer of momentum with thetiming, control and coordination of the three forces. A paired t-test within subject repeated measure designwas used via an SPSS 20.0.Results: Wearing an elastic band around one``s ankles significantly makes shorter time differences betweenthe moment of cross anterior / posterior forces and vertical force and median value of anterior / posteriorforces during the backswing, between medial and lateral maximum and anterior / posterior force from thetop of the back swing to the mid down swing, and creates an anterior / posterior maximum force.Conclusion: Wearing an elastic band around one’s ankles affects control and coordination between threedimensional forces, and anterior force power according to each phase of the golf swing.

        Adjuvant therapy using ex vivo-expanded allogenic natural killer cells in hepatectomy patients with hepatitis B virus related solitary hepatocellular carcinoma: MG4101 study

        Backgrounds/Aims: Fewer reports have been published regarding hepatectomy patients with solitary hepatocellular carcinoma (HCC) who received immunotherapeutic agents as adjuvant therapy. We evaluated the safety and efficacy of ex vivo-expanded allogenic natural killer (NK) cells in those patients with modified International Union Against Cancer (UICC) stage T3. Methods: From August 2014 to October 2015, five patients who underwent hepatic resection received ex vivo-expanded allogenic NK cells. Patients received five rounds of NK cells (2-3×10<SUP>7</SUP> cells/kg) at postoperative 4, 6, 8, 12, and 16 weeks. This study is registered with ClinicalTrials.gov, number NCT02008929. Results: The median age of the five patients (three men and two women) was 44.8 years (range, 36-54 years). All had hepatitis B virus-related HCC, and the median tumor size was 2.2 cm (range, 2.1-8.2 cm). None of the patients had any adverse events. HCC recurrence developed in two patients at one year after hepatic resection, but four patients were alive at 3 years. The two recurrence-free patients showed a higher ratio of CD8+ T lymphocyte populations before and after administration of ex vivo-expanded allogenic NK cells compared with the three patients who experienced recurrence. Conclusions: Immunotherapy using ex vivo-expanded allogenic NK cells in hepatectomy patients can be used safely. Further studies should be investigated for efficacy.

        Effects of thiol compounds on in vitro maturation of canine oocytes collected from different reproductive stages

        <P>Various thiol compounds are known to improve cytoplasmic and/or nuclear maturation of oocytes in vitro. The present study examined the effects of two thiol compounds, cysteine (0.1, 0.5, and 1.0 mM) and cysteamine (50, 100, and 200 µM), on cytoplasmic and nuclear maturation of canine oocytes. Oocytes collected from different reproductive stages were cultured in TCM-199 supplemented with 10% fetal bovine serum, 2.2 mg/ml sodium carbonate, 2.0 µg/ml estrogen, 0.5 µg/ml FSH, 0.03 IU/ml hCG, and 1% penicillin–streptomycin solution for 72 h. Data were analyzed by two-way ANOVA after arcscine transformation and protected by Bonferroni post hoc test. The effects of cysteine and cysteamine on canine IVM were varied depending on the reproductive stage of oocyte donor bitches. In the follicular stage, significantly more oocytes reached the metaphase II (M II) stage when cultured with 0.5 or 1.0 mM cysteine (16.7% and 16.9%, respectively) compared to the control (6.2%). In the follicular stage, cysteamine increased oocyte maturation rate upto the M II stage (15.1% to 17.0%) compared to the control (4.4%). Both the 0.5 mM cysteine and 100 µM cysteamine, alone or together, increased the intracellular GSH level of canine oocytes compared to the control. Irrespective of reproductive stage, no further beneficial effects on nuclear or cytoplasmic maturation were observed when 0.5 mM cysteine and 100 µM cysteamine were supplemented together.       • Production of Porcine Cloned Embryos Derived from Conditionally Exogenous Gene Expressing cells using Cre-loxP

        The necessity of conditional gene expression in pigs for transgenic models is raised. Thus, in this study, Cre-loxP conditional expression in porcine fetal fibroblasts was investigated and the transformed fibroblasts were reprogrammed in enucleated oocytes for further early embryonic development. Fetal fibroblasts from miniature pigs were used for transfection with pCALNL-DsRed including floxed neomycin resistant gene and selected with 750 ug/mL neomycin for two weeks. The transfected cells did not express DsRed under fluorescence microscope. After transient transfection of plasmid DNA expressing Cre, the fibroblasts began to express DsRed. The cells expressing Ds- Red were employed into somatic cell nuclear transfer (SCNT). A total of 121 oocytes were used for SCNT and 76 cloned embryos (62.8%)were cleaved. Six blastocysts were grown up after SCNT and expressed DsRed. Deletion of floxed neomycin resistant gene was confirmed by RT-PCR in cloned blastocysts. Taken together, this study demonstrated that Cre-loxP recombination in miniature pig fibroblasts were successfully worked and those sequential transformed cells were developed into pre-implantation stage via SCNT.
