<P><B>Abstract</B></P> <P><B>Purpose</B></P> <P>To provide a systematic summary of total kidney volume (TKV) as an imaging biomarker in clinical trials for autosomal dominant polycystic kidney disease (ADPKD), focusing on the correlation between TKV and renal function.</P> <P><B>Methods</B></P> <P>A computerized literature search was performed using MEDLINE and EMBASE databases for studies that evaluated the correlation between TKV and the glomerular filtration rate (GFR) and between the TKV growth rate and GFR decline rate. A meta-analysis was performed to generate the summary correlation coefficient (<I>r</I>). A qualitative review was performed to evaluate the characteristics of TKV as an imaging biomarker.</P> <P><B>Results</B></P> <P>Eighteen articles including a total sample size of 2835 patients were retrieved. Meta-analysis revealed substantial correlations between TKV and GFR [<I>r</I>, −0.520; 95% confidence interval (CI), −0.60 to −0.43] and between the TKV growth rate and GFR decline rate [<I>r</I>, −0.320; 95% CI, −0.54 to −0.10]. The quantitative review revealed that baseline TKV can affect the TKV growth rate and GFR decline rate, such that patients with a higher baseline TKV showed faster TKV growth and GFR decline. There was significant variability in image acquisition and analysis methods.</P> <P><B>Conclusion</B></P> <P>There were significant negative correlations between TKV and GFR as well as between TKV growth and GFR decline rates, suggesting that TKV imaging is a useful biomarker in clinical trials. However, standardization—or at least trial-specific standardization—of image acquisition and analysis techniques is required to use TKV as a reliable biomarker.</P>

In general, the vibration problems for strength of members and serviceability of building structures are not considered in structural design process, but the prediction of the vibration is very important and essential to structural design process. Especially, it is necessary to design the structure with precise machinery such as wafer, electronic microscope. To predict the vibration response of the floor, it is necessary to know various excitation forces and dynamic characteristics of the floors. This study aims to predict the dynamic forces and vibration responses of the floor in terms of the transfer function method on the rahmen building structures. In order to know the dynamic forces and vibration responses, the modal test and the impact(impact hammer) excitation experiments were conducted several times on the floor. The results from the experiments are analyzed and compared with the measured excitation forces and vibration results. The results of this study suggest that the predicting the dynamic forces and vibration levels using transfer function method is possible to the extent of it.

Methemoglobinemia is a condition in which the iron portion of hemoglobin, which binds to oxygen, is oxidized to produce methemoglobin, which increases blood concentration. There are many causes of methemoglobinemia, the most common being food, drugs, and chemicals. A 75-year-old male patient who had taken an herbicide did not notice any nonspecific symptoms. However, after 4 hours, his methemoglobin levels increased to 17.1%, while after 7 hours it increased to 26.5%, at which time intravenous administration of methylene blue 1 mg/kg (an antidote) was started. After a total of five doses of methylene blue at 1 mg/kg due to reactive methemoglobinemia for about 36 hours, the methemoglobin levels increased to 23.7%. Because no more methylene blue could be administered, 10 g of ascorbic acid (vitamin C) was administered intravenously. After 82 hours, ascorbic acid 10 g was administered six times for repeated reactive methemoglobinemia. No additional reactive methemoglobinemia was observed. The ventilator and endotracheal tube were successfully removed on day 5 after admission.

Primary plasma cell leukemia (PCL) is a very rare form of plasma cell dyscrasia. Phagocytosis of cellular elements by malignant cells is generally considered to be a feature of monocytic-histiocytic disorders, althogh it has been reported to occur rarely in other forms of neoplasia. The patient was an 18-year old male who presented weadness, fever, nasal bleeding and hepatosplenomegaly of 1.5 months duration. Peripheral blood examination revealed severe anemia, thrombocytopenia and a normal leukocyte count with 32% plasma cells. On serum and urine protein electrophoresis, monoclonal gammopathy was found. The analysis of the abnormal gamma-globulin showed IgA and kappa type on immunoelectrophoresis. A bone marrow smear from the right anterior superior iliac spine disclosed hypercellularity with 66.1% plasma cells (48.7% immature type), Some of the plasma cells showed erythrophagocytosis. The number of other cellular elements in marrow was decreased. A partial remission was obtained through the following treatment with a combination of vincristine, cyclophosphamide, Mephalan and prednisolone, But increased plasma cells in peripheral blood, anemia and development of extramedullary plasmacytoma appeared 6 months later. We thought that this case fulfilled the primary PCL criteria.
