Barriers to Pain Management among Older Adults in Rural Areas

This study explored health or senior care service providers’views of barriers to optimal pain management among chronically ill older adults in rural communities in the United States. An exploratory, qualitative research design was used to capture service providers’ perceptions of barriers through individual, face-to-face interviews. Thematic analysis of the interview data revealed 5barriers: lack of transportation, lack of alternative pain treatments, difficulties with assessment of pain, hesitance to pain medicine, and lack of trained professionals. Findings of this study suggest need for increased resources at federal and state level and more education on pain and its management.

<P>The accumulation of extracellular matrix proteins in the interstitial area is the final common feature of chronic kidney diseases. Accumulating evidence suggests that transforming growth factor (TGF)-관1 promotes the development of renal fibrosis. Heat shock protein (Hsp) 90 inhibitors have been shown to repress TGF-관1 signaling, but whether they inhibit renal fibrosis is unknown. The purpose of this study is to determine the therapeutic efficacy of Hsp90 inhibitor on renal fibrosis. In TGF-관1-treated HK2 cells and unilateral ureteral obstruction (UUO) kidneys, we found that 17-allylamino-17-demethoxygeldanamycin (17AAG), an Hsp90 inhibitor, decreased the expression of 관-smooth muscle actin, fibronectin, and collagen I and largely restored the expression of E-cadherin. 17AAG inhibited TGF-관1-mediated phosphorylation of Smad2, Akt, glycogen synthase kinase-3관, and extracellular signal-regulated kinase in HK2 cells. Inhibition of Hsp90 also blocked TGF-관1-mediated induction of snail1. This 17AAG-induced reduction was completely restored by simultaneous treatment with proteasome inhibitor MG132. Furthermore, 17AAG blocked the interaction between Hsp90 and TGF-관 type II receptor (T관RII) and promoted ubiquitination of T관RII, leading to the decreased availability of T관RII. Smurf2-specific siRNA reversed the ability of 17AAG to inhibit TGF-관1 signaling. The effect of 17AAG on T관RII expression and renal fibrosis was confirmed in UUO kidneys. These findings suggest that Hsp90 inhibitor prevents the development of renal fibrosis via a mechanism dependent on Smurf2-mediated degradation of T관RII.</P>

임산부에서 발생하는 요로 감염은 적절히 치료되지 않을 경우 감염으로 인한 합병증과 함께 자궁의 조기 수축을 유발할 수 있어 적극적인 치료를 요한다. 단순 신낭종의 감염은 흔히 항생제 단독 치료에 잘 반응하지 않고 수술적 혹은 경피적 배액을 필요로 하는 것으로 알려져 있으며, 임산부에서 발생한 예는 국내외적으로 보고가 매우 드물다. 본 저자는 임신 28주 산모에서 발생한 신낭종 감염을 경피적 배액과 항생제 투여로 성공적으로 치료한 1예를 경험하였기에 보고한다.
High performance carbon nanotubes thin film transistors by selective ferric chloride doping

Single wall carbon nanotubes (SWNT) have been a significant research topic as active layers for thin film transistors (TFTs) due to their high charge carrier mobility beyond that of crystalline silicon. In this study, we report an effective approach to achieve a very high field-effect mobility and on/off ratio for solution processed semiconducting SWNT TFTs, by selective doping through contact with a thin ferric chloride (FeCl3) dopant layer. The semiconducting layer is formed by a double spin coating of the highly purified (>99%) high pressure carbon mono oxide (HiPCO) SWNT sorted by wrapping of poly (3-dodecylthiophene-2,5-diyl) (P3DDT). In order to achieve effective hole injection from the top Au source electrode without increasing the off-state drain current, less purified (98-99%) SWNTs produced by the plasma discharge process sorted by wrapping of poly (9,9-di-n-dodecylfluorene) (PFDD) are formed on the top of HiPCO film. Significantly improved TFT performance is achieved by the insertion of a few nanometers of a FeCl3 dopant layer at the semiconductor-contact interface. A significant high hole field-effect of 48.35±3.11 cm2V−1s−1 (bare: 6.18±0.87 cm2V−1s−1) with a reasonable on/off current ratio of 105, and low off current of ∼80 pA, are obtained by controlling the concentration of FeCl3 dopant (thickness = 1.5 nm) at the contact. Mobility is improved further at 2.5nm thickness of the FeCl3 dopant layer resulting in a hole mobility of 177±13.2 cm2 V−1s−1, an on/off ratio of 7.4×103, and off state current of 1.2×10−9 A.
HD 영상의 실시간 얼굴 검출을 위한 LBP 연산의 하드웨어 설계

