<P><B>Abstract</B></P> <P>Smoking is an acknowledged risk factor for vascular disorders, and vascular complication is a main outcome of diabetes. Hence, we investigated the impact of cigarette smoke on blood vessels in diabetes, postulating that smoking might aggravate diabetic vascular impairment. Sprague–Dawley rats were divided into four groups: control, cigarette smoke-exposed, diabetic, and cigarette smoke-exposed diabetic groups. Streptozotocin-induced diabetic rats were exposed to cigarette smoke by inhalation at total particulate matter concentration of 200 μg/L for 4 h/day, 5 day/week for a total of 4 weeks. Diabetes caused structural change of aorta, but additional cigarette smoke exposure did not induce further alteration. Collagen, a marker for fibrosis, was increased in media of diabetic aorta, and this increase was augmented by cigarette smoke. Cigarette smoke induced endothelial nitric oxide synthase (eNOS) uncoupling in the diabetic group. Malondialdehyde was increased and glutathione was decreased in blood from diabetes, but these effects were not exaggerated by cigarette smoke. Cigarette smoke caused NADPH oxidase (NOX) 2 expression in diabetic aorta and enhanced diabetes-induced NOX4 expression in aorta. Taken together, cigarette smoke exposure can aggravate vascular fibrosis and induce eNOS uncoupling in diabetes under experimental condition, suggesting that smoking might exacerbate diabetic vascular impairments.</P> <P><B>Highlights</B></P> <P> <UL> <LI> Impact of cigarette smoke on the progress of diabetic vascular impairment was investigated. </LI> <LI> Diabetes-induced structural change of aorta was minimally affected by cigarette smoke exposure. </LI> <LI> Cigarette smoke induced endothelial nitric oxide synthase (eNOS) uncoupling in diabetic aortas. </LI> <LI> Cigarette smoke further enhanced diabetes-caused collagen deposition in aorta. </LI> <LI> Cigarette smoke exposure induced NADPH oxidase (NOX) 2 and increased NOX4 expression in diabetic aorta. </LI> </UL> </P>
경피적 내시경적 괴사 제거술에서 완전 피복 식도 금속 스텐트의 유용성

괴사성 췌장염은 사망률이 높고 괴사된 췌장 부위에 감염이 동반되어 있을 경우에 패혈증과 다발성 장기 부전이 발생하여 사망률이 더욱 증가하는 중대한 질환으로 과거에는 수술적 괴사 제거술이 많이 시행되었지만 최근에는 경피적 배액술 또는 내시경적 괴사 제거술과 같은 최소 침습 시술이 치료에 널리 이용되고 있다. 저자들은 경피적 배액술과 내시경적 배액술을 시행하였으나 치료 반응이 없고 임상적으로 악화 경과를 보이는 환자에서 직경이 넓은 식도 금속 스텐트를 사용하여 경피적 내시경적 괴사 제거술을 시행하여 대장주변(paracolic gutter)에 있는 막으로 둘러싸인 괴사를 성공적으로 치료한 사례들을 경험하였고 식도 금속 스텐트를 사용하여 경피적 내시경적 괴사 제거술이 시행된 증례는 국내에서 이전에 보고된 바가 없기에 이를 문헌 고찰과 함께 보고하는 바이다. Necrotizing pancreatitis is associated with high rates of morbidity and mortality. Managing necrotizing pancreatitis is challenging, and minimally invasive treatment modalities recently replaced traditional open necrosectomy. Percutaneous catheter drainage and endoscopic necrosectomy are now widely used because they are less invasive, safer, and can more effectively remove necrotic materials. Various methods and novel techniques have been introduced to manage walled-off necrosis. Herein, we report a case series of patients with necrotizing pancreatitis who were successfully treated using fully covered esophageal metal stent and endoscopic necrosectomy via the percutaneous approach. Percutaneous endoscopic necrosectomy using a fully covered esophageal stent is an effective endoscopic treatment for patients with walled-off necrosis that extends along both sides of the paracolic gutter, reduces the number of necrosectomy sessions, and improves disease status faster than conventional treatment. Half-Deck을 포함한 60 m 경간 PSC 거더의 단면 및 텐던 프로파일 최적화 연구

본 연구에서는 60 m 급의 장경간 PSC 거더의 개발을 위하여 단면 개선부터 텐던 배치 형상까지 구조적인 성능뿐만 아니라 경제성과 시공성 등을 고려해가며 거더 개발에 관한 연구를 진행하였다. 단면 최적화를 통해 Bulb-T 형의 단면을 도출하였고 단면 평가를 통해 실제 설계 가능성을 평가하였다. 또한 텐던의 배치를 효율적으로 구성하여 사용하중에 대해 효과적으로 대처할 수 있도록 하였다. 실제 대상교량을 선정한 후 개발 거더를 적용해 유한요소해석을 수행한 결과 모든 하중단계에서 설계 허용응력을 만족하였다. 또한 사용하중에 의해 발생하는 응력이 허용하중보다 작아 안전측 설계가 가능한 것으로 나타났다. 이를 바탕으로 실물 크기의 60m 거더를 제작해 4점 실험을 실시한 결과 초기 균열이 사용하중의 2배 이상에서 발생하여 실험 결과를 통해서도 충분한 구조적 성능을 입증하였다. 본 연구를 통해 개발된 Half-Decked PSC 형식의 60 m 거더는 단면개선과 효율적인 텐던 배치로 경간대비 매우 낮은 형고를 갖게 되었고 구조적인 이점은 물론 경제성이나 시공성 등에서도 큰 장점을 가질 것으로 판단된다. This study focused on development of 60 m span PSC girder considering not only structural performance but also economical efficiency and constructability including from the improvement of cross-section to the tendon profiles in sequence. Bulb-T type cross section was derived from optimization and actual possibilities to design a bridge were assessed through cross section evaluation. Tendons were also arranged efficiently so that the girder could resist the service load effectively. After developed girder was applied to a sample bridge result of finite element analysis proved all load steps were satisfied with the allowable stress. Furthermore it seemed that sufficient redundancy will be available to design a bridge safely. Based on these a full-scale 60 m span girder was fabricated and 4 point bending test was performed. An initial crack occurred over twice of the service load in this experiment which establishes adequate structural performance. 60 m span Half-Decked PSC girder developed in this study has a lower height for the given span which resulted from cross section improvement and efficient tendon layout. This girder also has not only the structural advantage but also advantages in economical efficiency and constructability.
