Palmoplantar keratodermas (PPKs) are characterized by a thickening of the skin on the palms and soles of the feet caused by excessive keratin. Four forms can be seen, including diffuse, focal, punctate, and palmoplantar ectodermal dysplasias. Idiopathic, acquired PPKs should be reserved as a diagnosis of exclusion, when all the other etiologies have been investigated and ruled out. Herein, we report four cases of acquired punctate palmoplantar keratoderma with literature reviews. (Korean J Dermatol 2018;56(2):126∼129)
한국인 건선환자 30명을 대상으로 5년동안 Ustekinumab과 Topical Agent 병용치료의 효과에 대한 후향적 연구

Background: Psoriasis is a chronic inflammatory skin disorder affecting approximately 1∼3% of the general population. Ustekinumab is a recently developed human monoclonal antibody for psoriasis that binds to the p40 subunit shared by the interleukins IL-12 and IL-23. Objective: The purpose of this study was to evaluate the effect of combination treatment with ustekinumab and topical agents in 30 Korean patients with psoriasis regarding different clinical parameters. Methods: We retrospectively searched to identify patients with moderate-to-severe plaque-type psoriasis who had initiated treatment with ustekinumab between January 2012 and January 2016. Among them, our study was conducted in 30 patients with psoriasis who were treated with ustekinumab and topical agents for at least 16 weeks by analyzing their clinical charts and photographs. Results: Overall, 16.7%, 93.3%, and 96.2% patients achieved PASI 75 response rates at weeks 4, 16, and 40, respectively. Furthermore, fifteen patients achieved 90% improvement in their PASI score at 100 weeks and five patients maintained their PASI score at 160 weeks. The efficacy of treatment with ustekinumab was different in sub-group analysis. Non-smokers enjoyed a higher therapeutic effect than did smokers. In addition, the therapeutic effect of ustekinumab was lower in the groups with psoriatic arthritis and nail psoriasis. However, it was not statistically significant. None of the patients experienced serious adverse events requiring the interruption of treatment. Conclusion: Combination treatment with ustekinumab and topical agents provides effective treatment results for Korean patients with psoriasis. (Korean J Dermatol 2017;55(3):171∼177)

<P>As a guide for selective reactions toward either Z- or E-alkene in a metathesis reaction, the relative preference of metathesis Ru catalysts for each stereoisomer was determined by a method using time-dependent fluorescence quenching. We found that Ru-1 prefers the Z-isomer over the E-isomer, whereas Ru-2 prefers the E-isomer over the Z-isomer. The Z/E-alkene preference of the catalysts precisely predicted the Z/E isomeric selectivity in the metathesis reactions of diene substrates possessing combinations of Z/E-alkenes. For the diene substrates, the rate order of the reactions using Ru-1 was Z,Z-1,6-diene > Z,E-1,6-diene > E,E-1,6-diene, while the completely opposite order of E,E-1,6-diene > Z,E-1,6-diene > Z,Z-1,6-diene was exhibited in the case of Ru-2.</P>

Since the durability evaluation for improving the durability of PEMFC polymer membranes is very important for the development of PEMFC, research and development of the polymer membrane durability evaluation protocol (AST) continues. Recently, DOE's polymer membrane chemical/mechanical durability evaluation AST was developed and applied to Nafion XL for review. In order to shorten the evaluation time, oxygen was used as a cathode gas instead of air, and it was finished in 144 hours. Since DOE AST has a large number of voltage changes with 45 seconds of humidification and 30 seconds of drying, the degradation of the electrode has more influence on the MEA durability. Therefore, one cycle time was lengthened with 60sec of wet/300sec of dry, and the drying time was made longer than the humidification time to further deteriorate the polymer membrane, and it was finished in 240 hours. It was confirmed that the DOE AST for evaluation of the durability of the polymer membrane was accompanied by electrode degradation.
Crystallization of Local Anesthetics When Mixed With Corticosteroid Solutions