디지털 도어락 디지털 카메라 비디오 감시 시스템 등에서 사용되는 기존의 얼굴 검출 시스템은 비교적 낮은 해상도의 영상 처리를 사용하고 있으며 이를 위해서 소프트웨어 기반의 구현을 하고 있다. 하지만 이 경우에는 높은 해상도를 위한 얼굴 검출이 어려울 뿐만 아니라 수행해야할 영산 처리 양이 많기 때문에 실시간으로 얼굴을 검출하는데 어려움이 있다. 실시간 임 베디드 시스템에서 HD(High Definition) 영상을 위한 효과적인 얼굴 검출을 위해서는 하드웨어적인 접근이 필요하다. 이에 본 논문에서는 얼굴 검출을 위해 사용되는 전처리 과정 중에 하나이며 처리시간이 많이 소요되는 국부 이진 패턴(LBP Local Binary Pattern) 연산을 하드웨어 구조를 제시하고 설계했다. 그리고 제시한 하드웨어 구조를 FPGA(Field Programmable Gate Array) 칩을 통해서 구현 및 검증을 통해 고해상도 HD급 영상에서 효율적인 얼굴 검출이 가능 한 것을 확인했다. Existing face detection systems which are used for digital door locks digital cameras video surveillance systems and so on are software-based implementation for relatively low level resolution images. Therefore in this case there are difficulties in detecting faces in a real-time fashion due to the increasing amount of operational processing as well as in allowing the requirements of face detections for HD(High Definition) resolutions. A hardware approach is necessary to efficiently find faces for HD images in real-time embedded systems. This paper proposes and implements a hardware architecture for an LBP(Local Binary Pattern) operation which is a time-consuming part as one of preprocessing steps for face detection. The hardware architecture proposed in this research has been implemented and tested with a FPGA(Field Programmable Gate Array) chip and shown that the approach guarantees efficient face detection for HD images.
미국 내 가족 및 비공식 돌봄제공자 지원 제도: 지역사회거주관리국(ACL) 사례를 중심으로

미국에서 현재 비공식적으로 성인이나 아동을 돌보고 있는 인구는 4000만 명을 넘어서며, 미국 인구의 고령화 현상이나 알츠하이머병 및 관련 치매를 포함한 만성 중증 질환이 꾸준히 증가하는 추세를 감안할 때 향후 이 수치는 더욱 늘어날 것으로 전망된다. 미국 내 가족 및 무급 비공식 돌봄자들에 대한 지원 노력은 연방 기관인 지역사회거주관리국(ACL: Administration for Community Living)의 주도로 이루어져 왔다. ACL이 자금을 대는 주요 돌봄 지원 사업으로는 전미 돌봄 가족 지원 프로그램, 생애주기 휴식 지원 프로그램, 발달장애 우수 대학센터가 있고, ACL 산하의 돌봄 가족과 손자녀 돌봄 조부모를 위한 자문위원회에서 보건복지부에 관련 권고 사항을 전달한다. 이 글에서는 이들 프로그램의 세부 내용을 살펴보고 돌봄자들에게 미치는 영향과 향후 방향을 논의하고자 한다.
