        한국인 직무 스트레스 측정도구의 개발 및 표준화

        Background and Purposes: Over the past three decades, numerous studies performed in Korea have reported that job stress is a determinant risk factor for chronic diseases and work disability. Every society has its own culture and occupational climate particular to their organizations, and hence experiences different occupational stress. An occupational stress measurement tool therefore needs to be developed to estimate it objectively. The purpose of this study is to develop and standardize the Korean Occupational Stress Scale (KOSS) which is considered to be unique and specific occupational stressors in Korean employees. Subjects and Methods: Data were obtained from the National Study for Development and Standardization of Occupational Stress (NSDSOS Project: 2002-2004). A total of 12,631 employees from a nationwide sample proportional to the Korean Standard Industrial Classification and the Korean Standard Occupational Classification were administered. The KOSS was developed for 2 years (2002-2004). In the first year, we collected 255 items from the most popular job stress measurement tools such as JCQ, ERI, NIOSH and OSI, and 44 items derived from the a qualitative study (depth interview). Forty-three items of KOSS, in the second year, were retained for use in the final version of the KOSS by using Delphi and factor analysis. Items were scored using conventional 1-2-3-4 Likert scores for the response categories. Results: We developed eight subscales by using factor analysis and validation process: physical environment (3 items), job demand (8 items), insufficient job control (5 items), interpersonal conflict (4 items), job insecurity (6 items), organizational system (7 items), lack of reward (6 items), and occupational climate (4 items). Together they explained 50.0% of total variance. Internal consistency alpha scores were ranged from 0.51 to 0.82. Twenty-four items of the short form of the KOSS (KOSS-SF) were also developed to estimate job stress in the work setting. Because the levels of the subscales of occupational stress were gender dependent, gender-specific standard norms for both the 43-item full version and the 24-item short form using a quartile for the subscales of KOSS were presented. Conclusion: The results of this study suggest that KOSS might be an appropriate measurement scale to estimate occupational stress of Korean employees. Further and more detailed study needs to be conducted to improve the validity of this scale.

        In vivo study for the hemostatic efficacy and foreign body reaction of a new powder-type polysaccharide hemostatic agent

        Purpose: Various hemostatic agents have been introduced in therapy as postoperative bleeding is a poor prognostic factor for postoperative outcomes. These products can be divided into those that directly promote the hemostatic cascade and those that physically form a barrier by absorbing blood. The latter, powder-type hemostatic agents have the advantages of being inexpensive and more absorbable with less foreign body reactions (FBRs) and are applicable to a relatively wide area. This study was conducted to verify the safety and efficacy of a newly invented polysaccharide product (OOZFIX, Theracion Biomedical), which improves blood absorption and hemostatic effects. Methods: Two separate animal experiments were performed. The first evaluated FBRs histologically at 3 days, 2 weeks, and 4 weeks, after implantation of OOZFIX in rats, and the second compared hemostatic performance of OOZFIX and Arista AH (Bard) in the porcine liver punch biopsy model. Results: We found minimal FBRs in the 3-day group and no reactions in both the 2-week and 4-week groups after implantation of hemostatic agents. The time to hemostasis of OOZFIX was not significantly different from that of Arista AH (median [interquartile range]: 9 [6–10] minutes vs. 8 [6–10] minutes, respectively; P = 0.522). When comparing the serial bleeding grade tendency, there was no statistical difference between OOZFIX and Arista AH (P = 0.656). Conclusion: OOZFIX caused a minimal FBR that disappeared within 2 weeks in vivo, and its hemostatic performance was comparable with that of an existing agent, Arista AH. Further clinical studies are required in the future.

