        아로마요법이 혈액투석환자의 불안과 우울에 미치는 효과

        Introduction : Chronic hemodialysis patients have been psychological problems because of being compelled to follow a dialysis schedule for a long period of time. Specificially, depression and anxiety are so frequently observed that psychological management is requireed for most hemodialysis patients. Fragnance has been known to have beneficial psychological and physiological effects on human. This study was designed to investigate the psychological effect of aromatherapy on chronic hemodialysis patients. Methods : 40 subjects of this study selected from Je Young-Sung Internal Medicine Clinic in Busan. The experimental group were random assigned to 20 subjects who had visited on Monday,& Wendesday & Friday and received aromatherapy every day for 2 weeks. the control group were random assigned to 20 subjects who had visited on Tuesday & Thursday & Saturday and did not receive any intervention. Experiment had been conducted from July. 7th July 21, 2003 and anxiety by Spilberger's state anxiety scale and depression by Zung's Scale were measured in the course of aromatherapy for both experimental group and control group. A control period consisting of natural hospital smells was established before each test session, and then aromatic test conditions were systematically evaluated for odorless conditions as well as aromatic conditions containing 100% lavender necklace for 2 weeks. Data were analyzed by, frequency, percentage, mean, S.D. χ²- test, t-test. Results : 1) Anxiety scores were not significantly decreased in the experimental group(54.57±13.20) after treatment compared to the control group(49.45±12.41)(t=1.225, p=.228). 2) Depression scores were significantly decreased in the experimental group(43.95±5.63) after treatment compared to the control group(49.65±9.84)(t=-2.367, p=.023). Conclusions : These findings indicate that the aromatherapy may decreased depression. It is suggested that the aromatherapy could be an effective nursing intervention to reduce to depression by chronic hemodialysis patients.

        Probiotic and Antioxidant Properties of Novel Lactobacillus brevis KCCM 12203P Isolated from Kimchi and Evaluation of Immune- Stimulating Activities of Its Heat-Killed Cells in RAW 264.7 Cells

        The purpose of this study was to determine the probiotic properties of Lactobacillus brevis KCCM 12203P isolated from the Korean traditional food kimchi and to evaluate the antioxidative activity and immune-stimulating potential of its heat-killed cells to improve their bio-functional activities. Lactobacillus rhamnosus GG, which is a representative commercial probiotic, was used as a comparative sample. Regarding probiotic properties, L. brevis KCCM 12203P was resistant to 0.3% pepsin with a pH of 2.5 for 3 h and 0.3% oxgall solution for 24 h, having approximately a 99% survival rate. It also showed strong adhesion activity (6.84%) onto HT-29 cells and did not produce β-glucuronidase but produced high quantities of leucine arylamidase, valine arylamidase, β-galactosidase, and N-acetyl-β- glucosaminidase. For antioxidant activity, it appeared that viable cells had higher radical scavenging activity in the 2,2-diphenyl-1-picryl-hydrazyl (DPPH) assay, while in the 2- azinobis-(3-ethylbenzothiazoline-6-sulfonic acid (ABTS) assay, heat-killed cells had higher antioxidant activity. Additionally, L. brevis KCCM 12203P showed higher lipid oxidation inhibition ability than L. rhamnosus GG; however, there was no significant difference (p < 0.05) between heat-killed cells and control cells. Furthermore, heat-killed L. brevis KCCM 12203P activated RAW 264.7 macrophage cells without cytotoxicity at a concentration lower than 108 CFU/ml and promoted higher gene expression levels of inducible nitric oxide synthase, interleukin-1β, and interleukin-6 than L. rhamnosus GG. These results suggest that novel L. brevis KCCM 12203P could be used as a probiotic or applied to functional food processing and pharmaceutical fields for immunocompromised people.

        Clinical Course of COVID-19 in Patients with Inflammatory Bowel Disease in Korea: a KASID Multicenter Study

        In 2020, the novel coronavirus disease 2019 (COVID-19) began to spread worldwide and remains an ongoing medical challenge. This case series reports on the clinical features and characteristics of patients with inflammatory bowel disease (IBD) and confirmed COVID-19 infection. From February 2020 to March 2021, nine patients with IBD had confirmed COVID-19 across four hospitals in Korea. The median age at COVID-19 diagnosis was 42 years. Six patients were male, and seven patients had ulcerative colitis (UC). No patients required oxygen therapy, intensive care unit hospitalizations, or died. The most common symptom was fever, and gastrointestinal (GI) symptoms developed as diarrhea in five patients with UC. Oral steroids were used to combat UC aggravation in two patients. In this case series of nine IBD patients diagnosed with COVID-19 in Korea, the clinical presentation was predominately a mild respiratory tract infection. Most patients with UC developed new GI symptoms including diarrhea.