Objective To evaluate at which pH level various local anesthetics precipitate, and to confirm which combination of corticosteroid and local anesthetic crystallizes.Methods Each of ropivacaine-HCl, bupivacaine-HCl, and lidocaine-HCl was mixed with 4 different concentrations of NaOH solutions. Also, each of the three local anesthetics was mixed with the same volume of 3 corticosteroid solutions (triamcinolone acetonide, dexamethasone sodium phosphate, and betamethasone sodium phosphate). Precipitation of the local anesthetics (or not) was observed, by the naked eye and by microscope. The pH of each solution and the size of the precipitated crystal were measured.Results Alkalinized with NaOH to a certain value of pH, local anesthetics precipitated (ropivacaine pH 6.9, bupivacaine pH 7.7, and lidocaine pH 12.9). Precipitation was observed as a cloudy appearance by the naked eye and as the aggregation of small particles (<10 m) by microscope. The amount of particles and aggregation increased with increased pH. Mixed with betamethasone sodium phosphate, ropivacaine was precipitated in the form of numerous large crystals (>300 m, pH 7.5). Ropivacaine with dexamethasone sodium phosphate also precipitated, but it was only observable by microscope (a few crystals of 10–100 m, pH 7.0). Bupivacaine with betamethasone sodium phosphate formed precipitates of non-aggregated smaller particles (<10 m, pH 7.7). Lidocaine mixed with corticosteroids did not precipitate.Conclusion Ropivacaine and bupivacaine can precipitate by alkalinization at a physiological pH, and therefore also produce crystals at a physiological pH when they are mixed with betamethasone sodium phosphate. Thus, the potential risk should be noted for their use in interventions, such as epidural steroid injections.
Lobular capillary hemangioma after automicroneedle therapy system in an oriental medicine clinic

Lobular capillary hemangioma(LCH) is a common benign vascular tumor presenting as a rapidly growing angiomatous papule. Trauma and underlying microscopic arteriovenous malformations have been implicated in LCH. Automicroneedle therapy system(AMTS), usually used to treat acne scars and wrinkles, improves transdermal delivery making multiple mechanical holes into the skin. In these days, AMTS is currently being used for cosmetic purposes mainly in oriental medicine clinics. We present a case of 41-year-old woman who had two erythematous papules on the preexisting infantile hemangioma site of the neck for several months. She had history of AMTS procedure on the lesion in an oriental medicine clinic several months ago. Histological examination demonstrated granulation tissue, mainly composed of proliferative capillaries, in the papillary dermis. The patient is scheduled for the pulsed dye laser treatment of the remaining infantile hemangioma. With the rising trends of herbal procedures conducted without accurate knowledgements regarding cutaneous lesions, dermatologists need to understand the current status of oriental medicine clinics and caution the public against indiscreet procedures.
비선형 프로그래밍에 의한 최소시간 삼차 스플라인 경로계획

Minimum-time trajectory planning is considered in this paper under the assumption of bounded actuator forces, fixed via points and cubic spline curves passing the points. The trajectory planning is formulated as a constrained optimization problem and a nonlinear programming technique is applied to derive a solution, in which the objective function is the total traveling time and the constraint is the actuator torque bounds. Application example for a SCARA-type robot is given.

In order to improve the PEMFC (Proton Exchange Membrane Fuel Cell) durability, it is important to accurately evaluate the durability of the membrane in a short time. Recently, DOE (Department of Energy) reported a protocol that combines the chemical and mechanical durability of membranes to evaluate them effectively. This protocol applies chemical/mechanical deterioration to the membrane by repeating wet/dry while OCV (Open Circuit Voltage) holding. The problem of this protocol is that it is highly affected by electrode degradation due to change cycles in OCV and that the evaluation time is long. By using oxygen instead of air as the cathode gas while leaving the other conditions of the DOE protocol as it is, the durability evaluation time could be reduced from 408 hours to 144 hours. By reducing the number of voltage change cycles to 1/3, the electrode degradation due to the voltage change cycle was reduced to 1/12 when oxygen was used compared to air at the end, thereby enabling more accurate evaluation of polymer membrane durability.