        리더-구성원의 성별조합에 따른 LMX의 효과성 연구

        본 연구는 리더의 성별에 대한 리더십 연구의 한계점을 인식하고 리더·구성원의 성별조합에 따른 리더십 효과성에 대해 연구하였다. 특히 리더·구성원의 신뢰를 바탕으로 한 수평적 상호관계와 구성원의 중요성을 인식한 LUX(Leader Member Exchange)이론을 기본으로 하였다. 연구결과 첫째, 성별조합 중 남성 리더와 남성 구성원에서 LMX 효과성이 가장 높게 나타났으나 LMX 효과성의 하위요소 중 정서적 유대감, 공헌에서는 남성 리더와 여성 구성원이 가장 낮게 나타났다. 즉, 리더의 성별이 남성이라도 구성원의 성별에 따라 결과가 다르게 나타났다. 둘째, 리더·구성원의 성별조합에 따른 LMX 효과성의 차이에서는 조절변수인 조직풍토의 하위요소 중 자율성을 제외한 직위구조, 지원, 보상에서 여성 리더와의 조합일 때 높은 효과성을 지닌 것으로 나타났다. 본 연구결과에 의하면 LMX 효과성은 리더의 성별이 남성이나, 여성이냐의 차원이 아닌 구성원을 고려한 리더와 구성원의 성별조합의 차원에서 달리 나타난다는 발전적인 시사점을 주고 있다. This study was based on the recognition of the problems involved in the effectiveness research on the leadership classified according to their genders. In particular, the study employed Leader Member Exchange (LMX) theory. The research findings of the current study are as follows: First, the maximum LMX effectiveness was found between the male leader and the male subordinate. In the parts of psychological relationship and contribution among the four categories of the LMX effectiveness, the pair of the male leader and the female subordinate showed the lowest level of LMX effectiveness. Second, out of the four control variables present in the current study, three variables such as position structure, support system, compensation system were observed to be manifested in a higher level of effectiveness when the leader is female. Such research findings suggest that it is the combination of gender relationship between the leader and his or her subordinate that affects the level of effectiveness, rather than the gender itself of the leader.

        복지권으로서 교육권 보장을 위한 『장애인 등에 대한 특수교육법』

        결핍에서 오는 필요의 개념은 장애인에게 복지권으로서 교육받을 권리를 가장 잘 말해주고 있다. 장애인은 신체적·인지적 손상으로 발생하는 기본적인 생존적 필요의 충족뿐만 아니라 동시에 교육기회 균등이나 개인차의 고려 등을 통해 무지로부터 벗어날 수 있는 보편적 필요가 충족되어야 함을 논의하였다. 장애인들에게 이러한 결핍에 따른 필요를 충족시켜 줄 이론적 근거가 롤즈(J. Rawls)의 정의론이라 할 수 있다. 정의론의 '차등의 원칙'에 따르면, 교육에 있어 비장애인과 장애인 중 먼저 최소 수혜자인 장애인의 교육복지를 우선하여 극대화할 필요성을 제시함으로써, 그들의 교육권 보장을 위한 이론적 근거에 대한 정당화를 논의하였다. 기존의 특수교육진흥법은 '국가가 교육할 권리'를 가지는 국가 주도적 교육이었다면, 「장애인 등에 대한 특수교육법」 제정은 수년간 장애인의 교육권 확보를 위해 애쓴 장애인 교육 주체들이 노력 끝에 '교육받을 권리'를 찾게 된 의미 있는 결실로 평가되어 진다. The concept of need which comes from lack represents well the right to education as welfare rights to individuals with disabilities. It is necessary to meet the universal need of individuals with disabilities such as an equal opportunity for education and the consideration for individual difference as well as their substantial need. The rationale which may satisfy the need associated with the lack can be J. Rawls's a Theory of Justice. The difference principle by Rawls presents the need of the educational welfare of individuals with disabilities(the least advantaged) to take precedence over that of the non-disabled and be maximized, it is considered that he created the rationale that makes secure their right to education. While established Special Education Promotion Law was national-driven education that state had to the education right, the enactment of 'the Special Education Law for the Individuals with Disabilities, etc.' can be a significant fruit which takes back 'the right to education by citizens' by the educational subjects of the individuals with disability who have taken pains to secure their right to education for years.