        Clinical course of ulcerative colitis patients who develop acute pancreatitis

        <P><B>AIM</B></P><P>To investigate the clinical course of ulcerative colitis (UC) patients who develop acute pancreatitis.</P><P><B>METHODS</B></P><P>We analyzed 3307 UC patients from the inflammatory bowel disease registry at Asan Medical Center from June 1989 to May 2015. The clinical course of UC patients who developed acute pancreatitis was compared with that of non-pancreatitis UC patients.</P><P><B>RESULTS</B></P><P>Among 51 patients who developed acute pancreatitis, 13 (0.40%) had autoimmune, 10 (0.30%) had aminosalicylate-induced, and 13 (1.73%) had thiopurine-induced pancreatitis. All 13 patients with autoimmune pancreatitis (AIP) had type 2 AIP. Two (15.4%) patients had pre-existing AIP, and three (23.1%) patients developed AIP and UC simultaneously. Compared to non-pancreatitis patients, AIP patients had UC diagnosed at a significantly younger age (median, 22.9 years <I>vs</I> 36.4 years; <I>P</I> = 0.001). AIP and aminosalicylate-induced pancreatitis patients had more extensive UC compared to non-pancreatitis patients. All patients with pancreatitis recovered uneventfully, and there were no recurrences. Biologics were used more frequently in aminosalicylate- and thiopurine-induced pancreatitis patients compared to non-pancreatitis patients [adjusted OR (95%CI), 5.16 (1.42-18.67) and 6.90 (1.83-25.98), respectively]. Biologic utilization rate was similar among AIP and non-pancreatitis patients [OR (95%CI), 0.84 (0.11-6.66)]. Colectomy rates for autoimmune, aminosalicylate-induced, and thiopurine-induced pancreatitis, and for non-pancreatitis patients were 15.4% (2/13), 20% (2/10), 15.4% (2/13), and 7.3% (239/3256), respectively; the rates were not significantly different after adjusting for baseline disease extent.</P><P><B>CONCLUSION</B></P><P>Pancreatitis patients show a non-significant increase in colectomy, after adjusting for baseline disease extent.</P>

        화력발전설비 위험도 평가를 통한 기기별 정비주기 예측

        위험도 평가 기술은 주로 플랜트의 많은 운영설비 중 대형사고나 피해를 유발할 수 있는 위험설비를 선별하는 목적으로 개발되었다. 설비의 위험도를 평가하여 위험도의 크기에 따라 순위를 정하고 이 순위를 기준으로 정비자원을 투입하는 순서나 정비작업의 시급성을 판단한다. 위험도란 고장이 발생할 확률과 고장이 발생할 경우에 수반되는 파손피해의 곱으로 정의된다. 위험도 평가방법으로는 간단한 손상 및 고장 평가기법을 이용하여 기본적인 고장확률 데이터를 확보하고 정성적인 문진을 통해 이를 보완하는 준 정량적 방법이 많이 사용되고 있다. 본 논문에서는 준 정략적 평가를 이용하여 석탄화력발전소의 보일러설비에 대한 위험도 평가를 수행하고 이를 기반으로 설비 별차기 정비주기를 선정하였다. Risk-based inspection (RBI) is a well-known method that is used to optimize inspection activities based on risk analysis in order to identify the high-risk components of major facilities such as power plants. RBI, when implemented and maintained properly, improves plant reliability and safety while reducing unplanned outages and repair costs. Risk is given by the product of the probability of failure (POF) and the consequence of failure (COF). A semi-quantitative method is generally used for risk assessment. Semi-quantitative risk assessment complements the low accuracy of qualitative risk assessment and the high expense and long calculation time of quantitative risk assessment. The first step of RBI is to identify important failure modes and causes in the equipment. Once these are defined, the POF and COF can be assessed for each failure. During POF and COF assessment, an effective inspection method and range can be easily found. In this paper, the calculation of the POF is improved for accurate risk assessment. A modified semi-quantitative risk assessment was carried out for boiler facilities of thermal power plants, and the next maintenance schedules for the equipment were decided.

