상부소환관협착에 대한 Savary-Gilliard Dilatation의 치료효과와 안전성에 관한 관찰

For the patient with stenosis of upper digestive tract caused by either benign or malignant process, esophageal dilatation is an important therapeutic modality. We reviewed retrospectively 39 cases treated by Savary-Gilliard dilatation for upper digestive tract stenosis, and results were follows : (1) Dysphagia was improved in 95% of the patients. (2) Eight patients(20%) sufferd perforation, five of them were managed with conservative medical measures, others were managed operatively. (3) In the cases with benign stenosis dysphagia was not noticed for 8.8 months, but with malignant stenosis dysphagia was reappeared after 2.8 months despite concomitant chemoradiotherapy. In conclusion, Savary-Gilliard dilatation was an effective measure for symptome due to upper digestive tract stenosis, but more careful attention for perforation should be necessitated. Also another therapeutic modality for treatment of malignant stenosis was inevitable.

A clinical analysis was performed of 3055 pateints with peptic ulcer who were diagnosed with gastroduodenoscopy, in Hospital of Chungnam National University College of Medicine from July 1988 to May 1993. 1) During the period 1988-1993, The proportion of gastric ulcer among peptic ulcer decreased from 47.8% to 42.8%, but that of duodenal ulcer increased from 36.8% to 39.6%. 2) Of the 3055 cases, the number of patients with esophageal ulcer was 98(3.2%), with gastric ulcer 1407(41.6%), with. duodenal ulcer 1104(36.1%), with channel ulcer 149(4.9%), and with combind ulcer 297(9.7%). 3) We observed a peak incidence of peptic ulcer in the 50-69age group, gastric ulcer in the 5069age, duodenal ulcer in the 40-59age group. The ratio of male to female was 3.6 : 1 in peptic ulcer, 4.4 : 1 in gastric ulcer, 3.0 : 1 in duodenal ulcer. 4) The common site was angle in gastric ulcer, and bulb in duodenal ulcer. 5) The active stage was 39.4% of gastric ulcer, and 37.4% of duodenal ulcer, the healing stage was 34.2% of gastric ulcer, and 17.9% of duodenal ulcer, and the scar stage of gastric ulcer was 26.4%, and duodenal ulcer was 44.7%. 6) The size of ulcer was less than 1Cm in 66.7% of gastric ulcer, and in 75.5% of duodenal ulcer, the size of ulcer was greater than 2Cm in 13.2% of gastric ulcer, and in 6.5% of duodenal ulcer. 7) The frequency of multiple ulcer was 29.9% in gastric ulcer, and 16.5% in duodenal ulcer.

This study attempted to determine the degree of asbestos release from the aging slate roof by comparing the asbestos content in the slate roof rain gutter with a colored steel plate (tin plate) over the slate roof. Four slate roof houses located in Haengmok-ri, Asan-si, Chungcheongnam-do, and one house constructed with a colored steel plate on the slate roof were selected to collect the sediment of the roof rain gutter. The asbestos fiber content was calculated by a point counting method using a polarization microscope after pretreatment with conversion treatment and hydrochloric acid treatment. The average asbestos content of the four slate roof rain gutter were 1.89%. However, asbestos was not detected in the Slate covering roof rain gutter, which were constructed on the slate roof. Asbestos fiber content was the highest at 2.89% in the slate roof rain gutter installed in 1976, followed by 2.44% in 1953. From the above results, it is necessary to minimize secondary damage as asbestos fibers released from slate roof houses to the surrounding atmosphere or leaked from slate roofs as rainwater may cause soil pollution and seriously affect residents' health. Although covering with colored steel plates (tin plates) has been shown to prevent the leakage of asbestos fibers to some extent, it is believed that a policy alternative to remove the slate roof as soon as possible is needed to solve the fundamental problem.
Rapid Detection of Listeria monocytogenes by Real-Time PCR in Processed Meat and Dairy Products

<P>The objectives of this study were to evaluate the detection of Listeria monocytogenes in different ready-to-eat foods using real-time PCR (RT-PCR). Various concentrations (100 to 105 CFU/ml) of L. monocytogenes ATCC 19115 were inoculated into ham, sausage, ground meat, processed milk, cheese, and infant formula. L. monocytogenes ATCC 19115 in the samples was then enumerated on Oxford agar, and DNA was extracted from the samples before and after incubation at 36°C for 4 h. A set of primers and hybridization probe designed in this study was then used to detect the pathogen. The standard curve was then prepared by plotting cycle threshold values for each dilution versus L. monocytogenes cell counts (log CFU). The specificity of the set of primers and hybridization probe was appropriate. A 4-h incubation at 36°C before DNA extraction produced optimum standard curves in comparison to the results for a 0-h incubation. Thus, a 4-h incubation at 36°C was applied for monitoring L. monocytogenes in collected food samples. To monitor L. monocytogenes in foods, 533 samples (ham, 129; sausage, 226; ground meat, 72; processed cheese, 54; processed milk, 42; and infant formula, 10) were collected from retail markets and from the step before pasteurization in plants. Of all 533 samples, 4 samples (0.8%) showed positive signals in RT-PCR. Two samples from hams (1.6%) and two samples from sausages (0.9%) were determined to be positive for L. monocytogenes at <100 CFU/g. The results indicate that the RT-PCR detection method with the set of primers and hybridization probe designed in this study should be useful in monitoring for L. monocytogenes in processed meat and milk products.</P>

The identity of 45 Hanwo and 47 imported beef (non-Hanwoo) samples from USA and Australia were verified using the microsatellite (MS) marker and single nucleotide polymorphism (SNP) methods. Samples were collected from 19 supermarkets located in the city of Seoul and Gyeonggi province, South Korea, from 2009 to 2011. As a result, we obtained a 100% concordance rate between the MS and SNP methods for identifying Hanwoo and non-Hanwoo beef. The MS method presented a 95% higher individual discriminating value for Hanwoo (97.8%) than for non-Hanwoo (61.7%) beef. For further comparison of the MS and SNP methods, blood samples were collected and tested from 54 Hanwoo ${\times}$ Holstein crossbred cattle (first, second, and third generations). By using the SNP and MS methods, we correctly identified all of the first-generation crossbred cattle as non-Hanwoo; in addition, among the second and third generation crossbreds, the ratio identified as Hanwoo was 20% and 10%, respectively. The MS method used in our study provides more information, but requires sophisticated techniques during each experimental process. By contrast, the SNP method is simple and has a lower error rate. Our results suggest that the MS and SNP methods are useful for discriminating Hanwoo from non-Hanwoo breeds.