      • 백혈병 세포에서 Multidrug Resistance Gene-1 (mdr1)의 과발현이 ^99m Tc-sestaMIBI 섭취에 미치는 영향

        Purpose: To determine whether 99mTc-MIBI is recognized by the multidrug resistant P-glycoprotein (Pgp), we have measured quantitatively 99mTc-MIBI uptake in cancer cells. The effects of various Pgp reversing agents on cellular 99mTc-MIBI uptake were also investigated in the presence of multidrug resistance gene-1(mdr1 gene) overexpression. Materisls and Methods: We measured percentage uptake of 99mTc-MIBI at different incubation temperatures both in mdr1 positive and negative cells. The effects of verapamil, cyclosporin, and dipyridamole on cellular uptake of 99mTc-MIBI were also evaluated with or withouts overexpression of mdr1 gene in Cultured murine leukemia L1210 cells. Results: The mdr1 gene expressing cell lines were effectively induced in in vitro with continuous application of low-dose adriamycin or vincristine. Cellular uptake of 99mTc-MIBI was higher in mdr1 negative L 1210 cells than those of mdr1 positive cells, and higher when incubated in 37℃ than 4℃. In the presence of verapamil, cyclosporin or dipyridamole, 99mTc-MIBI uptake was increased upto 604% in mdr1 positive cells. Conclusion: Cellular uptake of 99mTc-MIBI is lower in leukemia cells over-expressing mdr1 gene, and MDR-reversing agents increase cellular uptake. These results suggest that 99mTc-MIBI can be used for characterizing Pgp expression and developing MDR-reversing agents in vitro. (Korean J Nucl Med 1999;33: 152-62)

        피로와 의료이용, 업무상 사고 및 질병결근

        Objectives: This study examined the relationship of fatigue to medical utilization, occupational accident and sickness absence. Subjects and Methods: Data were obtained from the National Study for Development and Standardization of Occupational Stress (NSDSOS Project, 2002~2004). A total of 30,146 employees categorized within the Korean Standard Industrial Classification and the Korean Standard Occupational Classification were recruited. A structured-questionnaire was used to assess the participants' sociodemo-graphics, job-related factors, health-related behaviors, personality trait, self=perceived fatigue (MFS) and the frequency of medical utilization, occupational accident and sickness absence. Data on medical utilization and occupational accident were confirmed from company health records. Chi-square test and logistic regression analysis were used to elucidate the relationship of self-perceived fatigue to medical utilization, occupational accident and sickness absence. Results: In logistic regression analyses, self-perceived fatigue was associated with medical utilization (outpatient), occupational accident and sickness absence although the strength of the associations was higher in the blue collar workers except for medical utilization. For medical utilization, the white collar worker's adjusted odds ratios of outpatient utilization for workers scoring in the third quartile and the highest quartile, compared to workers scoring in the lowest quartile were 1.50 (95% CI: 1.20-1.87) and 2.16 (95% CI: 1.69-2.75), respectively. The blue collar workers' adjusted odds ratios of outpatient utilization for workers scoring in the second (OR: 1.23, 95% CI: 1.06-1.43), third (OR: 1.42, 95% CI: 1.22-1.66) and highest quartile (OR:1.83, 95% CI: 1.55-2.17), respectively, compared to workers scoring in the lowest quartile were statistically significant. However, no associations were found between fatigue and inpatient utilization for either the white or blue collar workers. The blue collar workers' adjusted odds ratios of being injured for workers scoring in the second, third and highest quartile were 1.41 (95% CI: 1.00-2.00), 1.57 (95% CI: 1.10-2.22) and 2.41 (95% CI: 1.69-3.44), respectively, compared to those scoring in the lowest quartile, but, those of the white collar workers were not significant. There was more likely to an increased risk of sickness absence in workers scoring in the second, third and highest quartile compared to those scoring in the lowest quartile in both white and blue collar workers, but the magnitudes of risk were higher in the blue collar workers than the white collars workers. Conclusion: This result suggests that fatigue is a determinant predictor of medical utilization, occupational accident and sickness absence, and that the pattern of risks for the three outcomes differs according to the occupational type. Some limitations of this study and considerations for future study were also discussed.

      • 太谿穴의 臨床 活用에 대한 文獻考察

        Literally, the mean of Tae-gye is highest brook. And on the basis of the acupuncture theory, Tae-gye point is the Soo earth point and source point of the Kidney Channel as well. In addition, Tae-gye point is one of the Yang-Returning Nine points. We were trying to study bibliographically on the Tae-gye point because Tae-gye point can be used very broadly across the symptoms in the theoretical aspect described above. As a results, we found out that according to the classical books of acupuncture, Tae-gye point is entering point of the Meridian Water as a source point and can be used at both Kidney-Sufficient Syndrom and Kidney-Deficient Syndrom. And Tae-gye can be applied to the disease of kidney or bladder that is urogenital symtoms, Also, Tae-gye is an useful option of tooth-ache, asthma, indigestion, constipation, edema etc whose cause is related with decrease of Kidney ki.