        Viscoelastic and thermal behavior of woven hemp fiber reinforced poly(lactic acid) composites

        <P><B>Abstract</B></P> <P>This study examined the physical behavior of hemp/poly(lactic acid) (PLA) composites, particularly the thermal properties and viscoelastic behavior. Twill and plain woven hemp fabrics were used as reinforcements and hemp fabrics-reinforced PLA composites were produced using a film stacking method. The coefficient of thermal expansion of the composites decreased sharply with increasing the volume fraction of fiber. The twill structure was found to be suitable for reinforcing a PLA resin with higher impact strength and better mechanical properties than the plain woven. The viscoelastic properties of the composites including the storage modulus, loss modulus and loss tangent were also examined by dynamic mechanical analysis. In addition, morphological analysis was performed using scanning electron microscopy.</P>

      • A case of Adult-onset Cyclic Neutropenia Presenting with Arthritis

        Cyclic neutropenia is a rare disorder characterized by repetitive episodes of neutropenia every 14 to 28 days. During the neutropenic periods, patients may have fever, recurrent aphthous stomatitis, and occasionally infections. Usually, the disease begins in childhood and a genetic mutation in the elastase gene (ELANE) has been reported. However, adult-onset cyclic neutropenia is considered an autoimmune disorder in association with proliferation of cytotoxic T cells. We report here a case of 22-year old man with adult-onset cyclic neutropenia presenting with fever and polyarthritis. He had periodic neutropenia with intervals of about 21 days and his arthritis symptoms were aggravated simultaneously with the neutropenic episodes. Additionally, he had oral ulcer, abdominal pain, or fever in the neutropenic period. Bone marrow study, peripheral blood smear, peripheral lymphocyte subset analysis, and ELANE sequencing did not show any abnormal findings. Initial trial of cyclosporine had no effect on his cyclic neutropenia episodes. After regular administration of G-CSF subcutaneous injection, the duration of neutropenia was shortened and his arthritis was improved. To the best of our knowledge, this is the first case of adult-onset cyclic neutropenia in Korea.

        The Internalization of Bacillus Calmette-Guerin (BCG) in Bladder Cancer Cells May be Inhibited by Human β-defensin 3

        Purpose: To investigate whether secretion of human beta-defensin 3 (HBD-3) is induced by bacillus Calmette-Guerin (BCG) and to determine whether HBD-3 affects BCG internalization in bladder cancer cells. Materials and Methods: RTPCR analysis was used to determine whether HBD-3 mRNA increases after incubation with BCG. HBD-3 proteins in 5637 and T24 human bladder cancer cell lines were assayed by ELISA. The internalization rate was evaluated by double immunofluorescence assay and confocal microscopy to test the optimal dose of HBD-3 for BCG internalization. We also investigated the difference in internalization rates and cell viability between recombinant HBD-3 protein, anti-HBD-3 antibody, and HBD-3 plus anti-HBD-3 antibody pretreatments. Results: BCG induced HBD-3 mRNA expression and HBD-3 production dose and time-dependently in bladder cancer cells and affected BCG internalization. Pretreatment with recombinant HBD-3 protein lowered internalization of BCG dose-dependently. Moreover, anti-HBD-3 antibody prevented the effect of HBD-3 on BCG internalization in bladder cancer cells. The internalization rate of BCG pretreated with anti-HBD-3 antibody was higher than that in the control. The BCG internalization rate in cells pretreated with anti-HBD-3 antibody plus recombinant HBD-3 protein was higher than that in the control. BCG decreased bladder cancer cell viability, and anti-HBD-3 antibody prevented the inhibitory role of HBD-3 on the anti-proliferative effects of M. bovis BCG in bladder cancer cells. Conclusions: Bladder cancer cells produce HBD-3 when they are infected by BCG to defend themselves against BCG internalization, which plays an important role during the initiation and propagation of the immunotherapeutic response in bladder cancer cells.

      • Stress Analysis of HTS Magnet for a 600 kJ SMES

        <P>Auto tuning niching genetic algorithm was used to design optimal HTS magnets for the 600 kJ class SMES system under several design constraint conditions. Constraint conditions were operation loss of magnet (less than 2 W), inductance of magnet (less than 24 H), the number of double pancake coils (about 10 DPCs), the number of turns of DPC (less than 300 turns), outer diameter of DPC (close to 800 mm) and total length of HTS wire in a DPC (less than 500 m). As a result of optimum design, we obtained design parameters for the 600 kJ SMES magnet according to two operating currents, 360 A and 370 A. However, even though the HTS magnet was designed optimally in respect to the electromagnetics, consideration of mechanical integrity due to the stress by Lorentz force must not be neglected for the stable operation of the SMES system. Therefore, we developed a program, through the finite element method (FEM), for stress analysis due to Lorentz force in operation of the SMES system. In this paper, the stresses (radial and hoop stress) imposed on the designed HTS magnets were calculated by the program, and the results of stress analysis were discussed.</P>