        여성 노인의 건강인식에 관한 질적 연구

        이 연구는 실증주의적이고 계량적 연구방법에 치우쳐 온 건강과학분야에서 노년기 건강에 관한 일반적 연구방법의 한계를 보완하고자 시도되었다. 반-구조화된 질문과 심층면접을 통해 생애단계별로 수집된 구술 자료를 분석하여, 여성 노인들이 자신의 건강을 어떻게 평가하고 있으며 현재의 건강을 결정하는데 중요하다고 인식하는 사건이나 요인은 무엇인지 살펴보고자 하였다. 대상은 기준표본방법(criterion sampling)에 의거해 서울거주 67-79세 여성 노인 12명을 모집하였고, 기초생활수급자 및 비수급자를 비례적으로 포함하였다. 분석은 colaizzi의 현상학적 분석 방법을 사용하였다. 연구에 참여한 여성 노인들은 ‘젊어서 고생’, ‘자식 사망’이나 ‘사업 실패’와 같은 충격적 사건이나 ‘화병’, ‘유산, 출산, 산후조리와 같은 여성특이적 경험’, ‘어릴적 영양상태’와 같이 “생애사유인(誘因)적”으로 건강과 질병의 원인을 찾는 경향이 있었다. 더불어 질병이나 노화를 필연적 인생과정으로 인식하거나, 타고난 체질로 받아들이는 “인생순응(順應)적” 범주도 나타났다. 여성 노인들에게 현재의 건강과 질병은 의학적 진단명이나 임상적 수치로만 표현할 수 없는, 일생동안 축적된 슬픔과 한의 간접적 표현이기도 하고, 사회적 관계의 건전성과 관련된 포괄적 삶의 질 지표이기도 하다. 노인의 입장에서 건강의 의미를 찾고 문제를 검토하는 다양한 연구가 필요하다. This phenomenological study describes how older women evaluate their health and what they perceive attributes to their current health status. Despite recent growing recognition of the need for qualitative methods, there has been little literature on the health of older Korean women from their own subjective perspective. In an effort to fill this gap, we conducted semi-structured, face-to-face in-depth interviews with 12 women aged 67 to 79, residing in Seoul. Each woman was interviewed for 50 to 120 minutes regarding her life course and health perception. We found that, in the case of older females, health and illness might be an expression of accumulated sadness/anger and that their perceptions of health were influenced by life trajectory events according to ‘her lived experience’. Moreover, an individual’s health status in old age seemed to reflect the overall quality of life, particularly in regards to the soundness of social relations. This analysis, following Colaizzi's method, revealed such ‘life experiences’ categories as ‘overwork in younger ages,’ ‘death of a child,’ ‘business failure,’ ‘abortion or post-partum care’ and ‘wha-byung (culturally bounded mental illness due to suppressed anger).’ At the same time, the women interviewed seemed to accept diseases and frailty as ‘part of being old,’ or a ‘natural process.’ For a deeper understanding of health and the meaning of health in later life, further studies should take into consideration, and explore in more details, not only what is answered, but the unique perspective, from which it is said.

        보건관리 대행사업장 근로자에서 건강증진에 대한 5년 사이의 지식, 태도 및 실천의 변화

        Before and after conducting the health promotion program by group occupational health service during 5 years, we surveyed knowledge, attitude and practice about general health (30 items) and occupational health(30 items) among 25 small and medium scale industry workers(before: 355 workers, after: 279 workers) for evaluation of effectiveness. The scores of knowledge for smoking, drinking, cholesterol, mental stress, management of hypertension, and risks in younger or women workers were significantly increased after conducting group occupational health service. The scores of attitude for controlling of fatty food consumption and cleaning workplace and bathing for health and that of practice for taking the periodic health examination and checking blood pressure were significantly increased but most of items in attitude and practice didn't be changed after services. According to sex, the scores of men were significantly increased in knowledge of general health, but there were not significant differences in age, job status and work duration group. In conclusion, through the group occupational health service during 5 years, only the scores of knowledge in general health were increased. It suggested that the effective program which can change workers' attitude and practice for health promotion, should be developed and conducted in small and medium scale industry workers.
